Transcatheteric implantation of aortal valve in a woman-patient with prominent chronic obstructive pulmonary disease

В течение 10 лет после аортокоронарного шунтирования (АКШ) 60 % венозных шунтов и 90 % трансплантатов внутренней грудной артерии (ВГА) остаются функционирующими. Частота прогрессирования проксимальных стенозов шунтированных артерий до полных окклюзий составляет 22 % при использовании ВГА как артериального анастомоза и 48 % в среднем после применения аутовенозных трансплантатов. В соответствии с литературными данными, при многофакторном анализе больные, перенесшие АКШ и подвергающиеся последующей коронарной ангиопластике, имеют более высокий риск вмешательства в отношении смертности и послеоперационных осложнений. Если реканализация артерии прошла успешно, смертность достигала 2,6 %, в случае частичного успеха – 5,2 %, а в случае неудачи летальность составила 8,2 %. При вмешательстве у больных после АКШ предпочтительной является реканализация окклюзий нативных коронарных артерий, нежели аорто-коронарных трансплантатов. В настоящей работе представлен анализ возможных интервенционных подходов к реканализации коронарного русла у больных после АКШ, обсужден собственный опыт и практические наработки. Реканализации хронических окклюзий коронарных артерий в различные сроки после АКШ с общей технической эффективностью 75 % подверглись 52 больных.

Цель. Изучение динамики размеров аневризматического мешка после эндопротезирования брюшного отдела аорты и оценка ее влияния на последующее клиническое течение и необходимость повторных эндоваскулярных/хирургических вмешательств.Материалы и методы. В исследование включены 140 последовательных пациентов с аневризмой абдоминального отдела аорты (ААА), которым выполнено эндопротезирование аорты (EVAR). Субанализ проведен у 74 пациентов после EVAR, у которых были доступны результаты долгосрочного наблюдения с применением серии МСКТ в течение 5 лет. I группу составили пациенты, у которых диаметр аневризматического мешка сократился на >10% от исходного дооперационного показателя (n=36) в течение 6 месяцев после EVAR. Во II группу вошли пациенты, у которых не фиксировалось послеоперационное уменьшение размеров мешка или степень его уменьшения была <10% от исходного (n=38).Пациентам в период 3–6 месяцев, 2, 3, 5 лет выполнены серии МСКТ, и далее наблюдение проводилось индивидуально.Результаты. Отмечено, что в I группе наблюдения размер мешка при повторной МСКТ уже через 6 месяцев был 4,6+0,64 см (–0,92 см (–16,7%)), тогда как во II группе изменение размера аневризматического мешка в этот период составило +0,04 см. Такая динамика в группах сохранялась, и уже ко 2–3-му послеоперационному году размер мешка в I группе составил 4,2+0,83 см (–1,32 см (–24%)). К 5-му году наблюдения размер аневризматического мешка в I группе составил 3,92+0,76 см (–1,6 см (–29%)).В отдаленном периоде наблюдения не отмечено связанных с аневризмой негативных событий. Аналогичные показатели во II группе составили 5,6+0,62 и 5,67+0,58 см через 2–3 года и 5 лет наблюдения (–1% и 0%) соответственно. Необходимость в повторных вмешательствах в зоне имплантированного стента составила 13,2%.Выводы. После эндопротезирования БЧА отмечается динамика размеров аневризматического мешка с уменьшением его размера в первые 5 лет, с последующей тенденцией к росту мешка в период 6–10 лет. На основании данных литературы и результатов собственных исследований очевидно, что степень и скорость сокращения аневризматического мешка после EVAR может служить маркером долгосрочных результатов эндопротезирования. Purpose. To study the dynamics of the size of the aneurysmal sac after EVAR and to assess its impact on the subsequent clinical course and the need for repeated endovascular/ surgical interventions.Materials and methods. The study included 140 consecutive AAA patients who underwent EVAR. Subanalysis was performed in 74 patients after EVAR, where the results of long-term follow-up were available using a series of MSCT for 5 years. The main (group I) consisted of patients where the diameter of the aneurysmal sac decreased by >10% from the initial preoperative within 6 months after EVAR (n=36). The second group of patients included patients where there was no postoperative decrease in the size of the sac or the degree of its decrease was <10% of the initial (n=38).The complex of preoperative studies included general clinical methods, as well as special studies – MSCT, ultrasound, in some cases, angiography and coronary angiography. After the operation, all patients underwent a series of MSCT in the period of 3–6 months, 2, 3, 5 years, and then the observation was carried out individually.Results. Tracing the state of the aneurysmal sac in the analyzed groups separately, we noted that in the I st observation group the size of the sac after 6 months was 4.6+0.64 cm (–0.92 cm (–16.7%), then, as in group II, the change in the size of the aneurysmal sac during this period was (+0.04 cm). The noted dynamics in the groups remained and already by 2–3 postoperative year the size of the sac in group 1 was 4.2+0.83 cm (–1.32 cm (–24%)). By the 5th year of observation, the size of the aneurysmal sac in group I was 3.92+0.76 cm (–1.6 cm (–29%)). No negative events associated with aneurysm. Similar indicators in group II were 5.6+0.62 and 5.67+0.58 cm after 2–3 years and 5 years of follow-up (–1% and 0%), respectively. The interventions in the area of the implanted stent accounted for 13.2%.Conclusions. After EVAR, the dynamics of the size of the aneurysmal sac after EVAR is noted with a decrease in its size in the first 5 years, followed by a tendency to the growth of the sac in the period of 6–10 years. Based on the literature data and the results of our own studies, it is obvious that the degree of shrinkage of the aneurysmal sac in the EVAR patients can serve as a marker of long-term results and rate of repeated interventions.
