Aortic dissection involves a break in the intimae of the vessel, usually preceded by degeneration or cystic necrosis of the middle layer. The echocardiographic evaluation of the aorta is often part of the standard assessment in this pathology. Transesophageal echocardiography provides morphological information of hemocinetic interest in evaluating the dissection. The report and description of a case of aortic dissection associated with severe aortic regurgitation in a young patient, who had a Bentall surgery plus an an brachiocephalic aorto bypass followed by the implanting of stents in descending aorta is performed. After the comprehensive evaluation of the patient, the diagnostic revealed a dissecting aortic Bakendy Stanford A-I aneurysm. Due to the complexity of the case, an emergency surgery was suggested as part of the treatment: Surgery of Bentall of Bono, plus an aorto -brachiocephalic trunk bypass. Subsequently, percutaneous treatment (stent implantation in the descending aorta) with successful results was performed. Echocardiography can provide information of the thoracic aorta in virtually all of it. In our patient, the surgical and percutaneous treatments were complemented properly. La evaluación ecocardiográfica de la aorta es parte habitual de la valoración estándar en esta patología. La ecocardiografía transesofágica aporta información morfológica y hemocinética de interés en la evaluación de la disección. Se realiza el reporte y descripción de un caso de disección de aorta asociado a regurgitación aortica severa en un paciente joven, a quien se le realizó en primera instancia una cirugía de Bentall más By pass aorto braquiocefálico y posteriormente el implante de endoprótesis en aorta descendente. Luego de la evaluación integral del paciente se tuvo como diagnóstico Aneurisma disecante de aorta Standford A-Bakendy I, debido a la complejidad del caso se planteó como parte del tratamiento Cirugía de emergencia: Cirugía de Bentall de Bono más bypass aorto -tronco braquiocefálico. Posteriormente se realizó el tratamiento percutáneo ( implante de endoprótesis en aorta descendente) con resultados exitoso. La Ecocardiografia puede aportar información de la aorta torácica en prácticamente toda su extensión. En nuestro paciente el tratamiento quirúrgico y percutáneo se complementaron en forma adecuada.
