Відеоасистовані резекції частки легеніДУ «Національний інститут фтизіатрії і пульмонології імені Ф.Г. Яновського НАМН України», м. Київ Відеоасистовані (VATS) лобектомії виконуються з використанням ендоскопічних інструментів на основі степлер-техніки та загальнохірургічних елементів. Пріоритет надається виконанню типових резекцій з роздільною обробкою елементів кореня частки (артерій, вен, бронха).Матеріали і методи. За останні 5 років (з 2014 по 2018) у відділенні торакальної хірургії і інвазивних методів діагностики ДУ «Національний інститут фтизіатрії і пульмонології імені Ф. Г. Яновського НАМН України» було виконано 36 VATS-резекцій частки легені. Усі 36 (100,0%) VATS-резекцій проводилися при використанні бронхіальної інтубації контрлатеральної легені. Мініторакотомний розріз був не більше 8 см. Обробка елементів кореня частки виконувалась із застосуванням ендоскопічних зшивальних апаратів на основі степлер-техніки, інтраопераційно виконувалася механічна френікотрипсія.Результати. Інтраопераційні ускладнення були діагностовані у 2 (5,6%) пацієнтів. У 1 (2,8%) хворого була проведена конверсія в широку торакотомію із зупинкою кровотечі. Загальний рівень післяопераційних ускладнень склав 5 (13,9%) спостережень. Середня тривалість резекційних оперативних втручань становила 188,4±73,6 хв, а середня інтраопераційна крововтрата -86,1±154,1 мл. Середня тривалість лікування у післяопераційному періоді склала 29,2±14,1 дня.Висновки. VATS-резекції частки легені є малотравматичними і перспективними методами лікування, що забезпечують анатомічне препарування елементів кореня частки легені. Їх можна розглядати як альтернативу відкритій лобектомії. Широкому використанню даного методу перешкоджає виразний фіброз кореня легені і злуковий процес внаслідок тривалого запального процесу і медикаментозного лікування, а також вартість ендоскопічних інструментів.
THORACIC SURGICAL TREATMENT IN PATIENTS WITH CORONAVIRUS INFECTION M. S. Opanasenko, B. M. Konik, S. M. Belokon, O. V. Tereshkovich, S. M. Shalagai, L. I. Levanda, M. I. Kalinichenko, V. I. Lysenko, M. U. Shamrai, A. M. Stepanyuk, O. D. Shestakova Abstract Aim. To familiarize physicians of different specialties with thoracic pathology in patients with coronavirus infection (COVID-19). Materials and methods. Since the beginning of the COVID-19 pandemic 63 patients were treated at the department of surgical treatment of tuberculosis and NLD, complicated by purulent-septic infections: 47 (74.6%) — with various complications of coronavirus infection, and 16 (25.4%) — COVID-19 and conditions, requiring thoracic surgery, not associated with COVID-19. Results. Pleural empyema was the most common bacterial complication — 18 (28.5%) cases. Only 6 (9.5%) patients were diagnosed with broncho-pleural fistula, while 12 (19.6%) patients already had a functioning broncho-pleural fistula at the stage of hospitalization. In 15 (23.8%) patients, the destructive cavities remained on admission, and only 3 (4.7%) had isolated pleural empyema without destruction of pulmonary parenchyma (complete scarring of abscesses). 17 (26.9%) patients with empyema underwent videothoracoscopic (VATS) drainage of the pleural cavity with polydrainage and the use of long-term active aspiration in the postoperative period. In 1 (1.5%) case, due to the extremely severe condition of the patient, only drainage of both pleural cavities was performed. 1 (1.5%) patient with bilateral pleural empyema died of progressive respiratory and cardiovascular failure. Nonspecific exudative pleurisy was diagnosed in 8 (12.6%) patients after coronavirus infection. Spontaneous pneumothorax without development of pleural empyema was diagnosed in 7 (11.1%) patients and in 3 (4.7%) cases pneumo hemothorax occurred. in 2 (3.1%) cases there was a need for parietal pleurectomy (there was a significant area of detachment of the visceral pleura and the impossibility of imposing intracorporeal sutures). All patients were discharged from the hospital with recovery. 6 (9,5 %) patients with necrotizing pneumonia comprised a challenging group of patients with large, treatment-resistant cavities. In 4 (6.3%) cases antibacterial therapy was ineffective, so transthoracic cavity drainage was performed. All 6 patients underwent radical resection interventions following long pre-operative period: 3 (4.7%) cases - pleurolobectomy, 2 (3.1%) — sublobar resection and 1 (1.5%) - resection of the 6th segment of right lung). Conclusions. Pulmonary purulent-destructive COVID-19 complications may occur much more rarely if timely treatment was used. The aim — to conduct a retrospective analysis of the connective tissue diseases diagnosis in 47 patients of different ages with respiratory manifestations.Materials and methods. A clinical analysis of the first diagnosed cases of connective tissue diseases in 47 patients treated on the basis of the department of thoracic surgery and invasive diagnostic methods, from 2010 to 2018 was done. Among patients, women predominated — 33 (70.2 %), men — 14 (29.8 %). Among them, at the age of 20 — 29 years old, there were 8 (16.8 %) patients; 30 — 39 years — 14 (29.8 %) patients; 40 — 49 years old — 12 (25.5 %); 50 — 59 years old — 9 (19.5 %); over the age of 60 years‑ 4 (8.4 %). The patients were hospitalized with the following diagnoses: disseminated pulmonary disease — 10 (21.3 %), disseminated pulmonary disease complicated by unilateral pleurisy — 12 (25.5 %) patients; disseminated pulmonary disease complicated by bilateral pleurisy — 1 (2.1 %) case; disseminated pulmonary disease complicated by unilateral pleurisy and intrathoracic lymphadenopathy — 2 (4.3 %) observations; rounded lung lesion — 1 (2.1 %) case; unilateral pleurisy— 4 (8.4 %); bilateral pleurisy — 6 (12.9 %); pleurisy with pericarditis — 11 (23.4 %) cases.Results and discussion. In all cases, the specificity of the autoimmune lesion was identified. The attention is paid to the need for early diagnosis of this pathology and the adequate pathogenic treatment appointment by profile specialists.Conclusions. Of newly diagnosed patients with total respiratory, cardiovascular and renal impairment without any history of chronic disease should be considered as a risk group for further diagnosis of connective tissue diseases. The use of video‑assisted thoracoscopic surgery diagnostic methods with biopsy of affected tissues should be the standard of study along with immunological confirmation when establishing the type of connective tissue diseases. The connective tissue diseases type identification should be done considering the clinical manifestations, based on the pathomorphological study results and the serological verification by specialists, since the nosology type establishment timely contributes to the immunopathological syndromes detection and allows the immune‑dependent complications prevention by adequately prescribed pathogenically valid treatment.
Цель исследования -провести анализ наиболее частых причин интра-и послеоперационных осложнений при хирургическом лечении туберкулеза легких и плевры на основе собственного опыта применения мини-инвазивных видеоассистированных торакальных оперативных вмешательств. Материалы и методи. В отделении торакальной хирургии и инвазивных методов диагностики Национального института фтизиатрии и пульмонологии имени Ф. Г. Яновского НАМН Украины за период с 2008 по 2019 гг. выполнено 169 VATS (video assisted thoracic surgery) операций пациентам фтизиохирургического профиля. В зависимости от вида VATS вмешательства пациенты были распределены на 3 группы: А -130 (79,3 %) больных, которым была проведена VATS резекция легкого или пульмонэктомия; В -35 (20,7 %) больных, которым была выполнена VATS плеврэктомия с декортикацией (ПЭ с ГК) легких; С -4 (2,4 %) больных, которым выполнены другие VATS вмешательства (пневмоторакс у больных туберкулезом (ТБ) без резекции легкого, коагуляция бул, плевродез).В группе А больным были выполнены следующие оперативные вмешательства: атипичная сегментарная резекция -у 28 (16,6 %) пациентов, типичная сегментэктомия -у 48 (28,4 %), лобэктомия -у 48 (28,4 %), билобэктомия -в 2 (1,2 %), пульмонэктомия -у 4 (2,4 %).Результаты. Частота интраоперационных осложнений при выполнении VATS резекции составила 3,8 %, при VATS ПК с ДК -11,4 %, послеоперационные осложнения зафиксированы у 17 (13,1 %) пациентов группы А и 12 (34,3 %) группы В. Послеоперационной летальности не отмечалось. Установлено, что причинами интраоперационных и послеоперационных осложнений при VATS оперативных вмешательствах по поводу ТВ легких и плевры могут быть такие факторы, как врожденная недостаточная выраженность междолевой борозды, ожирение и ряд медицинских (спаечный процесс в плевральной полости вследствие ТБ, эндотрахеальная интубация, неадекватный доступ), что в результате затрудняет проведение VATS оперативных вмешательств и увеличивает вероятность появления интраоперационных и послеоперационных осложнений.Заключение. Применение VATS является удобным и эффективным малотравматическим методом в лечении больных туберкулезом легких и плевры. Повышению эффективности и безопасности VATS оперативных вмешательств могут способствовать результаты проведенного авторами ретроспективного анализа причин интра-и послеоперационных осложнений.Ключевые слова: туберкулез легких и плевры, хирургическое лечение, видеоассистированные оперативные вмешательства, осложнения, причины.Укр. пульмонол. журнал. 2020, № 4.
