Macrodactyly is a rare congenital deformity of the extremities, and with clinodactyly is even rarer. Both conditions may also be associated with a lipofibromatous hamartoma of the median nerve. We report an adult woman with clinodactyly and macrodactyly of the fourth and fifth fingers of her left hand associated with a carpal tunnel syndrome.
Springback is a problem as important as tearing or thinning, while forming high-strength sheets. Springback is an undesirable situation and it is the form difference between the desired form of a part in theory and the form obtained due to mechanical characteristics and process inputs of the material after die forming. It affects operations in shearing, punching or bending dies in subsequent operations in forming die sets. If the part is not within the desired tolerance range, it creates problems during assembly. In order for cost effective production plans for automotive parts to be made, suitable sheet forming simulations are needed. Waste of time and failures during die construction are minimized by defining accurate parameters by finite element analyses and minimizing periods of trial-and-error. In this study, the material SCGA1180DUB in sheet thickness of 0.8 mm from multi-phase steel sheet group was U-channeled, using Autoform sheet forming analysis program, according to appropriate process conditions having wall angles of 7°,10°,12° and die radius values of R3, R5, R8 and the springback values were estimated. The results obtained were compared through the finite element program and suitable wall angle and die radius values for the material SCGA1180DUB for forming advanced high-strength sheets were determined. As the die radii increased at the same wall angles, the amount of spring back increased significantly. In particular, due to high yield and tensile strength of multi-phase high strength sheet, springback values were observed to be high. Negative springbacks were observed in the roof of the part. In the same die radii, under the same process conditions, as wall angles increase, springback values decreased. In the literature, it is interesting that there are few studies regarding forming, springback of high-strength sheets SCGA1180DUB. This study will contribute to the literature. Autoform program was used for Finite Element Analysis.
Neglected hip dislocation is a rare situation among children. The coexistence of acetabulum fracture is less common. The result of treatment depends on the time of diagnosis. Early treatment is mandatory in such cases. Seven years old female brought to our outpatient clinic with the complaints of right hip pain and walking difficulty. Her parents said that she had fallen from a horse one month ago. X-ray and CT showed a posterior dislocation of her right hip and a fracture on her right posterior acetabular lip. We performed an open reduction via posterior approach. Because the hip was unstable we fixed the femoral head to the acetabulum by a Kirshner wire. Pelvipedal cast was applied after the operation for 6 weeks. After 6 months avascular necrosis (AVN) developed on her right femoral head, the range of motion (ROM) of the hip was restricted and she had minimal pain on her right hip. Hastaya posterior yaklaşımlı açık redüksiyon yapıldı. Bir adet kirşner teli ile femur başı asetabuluma tespit edildi. Ameliyat sonrası 6 hafta süreyle pelvipedal alçı uygulandı. Hastanın 6 aylık takibinde femur başında avasküler nekroz (AVN) geliştiği, kalça hareketlerinde kısıtlılık olduğu, hastanın hafif derecede kalça ağrısının olduğu görüldü. Despite the screening programs for newborn children with hip ultrasonography, neglected developmental dysplasia of the hip (DDH) is still continuing to be a problem in the east and southeast parts of our country. The main complications are redislocation, avascular necrosis, and joint stiffness. We present an unusual complication, femoral neck fracture during passive motion under general anesthesia, of a six-year-old girl with neglected DDH treated by open reduction and Pemberton osteotomy without femoral shortening. The fracture was treated by open reduction and internal fixation combined with proximal femoral shortening. After 5 years the patient had excellent clinical results, no avascular necrosis was seen, and the radiologic appearance was type IA according to modified Severin classification. In conclusion older children with neglected DDH are more likely to have joint stiffness after open reduction. If there is even a little doubt about joint stiffness after open reduction, one should not refrain from femoral shortening. Also passive motion under general anesthesia should be applied very carefully with fluoroscopic control.
