SUMMARY
The clinical findings in 50 children with intermediate or mild spinal muscular atrophy are reviewed.
An early age of onset and the presence of tongue fasciculation carry a poor prognosis for disability. Scoliosis appears early in nearly all children with the more severe forms of spinal muscular atrophy and early energetic treatment is indicated. Contractures are also found in both the more severe and the milder forms and are the main cause of progressive disability, since continuous deterioration of muscle power was not evident in the present series. Respiratory failure is rare but potentially fatal, and may occur even in mild forms of the disease. Clinically evident weakness of the axial musculature may give warning of this complication.

L'amyotrophie spinale infantile: revue de 50 cas
Les données cliniques chez 50 enfants présentant une amyotrophie spinale infantile d'intensité modérée ou légère sont analysées.
Un début précoce et l'existence de fasciculations linguales sont de mauvais pronostic fonctionnel. La scoliose apparait précocément chez presque tous les enfants atteints de forme sévère d'amyotrophie spinale infantile et un traitement énergique précoce est alors indiqué. Les rétractions sont observées aussi bien dans les formes plus sévères ou plus légères et constituent la cause principale de l'incapacité progressive. La détérioration régulière de la force musculaire n'est pas évidente dans la série étudiée. La déficience respiratoire est rare mais fatale à terme et peut survenir même dans les formes légères d'amyotrophie spinale infantile. La faiblesse cliniquement évidente de la musculature axiale peut annoncer une telle complication.

Spinale Muskelatrophien im Kindesalter: ein Überblick über 50 Fälle
Über die klinischen Befunde von 50 Kindern mit einer intermediären oder milden Form einer spinalen Muskelatrophie wird ein zusammenfassender Bericht gegeben.
Ein frühzeitiger Beginn der Erkrankung und das Auftreten von Faszikulationen der Zunge führen zu einer ausgeprägten Körperbehinderung. Bei nahezu allen Kindern mit einer schweren Form von spinaler Muskelatrophie tritt frühzeitig eine Skoliose auf, die einer raschen Behandlung bedarf. Kontrakturen treten sowohl bei der schweren als auch bei der leichten Form auf. Sie sind die Hauptursache der fortschreitenden körperlichen Behinderung. Ein zunehmendes Nachlassen der Muskelkraft liess sich bei den Kindern nicht nachweisen. Ein Atemversagen ist selten, jedoch verhängnisvoll und kann sogar bei den milderen Formen von spinalen Muskelatrophie auftreten. Der klinische Nachweis einer Schwäche der Rumpfmuskulatur kann bedeuten, dass eine solche Komplikation drohend bevorsteht.

The serious prognostic implications of familial dysautonomia (FD) for the affected individual and his family make early definitive diagnosis mandatory. Familial dysautonomia has rarely been diagnosed in the neonatal period in hitherto unaffected families. We describe here three such newborn patients to reinforce the limited data available on this subject. In spite of the variability of expression and the incompleteness of the manifestations of FD in the neonatal period, as well as the presence of a number of "dysautonomic" features in normal newborns, we believe that it is possible to establish a diagnosis of FD neonatally. We pay particular note to the altered state of consciousness and behavior in neonatal FD, the unusual posture and limb movements, and the swallowing disorder with tendency to neonatal aspiration. In addition, the incidental finding of bile pigment in the amniotic fluid of an affected fetus without hemolytic disease may hint at a possible approach to fetal diagnosis of this condition.
