Ziel der Studie: Ermittlung und Vergleich von Konsummotiven bei Personen, die Amphetamin bzw. Methamphetamin konsumieren.
Methoden: Anhand einer Literaturrecherche und einer zus?tzlichen Expertenbefragung wurde ein Fragebogen entwickelt, der 145 Patienten in ambulanter und station?rer Behandlung sowie 247 Personen ?ber das Internet vorgelegt wurde. Diese wiesen in den zur?ckliegenden 12 Monaten einen Amphetaminkonsum auf (N=205) oder konsumierten (teilweise zus?tzlich zu Amphetamin) auch Methamphetamin (N=187).
Ergebnisse: W?hrend freizeit- und genussbezogene Motive bei allen Befragten eine gro?e Rolle spielten, wurden von der Methamphetamingruppe auffallend h?ufig weitere Motive berichtet, etwa der Konsum zur Tagesstrukturierung (39,0%), zur Unterdr?ckung von Albtr?umen (14,4%), zur Selbstbehandlung von St?rungen (36,9%) sowie um Sexualit?t ?berhaupt m?glich zu machen (18,7%).
Schlussfolgerung: Bei Personen, die Methamphetamin konsumieren, scheint im Vergleich zu Amphetamin ein breiteres Spektrum von Konsummotiven eine Rolle zu spielen. Hierbei handelt es sich teilweise um Motive, die mit dem Funktionsniveau im Alltag sowie mit psychischen Belastungen in Zusammenhang zu stehen scheinen. Eine sorgf?ltige Analyse der vielf?ltigen und komplexen Konsummotive bei Stimulanzienkonsumenten k?nnte dazu beitragen, therapeutische und pr?ventive Interventionen optimal auf deren individuelle Bed?rfnisse abzustimmen.

Zielsetzung:
Analyse von Bedingungsfaktoren f?r die Umsetzung von alkoholbezogenen Interventionen in der haus?rztlichen Praxis.
Methode:
Fragebogengest?tzte Befragung von 229 Haus?rzten sowie leitfadengest?tzte Fokusgruppeninterviews mit 29 Haus?rzten. Die Fokusgruppenteilnehmer wurden aus der teilnehmenden Grundgesamtheit randomisiert.
Ergebnisse:
Standardisierte Methoden zur Fr?herkennung von Patienten mit Alkoholproblemen kommen in der haus?rztlichen Praxis kaum zur Anwendung. Die Umsetzung von alkoholbezogenen Ma?nahmen korreliert mit den bisherigen Erfahrungen des Arztes sowie der patientenseitigen Motivation. Als strukturelle Barrieren werden fehlende zeitliche/finanzielle Ressourcen, fehlende Eigenqualifikation und die mangelnde Kooperation mit dem Suchthilfesystem angegeben. Vorhandene Fortbildungsangebote werden anscheinend nur unzureichend wahrgenommen.

Diese Studie zeigt die Komplexit?t der patientenseitigen, arztbezogenen und strukturellen Gr?nde, die eine fl?chendeckende Umsetzung von alkoholbezogenen Interventionen in der haus?rztlichen Praxis verhindern. Modelle zur Verbesserung der Versorgungssituation von Menschen mit Alkoholproblemen in der haus?rztlichen Praxis m?ssen die Bedarfe an Qualifikation, Vernetzung und Finanzierung ber?cksichtigen.
