A specific type of social phobia -dictation phobia -was the main and unusual treatment problem in the cognitive-behavioural therapy for an 11-year-old schoolboy. For case formulation and treatment rationale, the social phobia model by Clark and Wells was used. The posttreatment assessment revealed clear positive treatment effects with respect to school grades, social anxiety and overall functioning. The model's applicability for children and adolescents is discussed.
Im Sinne einer selektiven Prävention psychischer Störungen ist es notwendig, Personen zu identifizieren, die aufgrund sozialer oder biologischer Faktoren ein erhöhtes Risiko für die Entwicklung einer psychischen Störung aufweisen. Klinikaufenthalte im Kindesalter werden seit Beginn des 20. Jahrhunderts als mögliche psychosoziale Risikofaktoren diskutiert. In dieser Studie wurde die Auswirkung eines Klinikaufenthaltes auf die soziale Unsicherheit untersucht, einen starken Prädiktor für die spätere Entwicklung von psychischen Störungen, insbesondere von Angststörungen und Depression. Zu zwei Messzeitpunkten wurden bei 42 Kindern zwischen sechs und 13 Jahren die Konstrukte „Soziale Unsicherheit“, „Sozialbeziehungen“ und „Selbstwertgefühl“ im Selbst- und Elternurteil erfasst. Verglichen wurden eine Gruppe gesunder Kinder, chronisch kranker Kinder in ambulanter Behandlung und chronisch kranker Kinder in stationärer Behandlung. Die Ergebnisse zeigen keine negativen bzw. positiven Veränderungen der sozialen Unsicherheit, der Sozialbeziehungen und des Selbstwertgefühls. Die erhobenen Auffälligkeiten decken sich mit den Prävalenzraten aktueller epidemiologischer Studien. Ein Klinikaufenthalt allein stellt nach den Befunden der vorliegenden Untersuchung keinen psychosozialen Risikofaktor (mehr) dar.
Zusammenfassung. Das Hauptziel der vorliegenden explorativen und deskriptiven Studie ist die Erfassung und Analyse der fachspezifischen Leistungsangst von Schülern und Schülerinnen im Sportunterricht sowie möglicher Strategien zu deren Reduktion. Zum Vergleich wurde die fachspezifische Leitungsangst im Fach Mathematik erhoben. Untersucht wurde eine Stichprobe von 63 Schülern und Schülerinnen der 6. und 8. Klasse eines Spezialgymnasiums. Zusätzlich zum Ausmaß der Angst wurde die Lehrer-Schüler-Übereinstimmung erfasst. Die Ergebnisse zeigen eine Besorgnis erregend hohe Anzahl von Kindern mit klinisch relevanten Leistungsängsten (PHOKI, 26 von 63 Befragten). Besonders eindrücklich sind die Befürchtungen der Kinder, bei Leistungskontrollen im Sportunterricht ausgelacht, abgewertet oder mit dem Handy aufgenommen zu werden. Bzgl. der Leistungsstärke zeigte sich eine hohe Übereinstimmung zwischen Selbst- und Fremdurteil (Mathematik: r: .52, Sport: r: .63), während die Übereinstimmung bei der Einschätzung der Aufgeregtheit zwischen Selbst- und Fremdurteil deutlich niedriger ausfiel (Mathematik: r: .26, Sport: r: .29). Es wurden positive Korrelationskoeffizienten zwischen der Variablen „Leistungsangst“ und verschiedenen Dimensionen der „Leistungsmotivation“ gefunden. Zur anwendungsbezogenen und schlussfolgernden Einordnung und Diskussion der Ergebnisse wird das Modell von Clark und Wells (2005) herangezogen und Implikationen für die Praxis aufgezeigt.
Zusammenfassung. Ein traumafokussiertes kognitiv-behaviorales Behandlungsprogramm mit spielerischen Therapiebausteinen wurde für Kinder mit Traumafolgestörungen und ihre Bezugspersonen entwickelt. Untersucht wurde, ob eine Verminderung der posttraumatischen und phobischen Symptomatik sowie weiterer Verhaltensauffälligkeiten erreicht werden konnte. Die Daten von 47 Kindern im Alter von 5 bis 13 Jahren (61.7 % männlich) wurden vor und nach der Therapie sowie sechs Monate im Anschluss erhoben. Nach der Behandlung waren die selbstbeurteilten posttraumatischen und fremdbeurteilten phobischen Symptome, sowie die fremdbeurteilten Auffälligkeiten signifikant verbessert. Die Effektstärken fielen erwartungsgemäß moderat bzw. hoch aus; die Responseraten für die posttraumatischen und phobischen Symptome betrugen 70.4 % bzw. 19.2 %. Alle Kinder wiesen am Behandlungsende eine Remission der posttraumatischen Symptome auf. Das Behandlungsprogramm sollte zukünftig in randomisiert-kontrollierten Studien untersucht werden.
