El fenómeno del desplazamiento afecta a toda la población colombiana, especialmente a aquella que tiene mayor vulnerabilidad, como las mujeres, que ven afectada su salud por el cambio de roles que deben asumir. Objetivo: analizar cómo los roles que desempeñan las mujeres en situación de desplazamiento influyen en sus condiciones de vulnerabilidad psicológica y social. Materiales y métodos: se realizó una investigación cualitativa utilizando la teoría fundada; se tuvieron en cuenta fuentes primarias, constituidas por los discursos recogidos en entrevistas aplicadas personalmente a mujeres que hubieran sido desplazadas, residentes en tres asentamientos de la ciudad de Medellín y cuyas características se definieron por la diversidad cultural, étnica e idiosincrásica, y fuentes secundarias, compuestas por la revisión de documentos oficiales, literatura, otras investigaciones y demás material que tratara acerca de este tema. Se realizaron quince entrevistas semiestructuradas, se organizaron seis grupos focales y se realizaron seis entrevistas a profundidad. Para el análisis se utilizó la codificación, categorización e interpretación. Resultados: las mujeres desplazadas se ven sometidas a cambios significativos en sus roles luego de la llegada a la ciudad, los cuales agudizan sus condiciones de vulnerabilidad e influyen en su percepción de salud. La relación entre los múltiples roles y su efecto sobre las condiciones de vulnerabilidad no puede estudiarse analizando cada rol independientemente y sin considerar diferentes dimensiones de funcionamiento y adaptación de la mujer. Conclusión: la experiencia del desplazamiento afecta los roles asumidos por las mujeres, y estos a su vez las condiciones de vulnerabilidad y su percepción de salud.
Objective: to recognize the experiences derived from the search for healthcare alternatives performed by women who have been displaced by violence and currently live in three settlements in Medellin, Colombia, during 2013. Methodology: a qualitative research was conducted using grounded theory. Two types of sources were taken into account: primary sources were the discourses collected via interviews with women who have been displaced by violence and who lived in one of three settlements in Medellin at the time of this study. These interviews were applied personally and the women were culturally, ethnically and idiosyncratically diverse. The secondary sources were obtained by reviewing official documents, literature, other studies and additional material on this issue. Data were collected through semistructured
263 ResumenDesde un enfoque de determinantes sociales, se ha demostrado la relación existente entre las condiciones de vida estructurales y la situación de salud o enfermedad de poblaciones específicas. Esta relación tiende a complejizarse cuando se viven situaciones de conflicto, las cuales agudizan la vulnerabilidad. El objetivo fue describir las vivencias de mujeres en condición de desplazamiento y sus efectos en los procesos de salud-enfermedad. Con un enfoque cualitativo, se tomaron aspectos de la teoría fundamentada de Strauss y Corbin. Participaron 49 mujeres en situación de desplazamiento. Para el análisis se utilizó la codificación, la categorización y la interpretación, y finalmente se llevó a cabo la teorización. Se consideró la descripción conceptual de percepción, significado, la salud-enfermedad, la salud pública, el género y el desplazamiento. Como hallazgo principal se encontró que el desplazamiento y las representaciones socioculturales marcan considerablemente la significación y la forma de asumir los procesos de salud-enfermedad de las mujeres.Palavras-chave: migración; salud; enfermedad; salud de la mujer; rol; actividades cotidianas; determinantes sociales de la salud AbstractThe relationship between structural life conditions and the health status of specific populations has been proven through a social determinants approach. Moreover, this relationship tends to become more complex with the appearance of conflicts that accentuate vulnerability. The aim of this study was to describe the life experiences of women displaced by violence and the effects of displacement on health/disease processes. A qualitative approach with elements from Strauss' and Corbin's Grounded Theory was used in the study, which included a total of 49 displaced women. Data were analyzed via coding, categorization and interpretation before theorizing. In addition, the conceptual description of perception, meaning, health/disease, public health, gender and displacement was taken into account. The objective of this study is to describe the experience of displaced women in Medellín, Colombia, and how this circumstance has an impact on the roles and perceptions of health/illness. This was a qualitative study using symbolic interactionism. Instruments from the Grounded Theory methodology were used. The study population was composed of adult women who have been displaced for at least 1 year and who live in 3 settlements in the city of Medellín. The instruments for data collection were 15 semi-structured interviews, 6 focus groups, and 6 in-depth interviews. Coding, categorization and interpretation, and, lastly, theorizing were used for analysis. What women go through displacement affects the roles they play both in relation to their family - one of the most significant components of their lives -, and in the social/community and health/illness processes, turning them into people with high degrees of vulnerability and social marginalization. Women who have been displaced face multiple obstacles, losses, and transformations, leading to a decline in their physical and mental health.
Introducción: el uso del tabaco en mujeres está aumentando significativamente a pesar de los alarmantes peligros para la salud. Objetivo: determinar la prevalencia y factores asociados al consumo de tabaco en mujeres del departamento de Antioquia, Colombia. Materiales y métodos: estudio descriptivo transversal, con una muestra representativa de mujeres mayores de 18 años en las nueve subregiones del departamento, se utilizó un muestreo probabilístico multietápico, estratificado por región. Resultados: de 4377 mujeres, 10,8% eran fumadoras, 13,1% exfumadoras, y 75,9% nunca habían fumado. Variables relacionadas con mayor reporte de tabaquismo: tener más de 45 años (OR=3,2, IC 2,6-3,9); no tener educación o primaria incompleta (OR=2,2, IC 1,5-3,1), secundaria incompleta (OR=1,5, IC 1,1-2,2), consumir licor (OR=2,3, IC 1,9-2,7), no consumir verduras (OR=1,4, IC 1,0- 2,0), no consumir frutas (OR=1,6, IC 1,2-2,1). Variables relacionadas con menor reporte de tabaquismo: estar casada (OR=0,6, IC 0,5-0,8), y vivir en algunas de las subregiones. Conclusiones: se evidencia la importancia de programas enfocados a prevenir el consumo de productos nuevos como el cigarrillo electrónico, especialmente en población joven
