BackgroundMultidrug-resistant tuberculosis (MDRTB) remains a health problem for many countries in the world. The share of MDRTB is 10–30% among newly diagnosed cases and 20–70% among relapses and treatment failure. The aim of the study is to define the side effects of second line drugs used in the treatment of MDRTB on thyroid function.MethodsIn 30 patients with multidrug resistant tuberculosis, echostructure of thyroid was studied by ultrasound imaging method. Indices of thyroid function: plasma levels of free thyroxin, thyroid stimulating hormone were studied before chemotherapy initiated, at the end of intensive phase and after the treatment finished.ResultsDecreasing of thyroid function under antituberculosis chemotherapy was approved. Monitoring and correction of thyroid function during antituberculosis chemotherapy was suggested.ConclusionPatients with MDRTB taking ethionamide and PAS are at increased risk for hypothyroidism and goiter, and therefore require monitoring of thyroid function at all stages of antituberculosis chemotherapy for its timely correction.
У 40 больных туберкулезом легких и сахарным диабетом проведено ультразвуковое исследование эхоструктуры щитовидной железы и в сыворотке крови иммуноферментным методом определены уровни содержания свободного тироксина, тиреотропного гормона гипофиза, антител к тиреоглобулину и тиреопероксидазе и уровень селена до начала и в конце интенсивной фазы химиотерапии в зависимости от назначения селенита натрия. Эффективность противотуберкулезной химиотерапии оценивалась по общепринятым клиническим, бактериологическим и рентгенологическим критериям. Аутоиммунный тиреоидит с явлениями субклинического гипотиреоза диагностирован у всех обследованных лиц. Применение селенита натрия на протяжении фазы интенсивной химиотерапии восстанавливало функцию щитовидной железы. Темпы исчезновения симптомов интоксикации, абациллирования мокроты и заживления деструкций в конце фазы интенсивной химиотерапии достоверно выше в группе больных с назначением селенита натрия.
Туберкульоз є найпоширенiшою опортунiстичною хворобою i головною причиною смертi у хворих, що є ВIЛ-iнфiкованими. Щитоподiбна залоза є унiкальним органом гуморального iмуногенезу, але патоморфологiчнi змiни при ко-iнфекцiї туберкульоз/ВIЛ ранiш нiколи не вивчалися. Мета дослiдження: вивчення патоморфологiчних змiн при ко-iнфекцiї туберкульоз/ВIЛ. Проведено мiкроскопiчне дослiдження зафарбованих еозином та гематоксилiном гiстологiчних зрiзiв фрагментiв щитоподiбних залоз 30 пацiєнтiв, померлих вiд ко-iнфекцiї туберкульоз/ ВIЛ. В результатi дослiдження виявлена велика кiлькiсть значно збiльшених за розмiром фолiкулiв зi сплесканим епiтелiєм та щiльнiм колоїдом, що свiдчить про функцiональне пригнiчення органу. Зроблено висновок про необхiднiсть прижиттєвого скринiнгу функцiонального стану щитоподiбної залози у пацiєнтiв з ко-iнфекцiєю туберкульоз/ВIЛ.
