A osteogênese imperfeita (OI) é uma patologia geneticamente determinada, afetando diretamente estruturas e funções do colágeno tipo I, sendo este, representante de 90% de todo o colágeno tecidual, e responsável por grande parte dos tecidos fibrosos densos, que formam o sistema muscular esquelético. Trata-se de uma doença rara, a depender da classificação, dividida em dois grandes grupos, ‘’congênita’’ e ‘’tardia’’. Por certo, o diagnóstico é feito a partir do histórico do paciente, quadro clínico, aspecto ao exame físico e resultados radiográficos, inexistindo um exame específico e complementar para uma confirmação da doença, isto tudo devido a diversidades de apresentações fenotípicas, porém sempre com fragilidade óssea marcantes, frouxidão cápsulo-ligamentar, esclera com a cor azulada, surdez, dentinogênese imperfeita, deformidade nos ossos longos e hiperextensibilidade articular. Devido a vasta forma de apresentação, a OI possui como diagnósticos diferenciados, categorias baseadas em estágios de vida do paciente, devendo-se considerar três estágios, pré-natal/neonatal, infância e adolescência. Por se tratar de uma condição genética, com todos os estudos e apesar dos avanços tecnológicos, não existe uma cura para a OI, porém existem três formas fundamentais para tratamentos da doença, a forma terapêutica, a cirúrgica ortopédica e a reabilitação. Todo o prognóstico depende da forma como a OI se apresenta, o prognóstico pode ser muito variável, será observado o grau de severidade e acompanhamento terapêutico.
A hemorragia digestiva Alta (HDA), classificada como varicosa e não varicosa, trata-se de um sangramento originado do trato gastrointestinal próximo ao ângulo de Treitz, capaz de envolver o esôfago, estômago e duodeno. Mesmo com os avanços na ciência em relação a diagnósticos, a HDA está entre as emergências mais frequentes em serviços de saúde brasileiros, existindo a necessidade de atendimento mais ágil associado a um diagnóstico adequado, afim de evitar que o paciente venha a óbito. Dentre os principais fatores relacionados a HDA é possível citar a doença ulcerosa péptica e também as varizes esofágicas. Sabe-se que o sangramento de úlceras pépticas representa 25% dos casos de HDA e apresenta fatores de risco que contribuem para os sangramentos, quais sejam: infecção por Helicobacter pylori, utilização de anti-inflamatórios (AINEs), estresse e hipersecreção ácida. Por outro lado, as varizes esofágicas encontram-se presentes em 50% dos pacientes cirróticos, estando relacionada também a quadros de hipertensão portal. Por certo, o risco de sangramento de varizes esofágicas está intimamente relacionado ao tamanho, grau de disfunção hepática e também a presença de marcas vermelhas. Diante da grande variabilidade de achados clínicos, o diagnóstico etiológico é definido mediante a realização da endoscopia digestiva alta, devendo ser realizada de forma precoce, a partir da estabilização do paciente. É sabido que, no momento do diagnóstico de uma lesão sangrante a terapia endoscópica deverá ser imediatamente executada, a fim de promover a hemostasia e prevenir a recorrência do sangramento em grande parte dos pacientes.
A Síndrome Metabólica (SM) é um transtorno complexo representado por um conjunto de fatores de risco cardiovascular usualmente relacionados à deposição central de gordura e à resistência à insulina. Segundo estudos abordados, foi observado que grande parte dessa população, sobretudo os adolescentes em específico, apresenta-se em um quadro de obesidade ou sobrepeso, podendo levar à síndrome metabólica e disfunção endotelial, um marcador precoce de risco cardiovascular. A obesidade tem aumentado rapidamente em todo o mundo e é considerada um fator de risco para doenças crônicas não transmissíveis. Crianças e adolescentes têm sido seriamente impactados por essa tendência, já que um perfil pró-inflamatório parece também ser mais comum nessas crianças e adolescentes com excesso de peso. Através dos estudos analisados foi possível verificar que a elevada prevalência de obesidade em crianças e adolescentes sinaliza que políticas públicas de prevenção das doenças cardiovasculares e diabetes, quando ainda não se têm agravos estabelecidos, têm potencial de alcançar melhores resultados no enfrentamento das doenças cardiovasculares e diabetes, levando em consideração o fato de que essa condição pode ser um possível preditivo de obesidade na fase adulta, aumentando o risco do indivíduo de ser um portador de doença crônica.
A infecção por Helicobacter Pylori (HP) afeta bilhões de pessoas ao redor do mundo, sendo essa bactéria considerada o agente infeccioso mais prevalente nos seres humanos. Nos últimos anos, a literatura tem evidenciado uma possível associação entre essa infecção e o desenvolvimento de doença arterial coronariana (DAC), uma vez que ambas condições são consideradas doenças crônicas, compartilhando entre si de eventos fisiopatológicos semelhantes. O presente estudo objetiva avaliar a associação entre a infecção por HP e o risco de desenvolvimento de DAC. Para essa Revisão de Escopo, realizada através das plataformas PubMed e Cochrane Library entre 2022 e 2012, os descritores eleitos foram ‘’Helicobacter Pylori’’, ‘’Coronary Disease’’ e ‘’Heart’’, associados ao operador exclusivamente ao booleano ‘’AND’’. Os critérios de inclusão consistiram 1) ensaios clínicos, testes controlados e aleatórios, revisões e meta análises, 2) trabalhos acerca da associação entre DAC e HP, 3) estudos que avaliaram a extensão dos danos cardiovasculares em decorrência da infecção por HP e 4) artigos escritos em língua portuguesa, japonesa, inglesa ou em espanhol. Os critérios de exclusão incluíram inadequações ao tema, relatos de casos e estudos com animais, além de textos fora do limite temporal. As evidências encontradas sugeriram que os principais efeitos adversos cardiovasculares relacionados à infecção por HP estavam associados à DAC, a aterosclerose, a hipertensão arterial sistêmica e ao infarto agudo do miocárdio. Métodos de rastreio, avaliações intervencionistas e indicações para a utilização de marcadores laboratoriais, assim como mais análises clínicas acerca dessa interação ainda permanecem como tópicos a serem melhor elucidados.
Uma das desordens mais comuns do cérebro é a epilepsia, sendo capaz de afetar milhões de pessoas a nível mundial, ao passo que de acordo com estimativas do Ministério da saúde, é responsável por atingir em média 18,5 a cada 1000 crianças. Apresentada como uma doença crônica, nota-se um sintoma de distúrbio neurológico que geralmente é manifestado com forma de crises convulsivas frequentes e grau variável de intensidade e duração. Por outro lado, tem-se como transtorno o TDAH que se caracteriza pela presença de um inapropriado desempenho dos mecanismos que regulam a atenção de atividades motoras e de impulsos. Devido a complexidade do TDAH, é de suma importância a busca por parâmetros que ajudem no reconhecimento do distúrbio propriamente dito, no qual seja possível diferenciar a hiperatividade de outras doenças também capazes de ocasionar a agitação física e mental. É estimado que em média 40% a 50% das crianças e adolescentes epiléticos apresentem comorbidades psiquiátricas, enquanto o TDAH na população pediátrica tem uma prevalência em torno de 3% a 7%. Por certo, diante das sequelas mais comumente encontradas, é possível evidenciar os distúrbios de aprendizagem além alterações frequentes do humor. Atualmente, muito diferentemente de alguns anos atrás, a saúde física e social, tem recebido grande importância.
