Background: Study of pharmacokinetics of quercetin (Qu) and its derivatives after infusion is required in order to better understand mechanisms of therapeutic action of the formulation and development the strategic approaches to treat the underlying disease and pathologically similar syndromes. A pharmacokinetic study of Qu products is a complex analytical problem. Results: We developed and validated a new method for quantification of Qu and its metabolites in human plasma and urine following intravenous administration of Qu formulation. The method is based on the solid-phase extraction with the help of Oasis® HLB cartridges and ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) quantification. The calibration curves plotted for concentration range of 25-3000 ng/ml for Qu and isorhamnetin in blood plasma and of 100-8000 ng/ml for the same substances in urine were best described by a quadratic function. Qu conjugates underwent preliminary hydrolysis by the mixture of enzymes of sulphatase and β-glucuronidase. The antioxidant L-cysteine mixed with ascorbic acid was used for stabilization of Qu and its metabolites during handling of samples and chromatographic runs with mass detection. The method validation as well as the stability study results confirmed that the developed method meets the established requirements (selectivity, lower limit of quantification, accuracy, precision, recovery). The main pharmacokinetic parameters of Qu and its metabolites were determined in blood plasma and urine after intravenous administration of Corvitin® (parenteral formulation combining Qu and polyvinylpyrrolidone at a ratio of 1:9) in 12 healthy volunteers. Conclusions: We showed that the method development for Qu quantification in biological matrixes is valuable and reliable.
Expectorant phytomucolytic syrups are widely used pediatric OTC-medicines. Physicians, pediatricians, and pharmacists are traditionally concerned with the efficacy of the active ingredients in cough syrups, and rarely consider the safety aspects of excipients that however are not absolutely “inactive” and are proved to initiate some negative reactions and interactions with other drugs. This paper presents a review, categorization, and comparative analysis of the safety profile of excipients contained in the 22 best-selling OTC pediatric phytomucolytic syrups available in pharmaceutical markets in Ukraine and Germany and proposes an approach to the consideration of the excipients’ safety risks for a pharmacist in the process of pharmaceutical care. The study has revealed that only one of the twenty-two analyzed syrups does not contain any potentially harmful excipients. The results of this analysis were used for developing a specific decision tool for pharmacists that can be used for minimizing excipient-initiated reactions when delivering OTC phytomucolytic syrups for children.
Можна припустити, що інтраназальні форми препаратів, які чинять протизапальну дію (ПЗД), можуть бути ефективними для патогенетичної терапії гострого риносинуситу (ГРС) з урахуванням потенційного зменшення такого симптому як назальна обструкція. Мета роботи: визначити наявність протизапальних властивостей, оцінити їх дозозалежність та обґрунтувати вибір концентрації діючої речовини Енісаміуму йодиду у формі назального спрею при зовнішньому застосуванні. Матеріали та методи. Об'єктом дослідження став препарат «Амізон» (спрей назальний) виробництва ПАТ «Фармак» (Україна), діючою речовиною якого є Енісаміуму йодид у різних концентраціях. В якості референтного об'єкту був використаний Ібупрофен («Нурофен»)-таблетки, вкриті плівковою оболонкою, 200 мг, який застосовувався у дозі 48 мг/кг внутрішньошлунково. Був проведений первинний скринінг з вивчення антиексудативної дії Енісаміуму йодиду у трьох концентраціях за умови розвитку експериментального запального процесу з використанням моделі карагенінового запалення стопи у щурів. Протизапальну активність (ПЗА) Енісаміуму йодиду у концентраціях 5, 10 та 20 мг/мл в порівнянні з Ібупрофеном оцінювали у відсотках на 3 годину спостереження. Результати. Енісаміуму йодид в усіх вивчених концентраціях чинив статистично значиму протизапальну дію різного ступеня виразності, яка носила нелінійний характер при дослідженому шляху введення та обраному діапазоні концентрацій. Тому вибір найбільш оптимальної концентрації Енісаміуму йодиду у формі назального спрею було проведено за результатами порівняльного аналізу ПЗА тест-зразків дослідного засобу. Найвищий рівень ПЗА (35.3 %) Енісаміуму йодид проявив при застосуванні концентрації 10 мг/мл, яка й є оптимальною для подальших експериментальних досліджень. Висновки. Дослідження ПЗА Енісаніуму йодиду (спрей назальний) у діапазоні концентрацій 5, 10 та 20 мг/мл показало наявність нелінійної залежності «активність-концентрація». ПЗД досліджуваного засобу є найбільш вираженою при використанні концентрації 10 мг/мл. Енісаміуму йодид 10 мг/мл у формі назального спрею є засобом, перспективним для подальших доклінічних досліджень. (1) Background: this research aims at studying the nephroprotective properties of BNO 2103 in a model of chromate-induced renal failure in rats and at proving the possibility of using BNO 2103 in clinical practice for the complex treatment of chronic kidney disease (CKD). (2) Methods: fifty rats divided into five groups were studied. The drugs BNO 2103, Prednisolone and Lespephril were administered within 20 days. The excretory function and the functional state of kidneys, blood biochemical parameters and indicators of nitrogen metabolism were determined. (3) Results: under the influence of BNO 2103, there was a significant improvement in renal excretory function, in nitrogen metabolism and blood biochemical parameters compared with the control pathology group. BNO 2103 also outperformed the comparators in most indicators. (4) Conclusions: BNO 2103 has demonstrated nephroprotective, hypoazotemic and diuretic effects; and can be used to implement to the combined therapy of CKD.
