A propagação da aids tem sofrido mudanças em seu perfil, e o aumento de casos em mulheres tem sido cada vez mais frequente, contribuindo de modo decisivo para o fenômeno chamado feminização da aids. Levando em consideração essa realidade, este trabalho tem por objetivo analisar a exposição de adolescentes às DST/aids no semiárido nordestino. A metodologia de trabalho utilizada foi baseada na abordagem qualitativa em saúde, utilizando como modelo teórico-metodológico as Práticas Discursivas e Produção de Sentido (Spink, 2004; Spink e Medrado, 1999). Ao todo três estratégias metodoló-gicas foram utilizadas para a coleta de dados: 1024 horas de observação participante, 72 entrevistas semiestruturadas e 36 grupos focais, englobando dois grupos de atores: 72 profissionais de 8 equipes de saúde e 360 adolescentes, de ambos os sexos. A partir da análise dos resultados, constatou-se que a realidade nordestina é permeada por características que tornam a mulher vulnerável às DST/aids, tais como: a) a baixa escolaridade; b) as relações desiguais de gênero; c) a ausência de uma política de prevenção direcionada ao público adolescente; d) a inexistência de vínculo entre profissionais da saúde e população; e e) a frequente compreensão dos serviços de saúde como espaço exclusivamente da mulher. Diante desse contexto, aponta-se a necessidade da efetivação de políticas de atenção à saúde integral de adolescentes, que trabalhem os direitos sexuais e reprodutivos na perspectiva de gênero, para a efetiva prevenção das DST/aids, exercendo impacto direto na promoção da qualidade de suas vidas.
BackgroundPathways to care are actions and strategies employed by individuals in order to get help for health-related distress and the related processes of care providers. On several systematic reviews regarding pathways to mental health care (PMHC), studies regarding South American countries were not present. This review synthesizes qualitative and quantitative research about PMHC in Brazil.MethodsLILACS, MEDLINE and SCIELO databases were searched for papers regarding PMHC in Brazil. The results were organized in pathway stages, based on Goldberg and Huxley’s ‘model of Levels and Filters’ and on Kleinman’s framework of ‘Popular, Folk and Professional health sectors’. Analysis also considered the changes in national mental health policy over time.Results25 papers were found, with data ranging from 1989 to 2013. Complex social networks were involved in the initial recognition of MH issues. The preferred points of first contact also varied with the nature and severity of problems. A high proportion of patients is treated in specialized services, including mild cases. There is limited capacity of primary care professionals to identify and treat MH problems, with some improvement from collaborative care in the more recent years. The model for crisis management and acute care remains unclear: scarce evidence was found over the different arrangements used, mostly stressing lack of integration between emergency, hospital and community services and fragile continuity of care.ConclusionsThe performance of primary care and the regulation of acute demands, especially crisis management, are the most critical aspects on PMHC. RESUMOO artigo discute os impasses e desafios relacionados à implantação de ações educativas em saúde sexual para adolescentes na Estratégia Saúde da Família (ESF) das cidades de Petrolina-PE e Juazeiro-BA. A relevância deste trabalho pauta-se no fato de ainda persistirem, em muitas unidades de saúde, práticas curativistas que contrariam a política de promoção da saúde preconizada pelo SUS. Como instrumentos para coleta de dados, foram realizados grupos focais com 360 adolescentes, 72 entrevistas com profissionais de saúde e 1024 horas de observação participante em oito dispositivos da ESF. Observou-se a inexistência de ações educativas voltadas aos adolescentes, justificada pela precariedade de infraestrutura, pelo despreparo dos profissionais e pela falta de profissionais na rede. Assim, em defesa de uma educação popular e emancipadora, o estudo aponta para a necessidade de se pensar práticas de saúde pautadas na formação de sujeitos-cidadãos responsáveis pelo cuidado da própria saúde sexual.Palavras-chave: saúde da família; saúde do adolescente; educação em saúde. ABSTRACTIn the article we discusses the dilemmas and challenges related to implementation of educative actions in sexual health for adolescents in the Family Health Strategy (FHS) of the cities Petrolina-PE e Juazeiro-BA. The relevance of this work is based on the fact that still persists in many health units, curative practices that run against to the promotion of health recommended by SUS. As instruments for data collect, focal groups were conducted with 360 adolescents, 72 interviews with health professionals and 1024 hours of participant observation in eight devices of FHS. There was a lack of educational activities geared to teenagers, justified by the physical and material insecurity, the lack of professionals and the lack of professionals in the network. So, in defense of a popular and emancipatory education, the study points to the need to think about health care practices based on the formation of subject-citizens responsible for the care of their own sexual health.
Resumo O Sistema Único de Saúde (SUS) tem na Estratégia Saúde da Família (ESF) o modelo reorientador da atenção e do processo de trabalho. O artigo analisa concepções de profissionais da ESF relativas à saúde mental e à produção do cuidado em uma Unidade Básica de Saúde, no Nordeste brasileiro. Trata-se de uma pesquisa qualitativa, alinhada à vertente crítico interpretativa, empregando entrevistas, observação livre e análise documental gerando uma rede de significados organizada em dois eixos: a) “Saúde Mental” na perspectiva dos profissionais da Estratégia Saúde da Família e b) O cuidado em “Saúde Mental”: a práxis no contexto da atenção básica. Apesar do potencial dos dispositivos, os resultados do estudo evidenciam concepções acerca da saúde mental circunscritas a uma visão congruente com um projeto de institucionalização e medicalização. No que concerne à dimensão do cuidado, revelam-se lacunas relativas às experiências em saúde mental compartilhadas, através de incipiente parceria entre profissionais da ESF e NASF. Persiste o desafio de reconstruir uma visão alinhada aos preceitos da reforma psiquiátrica brasileira e, na materialidade do trabalho em equipe, criação de espaços que possibilitem no cotidiano da ESF discussões, planejamentos e pactuações.
A Atenção Básica da Saúde no Brasil é organizada, prioritariamente, pela Estratégia de Saúde da Família (ESF) e deve desenvolver ações democráticas e participativas nos espaços coletivos, dentre elas as direcionadas à atenção dos adolescentes. Partindo desta prerrogativa, o presente trabalho tem como objetivo tecer algumas reflexões sobre os desafios para a articulação de redes sociais na implantação de novas modalidades de cuidado à saúde sexual e reprodutiva de adolescentes. Para tanto, foi desempenhada uma pesquisaação (MORIN, 2004), com o suporte teórico do Construcionismo Social (SPINK; MEDRADO, 1999), na qual foram realizadas oficinas de trabalho com 72 profissionais de saúde objetivando o desenvolvimento de 128 rodas de conversas sobre sexualidade com cerca de 240 adolescentes, autorizados por seus pais ou responsáveis. Para construção das redes de cuidado, foram articuladas 7 escolas municipais, 1 escola militar, 1 CRAS, 1 ONG dirigida a jovens e 1 igreja católica. Os resultados indicam diversas dificuldades para o desenvolvimento de ações em saúde com adolescentes e sobre sexualidade. Tais dificuldades envolvem desde o perfil dos profissionais de saúde, centrado no modelo de atenção medicamentoso e especialista, que dificulta trabalhos participativos e dialógicos entre os profissionais, e entre eles e os adolescentes; até a falta de articulação da equipe de saúde com os equipamentos sociais locais, evidenciando a dificuldade dos mesmos trabalharem de forma intersetorial e no território, com vistas a uma atenção integral em saúde.
