Three exclusively breastfed term neonates were admitted with lethargy, poor feeding, and oligoanuria. All three babies were severely dehydrated and had a weight loss ranging from 18% to 40%. Serum sodium of more than 180 mEq/l and renal failure were observed in all three. Two had very high creatinine levels of 9.5 mg/dl and 6.7 mg/dl. Both these babies also had multiple seizures. One baby required mechanical ventilation. All three babies showed markedly hyperechoic renal medullary pyramids with speckled foci suggestive of crystal deposition that reversed completely on therapy. Urine showed abundant urate crystals in two and an elevated calcium/creatinine ratio of 1.6 in one. There was no evidence of distal renal tubular acidosis, Bartter syndrome, or high serum calcium. Supersaturation of the ions in a markedly hypertonic renal medulla may have led to crystallization, with resolubilization with hydration and restoration of good urine output. The hypernatremic dehydration was primarily due to lactation failure leading to inadequate fluid intake in the face of ongoing insensible losses. High breast milk sodium may have been a contributory factor in one patient.
Introduction: Ochronosis arthropathy (OcA) is a rare condition which may be treated with total knee arthroplasty (TKA) at the end stage. The condition is often discovered only intraoperatively and the ideal choice of TKA is unknown. Methodology: In the present study we studied 5 patients with worsening chronic bilateral mechanical knee pain had failed conservative therapy. Posterior stabilised (PS), cemented TKA and patella resurfacing was performed. Intraoperatively, collagenous structures such as the menisci and cartilage were noted to be black. Histological examination showed deposition of large amorphous brown material suggestive of ochronosis. He recovered well and underwent TKA of the contralateral knee the following year. At 2 years post index TKA, his outcome scores improved and he was satisfied. Discussion and Conclusion: With increasing TKA performed worldwide, a surgeon may eventually be surprised by the above findings once in their lifetime. However, as the pathogenesis of OcA appears to be inflammatory in nature, we suggest using cemented PS TKA with resurfacing of the patella.
PURPOSE: It is to prove the validness of MRI in diagnosing the internal structural pathology in IDK with confidence compared to gold standard arthroscopy. MATERIALS AND METHODS: Ours is prospective study. We took arthroscopy as gold standard and took systematic review of MRI and arthroscopy in diagnosing IDK in 54 pts. RESULTS: MRI is 100% sensitive in diagnosing ACL PCL, meniscal injuries and hence the best screening tool. But many of grade 1/2 tears of meniscal tears in MRI are normal at arthroscopy. Majority of false positives occur in posterior horn of lateral meniscus.
