Trata-se do relato de uma pesquisa sobre a organização das ações de saúde mental na atenção básica de Campinas (SP), a partir da implantação do arranjo de gestão Apoio Matricial. Esse arranjo visa a disparar a ampliação da clínica das equipes de referência e reorientar a demanda para a saúde mental. Desviando os encaminhamentos indiscriminados para uma lógica da co-responsabilização, ele pretende produzir maior resolutividade à atenção em saúde. Foram realizados grupos focais com profissionais de saúde mental, das equipes de referência e gestores. Numa abordagem hermenêutica-crítica, procurou-se encadear as linhas argumentativas dos grupos focais de modo narrativo, a fim de combinar a análise com a construção de sentidos para o material produzido. Observou-se a incorporação da saúde mental no fazer das equipes de referência e a diversificação dos dispositivos terapêuticos. Avaliou-se a necessidade de aprimorar a avaliação de risco e a importância da criação de espaços permanentes de análise sobre o Apoio Matricial, a fim de realimentar sua potencialidade em transformar as práticas hegemônicas em saúde. Pretende-se, com este artigo, contribuir para que as discussões geradas pelos profissionais possam ser significadas no interior das transformações políticas e assistenciais na área da Saúde Mental.
This article presents a preliminary discussion of potential methodological tools for qualitative research on the Network of Referral Centers for Psycho-Social Care (CAPS) in the Brazilian Unified Health System (SUS). The relevance of mental health within the field of public health is examined. The study focuses on the high prevalence of mental disorders and the disproportionate lack of studies on the interface between mental health and public health. The establishment of an interdisciplinary field between public health and mental health is proposed to meet common needs by achieving similar perspectives in knowledge and practice. A particular group of tools is proposed, emphasizing the importance of reclaiming and guaranteeing the roles of various social actors to shape the assessment process, the need for collecting and standardizing academic studies on the topic, and the importance of promoting a new research field focusing on public health policies to support policymakers, managers, and health teams in reshaping their practices.
Saúde mental na atenção primária à saúde: estudo avaliativo em uma grande cidade brasileira Mental Health in Primary Care: an evaluative study in a large Brazilian city Resumo A Atenção Primária à Saúde é reconhecida mundialmente como a responsável pela solução dos principais problemas de saúde da comunidade, incluso a mental. O presente estudo buscou avaliar a articulação entre as redes de atenção primária e de saúde mental em regiões de alta vulnerabilidade social de uma grande cidade brasileira (Campinas -SP) através dos parâmetros: pesquisa avaliativa, participativa e predominantemente qualitativa. Foram definidos dois grupos pela téc-nica de clusters: um de Unidades Básicas de Saúde com maior grau de implantação das ações inovadoras e outro com menor grau de implantação. Posteriormente foi realizada uma comparação. O apoio matricial se mostrou potente para definir fluxos, qualificar as equipes e promover uma assistência conjunta e compartilhada. Detectou-se um papel estratégico do agente comunitário de saúde para identificar ofertas em potencial e propiciar escuta de forma mais próxima à população. As prá-ticas de promoção à saúde ainda não estão consolidadas. Quando os arranjos possibilitam inserções dos profissionais em atividades extramuros, os usuários reconhecem e se apropriam dos espaços, possibilitando ações que se distanciam da tradicional queixa-conduta. The scope of this work was to evaluate the links between primary care and mental health in areas of high social vulnerability in a large Brazilian city (Campinas, State of São Paulo) using parameters defined in this study. Evaluative and participative -mainly qualitative -research, was conducted. Two groups were defined by cluster technique: one group of Basic Health Units with a higher degree of innovative strategies and another with a lower level of innovation, and the two were then compared. Matrix support was observed as a powerful tool to define workflows, to qualify the teams and to promote joint and shared care. The strategic role of community health agents was seen to identify potential benefits and listen more closely to the population's needs. Health promotion practices are not consolidated as yet. When innovative strategies take the professionals into extra-mural activities, users acknowledge and occupy new spaces, with actions that are different from the traditional complaint-behavior model.
OBJECTIVE:To compare the performance of Primary Care Units according to the implementation of new arrangements and strategies in primary care and mental health. METHODOLOGICAL PROCEDURES:Evaluative research with triangulation of methods and theoretical framework of critical hermeneutics, carried out at six Primary Care Units of the two most populous health districts of the city of Campinas (Southeastern Brazil) in 2007. The Primary Care Units were analyzed according to clinical resolution, articulation between the primary care and mental health networks and implementation of health promotion strategies. Two groups were defi ned by cluster analysis: one with higher and another one with lower degree of implementation of the actions. The groups were compared based on the improvement in clinical follow-up, given by the occurrence of cerebral vascular accident; evaluation of dispensation of psychiatric medicines; focal groups with workers, users and community health agents; and interviews with users and relatives. Inclusive and participatory research strategies were employed. ANALYSIS OF RESULTS:There were no pure models, but a mosaic of organizational proposals. Positive advances were identifi ed in the group with higher implementation of innovative strategies in relation to better integration of the community agents in the Units' teams; to the workers' and agents' perception of improvement in the assistance; and to the facility for referrals and assistance of mental health cases. The diffi culties identifi ed in both groups were: communication among the levels of care and within the teams, in the implementation of matrix support, and incipient health promotion actions. CONCLUSIONS:The development and implementation of mechanisms to fi x professionals in Primary Care in large cities are necessary. The community health agents are fundamental to perform the territorial work proposed by the Family Health Strategy, using mechanisms to integrate the community health agents into the healthcare teams in order to counterbalance the tendency to isolation. The researched arrangements proved to be potent to produce this integration.
The network of psychosocial care centers in Campinas stands out due to its originality in the implementation of six CAPS III and to its efficacy in providing comprehensive assistance to users and family members in the moment of crisis and in rehabilitation. The organization in reference technician and/or team prevails, as well as the development of therapeutic projects. Night teams reduction is the most important problem and the main source of workers' stress. The professionals' education proved to be insufficient to deal with the challenges faced by these services.
This study was designed to analyze actions in mental health from the psychosocial care network through the perspective of users. Fourth-generation participatory methodology was used. The findings highlight the challenges of implementing mental health services in primary care and in different points of the psychosocial network, as users recognize the existence of a health network but indicate limited access to it, because, as they see it, actions could be undertaken in spaces that would facilitate their inclusion and integration with the community. There is weakness in the articulation and in the implementation of integrated care plans in services and, consequently, mental health needs to remain centralized in specialized services, showing strong inhibition of the work in the psychosocial care network. We concluded that the limits of working in networks present challenges in the daily life of service users and in the construction of new possibilities and progress towards social inclusion. Santos, SP, Brazil. CEP 11015-000. Resumo O objetivo deste artigo é analisar ações em saúde mental a partir da rede de atenção psicossocial pela perspectiva dos usuários. Usou-se metodologia participativa de quarta geração. De acordo com os resultados, os dados evidenciam desafios na implementação de ações de saúde mental na atenção básica e em diferentes pontos da rede psicossocial, visto que os usuários reconhecem a existência de uma rede de saúde, mas indicam acesso limitado a ela, pois entendem que as ações poderiam ser realizadas em espaços facilitadores de sua aproximação e inserção na comunidade. Há fragilidade na articulação e na efetivação de cuidados integrados nos serviços e, consequentemente, as demandas de saúde mental se mantêm centralizadas nos serviços especializados, demonstrando forte inibição do trabalho em rede. Conclui-se que os limites do trabalho em rede apresentam desafios no cotidiano dos usuários dos serviços e apontam outros desafios para a construção de novas possibilidades e avanços em direção à inserção social. RESUMO Objetivo Descrever a atuação dos Centros de Atenção Psicossocial (CAPS) em quatro centros urbanos no Brasil. Métodos Realizou-se um estudo transversal (AcesSUS) com entrevista a 917 usuários acompanhados nos CAPS de Campinas, São Paulo, Porto Alegre e Fortaleza. Foram incluídas apenas as modalidades de CAPS destinadas a transtorno mental grave em adultos, sendo excluídos os CAPS voltados para uso de substâncias e para crianças e adolescentes. Os usuários responderam a questionário de múltipla escolha sobre seu percurso entre os serviços de atenção primária, CAPS e hospitais em relação aos problemas de saúde mental. Resultados Atendimento psiquiátrico e uso de medicação foram relatados em 90% a 100% dos usuários acompanhados nos CAPS, combinado com atendimento multiprofissional e grupos terapêuticos. O acesso à medicação foi interrompido nos 6 meses que antecederam a entrevista, especialmente em Fortaleza (64%) e Porto Alegre (51%). Em todos os municípios, menos de 10% dos usuários tiveram internação psiquiátrica após início do tratamento no CAPS. Conclusões A transição do modelo hospitalar para o comunitário continua em processo de efetivação no Brasil, com avanços comprovados. A situação, porém, é diversa nas diferentes redes de atenção, requerendo análise contextualizada de cada localidade, com identificação de dificuldades que impedem o adequado funcionamento dessas redes no paradigma psicossocial.
