Fluorated varrushes ResumenDesde mediados del siglo XX, los trabajos de Cox, Dean y Armstrong permitieron concluir que el Fluor aumenta la resistencia a la Caries Dental generando fluorosis en altas concentraciones (mayores de 1 ppm. 1 Hoy ,iniciando el siglo XXI y a pesar de todas las lineas de investigación sobre métodos preventivos de la Caries, ninguna propuesta ha superado al flúor como medida en costo, inocua y eficiencia. 2A continuación presentarnos una revisión bibliográfica del Flúor como método preventivo de Caries Temprana, enfocándonos en el Barniz de Flúor y mostrando a la comunidad odontopcdiátrica el protocolo de aplicación. Mecanismos de AcciónEl efecto preventivo de los fluoruros parece ser la suma de los resultados de diversos mecanismos: 1° Inhibicíón de la desllÚneralización y catálisis de la remineralización del esmalte desmineralizado.-En los años 60, se creía que un efecto preventivo de caries dental se lograba por el incremento en la cantidad de flúor en la superficie del esmalte, lo que reducía su solubilidad, prindpalmente a través de la formación de fluorapatita 2.Sin embargo este concepto ha cambiado, atribuyéndole el efecto preventiV'o a la presencia continua del ión flúor en el medio peridentario y no a la incorporación de flúor en el esmalte.3,4 Varios estudios demostraron que niveles altos de flúor en el esmalte no garantizan protección contra la caries dental. Arends, 1990. 5 Diversas investigaciones concluyen que niveles adecuados de flúor en el fluido de la placa favorecen el equilibrio entre los ácidos orgánicos, fosfatos, calcio y otros que pudieran retardar o inhibir la producción ácida y! o promover la remineralización (Fejerskov , 1981, White et al 1994, Rolla, Ekstrand, 1996. 6,7 Por otro lado es importante resaltar el papel del fluoruro de calcio, molécula que precipita intrabucalmente y se mantiene por unas 25 semanas luego de una topícación (Ogaard, 1994)7. Otros han descrito una pérdida total de CaF2, después de 24 horas de una aplicación de fluoruro"·9 El" F2Ca cumple la función de reservorio de fluoruro ph dependiente, es estable en la saliva a un pH neutro y es el más importante surtidor de iones De esta manera, el flúor se comportaría corno un catalizador que permite una remineralización más rápida de las lesiones incipientes, requiriendo en promedio 100 ppm (m g!L) de ión flúor
La odontología para el bebé surge en 1985 en la Universidad estadual de Londrina (UEL) -PR a través de un convenio con a FINEP , en respuesta a una tendencia mundial de atención odontológica en niños de baja edad, con el establecimiento de programas educativos y preventivos para el control de caries dental. De la aplicación del concepto atención odontológica a bebes y la divulgación de sus resultados paso a ser transferido al sector publico en el Brasil . En el Perú se ha implementando en setiembre de 1997 la unidad de bebé en el instituto especializado de salud del niño un programa destinado a la atención de niños menores de 3 años . En nuestro país como en muchos países latinoamericanos había un consenso dentro de la odontopediatría de cual era el momento ideal para comenzar los exámenes dentarios y los tratamientos , y ello era alrededor de los 2 o 3 años de edad pero estudios realizados en 1987 (Walter et al ;1986) demostraron que la prevalencia de caries en niños de 0- 30 meses de edad era alta (34%) que coincidía con datos mundiales. Por tanto gran parte de la población menor de 3 años era desasistida de la atención odontológica presentando necesidades curativas incluso antes del primer examen de rutina. Ante esta realidad el IESN implementa el servicio de odontología del bebe para brindar atención especializada a niños menores de 3 años distribuyendo en 2 grandes grupos : aquellos que no han desarrollado la enfermedad basados en el riesgo de caries y los que ya presentan la enfermedad basados en la actividad de caries De los años de trabajo e investigación, mostramos a la comunidad odontopediátrica un modelo de atención que puede guiar el trabajo en este grupo etáreo.
