Objetivo: Relatar o caso de uma paciente portadora de ureterolitíase que foi submetida à ureteroscopia, procedimento no qual ocorreram dificuldades devido ao histórico de radioterapia pélvica prévia da paciente. Detalhamento do caso: Paciente de 50 anos com história de dor lombar à direita a cerca de um ano com histórico de tratamento oncológico de braquiterapia para câncer de colo uterino. A anamnese revelou que a paciente já possuía história prévia de nefrolitíase, com cálculo de 1,6cm em pelve renal direita, mas após ser submetida à Litotripsia Extracorpórea por Onda de Choque (LECO), evoluiu com uropatia obstrutiva por impactação dos fragmentos de cálculo. Esse quadro levou a paciente a realização de tratamento endourológico. A realização de ureterolitotripsia endoscópica revelou ureter com estenoses em porção distal e paredes fibrosadas, relacionadas o tratamento radioterápico prévio. Considerações finais: Nesses casos, é necessário que o urologista esteja alerta aos antecedentes clínicos de cada paciente, pois – ao se deparar com histórico positivo, terá um sinal de alerta e estará mais preparado mediante possíveis dificuldades considerando que os efeitos adversos do tratamento radioterápico podem alterar os resultados esperados tanto da LECO quando da ureterolitotripsia endoscópica, impondo dificuldades significativas ao tratamento.
Objetivo: Comparar prevalência e grau de disfunção erétil (DE) entre os pacientes com Doença Renal Crônica (DRC) transplantados renais e em hemodiálise. Métodos: Estudo analítico e transversal, incluindo 92 indivíduos do sexo masculino em serviço de referência, com idade de 20 a 68 anos. Investigou-se presença de DE pelo Índice Internacional de Função Erétil (IIEF-5), e informações sobre idade, etiologia, medicações, tabagismo e tempo em substituição renal pelo prontuário. Resultados: A idade média foi 53.4±14.82 no grupo hemodiálise (HD) e 48.2±14.34 no grupo transplante (TR). Em HD, 78% apresentaram DE, e em 57.2% em TR. A classificação no IIEF-5 demonstrou 28% HD com DE moderada e 21.5% TR com DE leve/moderada. O tempo médio em hemodiálise foi 3.4 anos para HD e 4 anos para TR. O tempo em hemodiálise ≥ 4 anos afetou a pontuação IIEF-5 em ambos os grupos (p=0.0474). Faixa etária ≥ 50 anos teve associação com DE em HD (p=0.0411) e em TR (p=0.0045). Uso de inibidores de 5-fosfodiesterase foi encontrada em 30.9% TR e 10% HD, tendo impacto sobre o IIEF médio de ambos os grupos (p=0.0193). Conclusão: A prevalência de DE é alta em pacientes em terapia de substituição renal, sendo mais frequente na hemodiálise. O transplante renal aparece como alternativa para melhora e estabilização da função renal nos pacientes estudados, com impacto positivo na saúde mental e bem-estar dos renais crônicos.
Introdução: A Amazônia apresenta alta endemicidade de hepatites C e D que, em alguns casos, podem evoluir para cirrose, insuficiência hepática e morte, sendo o transplante hepático uma das formas de tratamento. Novas técnicas de segmentação hepática estão sendo desenvolvidas com o objetivo de realizar o transplante hepático com o fígado de doador vivo, o que reduziria a espera por um órgão na fila de transplante, diminuindo assim a sua mortalidade. Com o desenvolvimento da técnica de bipartição do fígado foi verificado que, em 5% dos casos, o transplante hepático não pode ser realizado devido às variações anatômicas; sendo que a artéria hepática (AH) pode apresentar de 25% a 40% de variações anatômicas na sua origem e nos seus ramos. Objetivo: Estudar a disposição anatômica e as possíveis variações da artéria hepática comum e de seus ramos em cadáveres humanos. Método: Utilizadas 12 peças em monobloco contendo esôfago, estômago, pâncreas, artéria aorta abdominal, fígado e seus vasos, de cadáveres provenientes do Instituto Médico Legal do Centro de Perícias Científicas Renato Chaves, Belém-Pará, removidos pela técnica de retirada de fígado para transplante ortotópico, seguida de dissecação da AH desde sua origem até o hilo hepático e catalogação fotográfica. Resultados: Mostraram que a AH se originou do tronco celíaco em 91,7% e em 8,3% dos casos do tronco hepatoesplênico. A artéria gástrica direita originava-se direto da artéria gastroduodenal em 8,3% dos casos. Conclusão: As principais variações anatômicas da artéria hepática comum encontradas foram: a presença, em sua origem, de tronco hepato-esplênico e a artéria gástrica direita como ramo da artéria gastroduodenal.
