The "acid mantle" is a topic not only of historical interest, but also of clinical significance and has recently been linked to vital stratum corneum function. Despite compelling basic science evidence placing skin pH as a key factor in barrier homeostasis, stratum corneum integrity, and antimicrobial defense, application of the acid mantle concept in clinical care is lacking. We review recent basic science investigations into skin pH, discuss skin disorders characterized by aberrant pH, and finally discuss practical application for preservation of the acid mantle. Recognizing factors that alter skin pH and selecting products that preserve the acid mantle is of prime importance in treating dermatologic patients.
Background: Sarcoidosis is a systemic disease that may present as tattoo granulomas.Objective: A patient with systemic sarcoidosis who developed a granulomatous reaction within a tattoo is presented to stimulate interest in this unusual phenomenon.Methods and Results: A patient with a 6-year history of pulmonary sarcoidosis developed sarcoidal granulomas restricted to one pigment of a tattoo. Previous reports of sarcoidal granulomas within tattoos are reviewed, and information about the pathogenesis of this process is explored.Conclusion: Sarcoid granulomas may develop in tattoos as an isolated local reaction or as the presenting sign of systemic sarcoidosis. The reaction itself may provide insight into further understanding the pathogenesis of sarcoidosis.Anté cé dents: La sarcoïdose est une maladie systé mique qui peut se pré senter sous forme de granulomes sur un tatouage.Objectif: Stimuler l'inté rê t pour ce phé nomè ne inhabituel en pré sentant le cas d'un patient souffrant d'une sarcoïdose systé mique qui a dé veloppé une ré action granulomateuse au niveau d'un tatouage.Mé thodes et ré sultats: Un patient souffrant depuis 6 ans de sarcoïdose pulmonaire a dé veloppé un granulome sarcoïdosique au niveau d'un seul pigment du tatouage. Un survol des cas rapporté s de granulome sarcoïdosique au niveau d'un tatouage et une exploration de la pathogenè se de ce processus sont effectué s.Conclusion: Le granulome sarcoïdosique peut se dé velopper dans un tatouage comme ré action locale isolé e ou comme signe d'une sarcoïdose systé mique. La ré action peut entraîner une meilleure compré hension de la pathogenè se des sarcoïdoses.
