Se han propuesto diversas hipótesis: un antígeno tumoral podría desencadenar una respuesta inmune cruzada contra el nervio periférico o el cáncer podría favorecer el SGB a través de mecanismos mediados por inmunodepresión. En resumen, ante la presencia de un SGB el clínico debe descartar, además de las etiologías habituales cómo las infecciones respiratorias o del tracto gastrointestinal, la existencia de una posible neoplasia, entre ellas el carcinoma escamoso de pulmón. A su llegada a urgencias no presentaba síntomas relevantes, y se encontraba hemodinámi-camente estable .La exploración fue normal a excepción de una herida en la mano y sibilancias espiratorias aisladas. Se realizaron Rx tórax y de cráneo que mostraron elementos metálicos (perdigones) a nivel retroesternal, de cuero cabelludo y oreja izquierda. El TAC torácico mostró la presencia de un cuerpo extraño metálico intramiocárdico, próximo a epicardio a nivel de VD, cerca del anillo tricúspide en ausencia de derrame pericárdi-co (Fig. 1). El ecocardiograma confirmó la posición del proyectil, descartó la lesión de otras estructuras y la presencia de repercusión hemodinámica. El ECG y la analítica fueron normales. Se trataría por tanto, de un traumatismo cardiaco grado II según la clasificación propuesta por Moore, con lesión miocárdica tangencial sin afectación del endocardio ni taponamiento.El paciente fue dado de alta tras un período de observación y tras descartar la aparición diferida de derrame o signos de infección. É registrada a ocorrência de artrópodes (Arthropoda) principal mente insetos (Insecta), em frutos dessecados e produtos correlatos, no município de Pelotas e arredores. As observações realizaram-se no perto do entre 1969 e 1975. As espécies constatadas como mais frequentes e abundantes são Plodia interpunctella, Arecerus fasciculatus, Carpophilus hemipterus e Carpophilus dimidiatus. Os caros predominam ao final do inverno. Os coleópteros do gênero Carpophilos spp. são os mais frequentes e prejudiciais dos artrópodes encantados em frutos dessecados.
