La enfermedad trofoblástica gestacional está compuesta por una gama de patologías poco comunes asociadas al embarazo, que van desde benignas hasta malignas. Es importante realizar un diagnóstico oportuno mediante el uso de ultrasonido; apoyándose con estudios de laboratorio y gabinete, estudios histológicos y moleculares. En términos generales, el tratamiento de la enfermedad molar premaligna va a consistir en la evacuación uterina. El uso de quimioterapeúticos y de intervenciones quirúrgicas, en conjunto con el uso de radioterapia, podría llegar a tener un papel importante en el tratamiento tanto de estas como de las enfermedades trofoblásticas neoplásicas. La hormona gonadotropina coriónica humana es un biomarcador crucial para medir la progresión, respuesta a tratamiento y seguimiento de estas enfermedades. Gracias a la implementación de mejoras en el diagnóstico y tratamiento, la presentación clínica y morbilidad de las enfermedades ha cambiado conforme pasan los años.
La diabetes mellitus gestacional es una condición en la que se desarrolla intolerancia a los carbohidratos y se diagnostica por primera vez en el embarazo sin una pre-existencia clara de diabetes tipo 1 o tipo 2. Es una patología de gran importancia debido a su creciente prevalencia y asociación a complicaciones maternas, fetales y/o neonatales que afectan a múltiples áreas de la medicina y la comunidad. Pese a que aún hay controversia respecto a su diagnóstico, múltiples estudios recomiendan el método de 2 pasos basado en un tamizaje con 50 gramos de glucosa oral y posteriormente una prueba de tolerancia de glucosa de 3 horas con una ingesta de 100 gramos de glucosa en aquellas que presenten un tamizaje positivo. El pilar del tratamiento va a consistir en la dieta y estilos de vida saludable. Para aquellas pacientes que requieran tratamiento farmacológico, el principal ente va a seguir siendo la insulina, pudiendo utilizar asimismo la metformina como una alternativa viable. Aún queda mucho por explorar y determinar en esta patología compleja, para lograr alcanzar un consenso entre las distintas guías y conseguir las mejores recomendaciones para la población a escala mundial.
Las alteraciones de la función tiroidea son un conjunto de patologías relativamente comunes en la población general. Estas patologías también se pueden presentar durante el embarazo y de no ser diagnosticadas y tratadas de forma oportuna pueden ocasionar complicaciones para la mujer gestante e incluso causar afectación fetal. El diagnóstico de la disfunción tiroidea durante el embarazo se realiza mediante la cuantificación de los niveles sanguíneos de TSH. Por su parte el tratamiento se encuentra bien definido para la patología clínica, ya sea con levotiroxina para el hipotiroidismo o con propiltiouracilo y derivados de imidazol para el hipertiroidismo. La terapéutica para el manejo de pacientes con enfermedad subclínica no se encuentra bien definido.
