Antecedentes: La displasia fibrosa y el fibroma osificante son patologías benignas que generalmente no presentan sintomatología dolorosa, son poco frecuentes y afectan el tejido óseo. Producen alteraciones estéticas y funcionales, motivo por el cual consultan los pacientes. Objetivo: Describir los desórdenes celulares característicos de la displasia fibrosa y el fibroma osificante, así como su relación con las formas clínicas y otras variables histopatológicas que constituyen un reto para el patólogo oral y el clínico. Descripción de los casos: Se presentan dos casos clínicos de pacientes adultos, hombre y mujer, que acudieron a la Unidad Estomatológica y Cirugía Oral de la Universidad de Cartagena en Colombia, por presentar asimetrías faciales, asintomáticas y unilaterales que comprometían la estética de los pacientes. Se describen sus características clínicas, radiográficas e histológicas. Conclusión: La displasia fibrosa y el fibroma osificante son alteraciones óseas benignas similares que requieren diagnóstico diferencial a fin de establecer tratamientos idóneo y específicos.
Burkitt's lymphoma, a form of non-Hodgkin lymphoma, is a neoplastic monoclonal proliferation of lymphoid cells in areas of the immune system. It can occur in HIV-positive patients, as AIDS is related to the development of non-Hodgkin lymphoma. Burkitt's lymphoma is a rare subtype, highly prevalent in patients with AIDS. Incisional biopsy, in situ hybridization and computerized axial tomography are the appropriate tests to determine the characterize of the lesions. The case of a 4-year-old HIV-positive patient, who developed Burkitt's lymphoma of the oral cavity, is reported in this paper. The aim of this case report is to describe the course of the pathology, taking into account its clinical imaging characteristics and treatment.
Inferior alveolar nerve block in the mandibular technique is the blockade that presents most failures in the dental practice, so this technique is considered the less effective of those used in Dentistry and the most frustrating to the dentist. This study aimed to determine pain intensity during dental procedures after using 4% articaine with epinephrine 1:100000 in the inferior alveolar nerve block with buccal infiltration in patients who were attended in the dental clinic of the University of Cartagena, Colombia. A clinical randomized trial was performed. The sample was of patients who required dental procedures in the jaw with local anesthesia. After standardization of the anesthetic technique, and use of 4% articaine with epinephrine 1:100000, the inferior alveolar nerve was blocked with 0.9 mL (half cartridge) followed by buccal infiltration with 0.9 mL in the first mandibular molar. Fifty patients (30 men - 60% and 20 women - 40%), with an average age of 25.3 years old (mean ± standard deviation [SD]: 25.3 ± 6.6) were included in the study. When assessing the pain puncture and during the injection, 92% of patients classified it as mild pain according to VAS. When evaluating the latency period, the average time was less than two minutes and the perioral soft tissue anesthesia was 97%. Only a small percentage of patients required complementary anesthesia. The authors recommend the use of 4% of articaine with epinephrine in mandibular procedures that require deep pulp anesthesia, using this with a buccal infiltration.
Dental transplantation is a surgical alternative, which consists of transplanting a tooth from one alveolus to another in the same patient, very useful when replacing lost dental organs in young patients, with a high success rate. Present advantages helpful for patients providing a vital, proprioceptive and excellent aesthetic rehabilitation, without altering adjacent teeth, additionally represents an alternative cheaper. The purpose of this case report is to show clinical sequence about surgical approach for dental autotransplantation from lower third molar on alveolus after extraction of right lower first molar, carried out without endodontic treatment. Female patient case of 15 years old with a diagnosis of extensive caries not restorable in 46 is presented. It is decided through an interdisciplinary board to carry out, treatment with an autologous transplant of right lower third molar in socket post-extraction of first lower molar from the same hemiarchate, post-surgical controls are performed for up to one year. Dental transplantation is an excellent treatment option when it comes to replacing missing teeth, since it offers multiple functional and aesthetic advantages, mainly in children and young adults.
Muchos trastornos sistémicos como la artritis o la osteoporosis son patologías responsables de las alteraciones crónicas de la articulación temporomandibular, creando así un problema a largo plazo que afectan la calidad de vida de aquellas personas que las padecen. Actualmente no existe tratamiento curativo para dichas patologías, aunque sí de tipo paliativo, que en muchas ocasiones tienen efectos secundarios en el tiempo o una limitación en su efectividad y eficacia, por lo que se hace necesario buscar alternativas con la implementación de terapias regenerativas para el tratamiento de aquellas enfermedades que afectan el sistema musculoesquelético. En muchos estudios se discute sobre el papel fundamental que cumple el zinc y el estroncio en la génesis de tejido tanto cartilaginoso como óseo, así como la relevancia de incorporarlos con diversos biomateriales en procedimientos de regeneración, sin embargo, este tema no es claro aún y requieren más atención por parte del clínico. El objetivo de este artículo es determinar la función cumplen el zinc y el estroncio en los procesos de regeneración del hueso y el cartílago desde una visión molecular y celular aplicada a la práctica clínica, con el fin de obtener nuevas alternativas en el tratamiento de diversas patologías y alteraciones musculoesqueléticas que mejoren las condiciones de salud oral de los pacientes, además de, contar con la revisión que nos aproxime a comprender los mecanismos de acción de diferentes moléculas que incorporadas a los biomateriales compatibles con el tejido duro y blando mejoren las condiciones biológicas para la regeneración.
Ameloblastic fibro-odontoma (AFO) is a mixed odontogenic tumor, which has an epithelial and mesenchymal component. It can be observed in imaging tests as a radiomixed lesion, with some authors claiming it represents only the predecessor of an odontoma. Epidemiologically, it appears between the second and third decade of life, predominantly in males and with a predilection for the posterior area of the mandible. This lesion presents a good prognosis and it responds well to enucleation by curettage with a low rate of recurrence. In the present article, two cases of ameloblastic fibro-odontoma in an unusual region of the maxilla are presented.
El penfigoide de la membrana mucosa es una enfermedad autoinmune, que afecta a los diferentes estratos de la mucosa y la piel, y se caracteriza por lesiones vesiculo-ampollosas polimórficas. Presentando combinaciones variables de lesiones orales, oculares, cutáneas, genitales, nasofaríngeas, esofágicas y laríngeas. Su relación con las neoplasias malignas es poco conocida y sigue siendo controvertida. El presente artículo describe el manejo multidisciplinario de una paciente de 46 años de edad que desarrolló un penfigoide de membrana mucosa a nivel de la cavidad oral. Esta patología estuvo asociada a un carcinoma de pulmón, y posteriormente presentó alteraciones multiorgánicas. El tratamiento fue realizado por las especialidades de dermatología, medicina interna, cirugía general, oftalmología, estomatología, cirugía oral y maxilofacial, dicho tratamiento se llevó a cabo por etapas y tuvo una duración de 2 meses aproximadamente, logrando la resolución de las lesiones que presentaba la paciente. Consideramos que se abre una puerta para futuras investigaciones que confirman la relación entre el penfigoide de la membrana mucosa y el carcinoma de pulmón.
La hiperplasia angiolinfoide con eosinofilia (HACE) es una enfermedad rara de origen desconocido, de carácter inflamatorio, que puede ser asintomática o presentarse con prurito, dolor o sangrado espontáneo, sin predilección por género. Las opciones de tratamiento son la criocirugía, el láser de colorante pulsado, el tacrolimus tópico, la cirugía, el curetaje y los retinoides orales. Se reporta un caso clínico de hiperplasia angiolinfoide con eosinofilia en cavidad oral, así como el tratamiento realizado. Paciente de sexo femenino, que acudió a consulta de estomatología y cirugía oral por presentar lesión localizada a nivel de vestíbulo entre órganos dentarios 11 y 12. Clínicamente se observó lesión nodular localizada en encía adherida de aproximadamente 1 cm de diámetro, asintomático a la palpación y de etiología desconocida. La hiperplasia angiolinfoide es una patología inusual en cavidad oral, de etiología desconocida, en la que el diagnóstico definitivo debe hacerse a través del examen histopatológico y los signos clínicos.
