Issu de la sexologie et conceptualisé par la psychanalyse, le masochisme est au carrefour de plusieurs concepts fondamentaux. Partant de la paradoxalité du masochisme dans la vie psychique, Freud explora les enjeux inconscients de trois grandes formes du masochisme : le masochisme primaire, le masochisme féminin et le masochisme pervers. Chaque forme renvoie à différents aménagements psychiques, à une fixation au stade prégénital ou œdipien du développement psychosexuel. Au fil des décennies, Freud montre comment le masochisme va au-delà du simple plaisir à souffrir. Le masochisme repose sur des fantasmes inconscients dans lequel la passivité ou la soumission à la douleur viennent garantir un équilibre pulsionnel chez le sujet. Nous rencontrons régulièrement le masochisme en structure douleur chronique, expression de l'équilibre psychique entre le patient et sa douleur. Sa compréhension éclaire les freins et les résistances au soulagement. Il s'agit alors d'en considérer les enjeux transférentiels dans la relation thérapeutique en clinique de la douleur.
La survenue de la maladie du cancer vient déséquilibrer le sujet, de par le potentiel morbide dont elle est chargée, et affecter son corps par les atteintes douloureuses qui vont l’accompagner. Les conséquences sur l’économie psychique du sujet sont nombreuses. Être douloureux lorsque l’on a un cancer, c’est être irrémédiablement ramené à l’organe malade et au cancer. Mais c’est aussi être focalisé sur son corps réel sans possibilité de s’en dégager. Il s’en suit une préoccupation en lien avec la douleur, avec la maladie et enfin avec des mouvements hypocondriaques tant l’impossibilité de penser à autre chose est présente. Ainsi, la douleur sera porteuse d’émotions liées à la douleur, à la maladie grave, au pronostic. Paradoxalement à ce qui peut être attendu et imaginé chez les patients, l’après-cancer, c’està- dire la phase de rémission, donne lieu à une somation de plaintes douloureuses dotée d’une grande détresse chez le sujet. En effet, cette période tant attendue est malgré tout appréhendée de façon ambivalente, puisqu’elle s’accompagne souvent d’angoisse diffuse et de douleurs résiduelles ou séquellaires des traitements. De cette expérience intense associée à la découverte du cancer, la douleur restera le curseur à disposition du sujet pour contrôler le possible retour de la maladie. Elle aura aussi pour effet de colorer le vécu ultérieur d’autres douleurs qui seront alors ramenées à cette douleur inaugurale doublement traumatique.
Dans nos consultations douleur, la façon dont se manifeste le masochisme, tels un entretien ou une répétition de la plainte douloureuse, a du sens au regard de l'équilibre psychique. Au travers de cet article, nous verrons dans un premier temps comment le masochisme permet d'intégrer la douleur comme une partie de soi et ainsi garantir le lien du sujet avec sa douleur, comment le masochisme garantit le lien entre le sujet et sa souffrance pour que cette souffrance trouve sa place dans l'économie psychique. Dans certains troubles psychiques, la tentative de liaison du moi et de la douleur peut être mise en échec par l'épuisement du jeu des pulsions ou par une fragilité narcissique préexistants. Dans un second temps sont présentés les différents aménagements masochistes dans plusieurs troubles psychopathologiques : les états limites, les réactions post-traumatiques, chez l'obsessionnel, le mélancolique et l'hypocondriaque. Il convient alors de privilégier une lecture clinique et psychopathologique de ces conduites masochistes qui sont à haut risque de passage à l'acte en influençant la relation thérapeutique.
