INTRODUÇÃO: O pólipo antrocoanal ou pólipo de Killian é uma lesão polipóide solitária benigna, que acomete principalmente crianças e adultos jovens. Estudos demonstram que o pólipo de Killian representa entre 4-6% de todos os pólipos nasais da população em geral. Contudo, na população pediátrica, esta porcentagem atinge 33%. Origina-se de uma hipertrofia da mucosa do antro do seio maxilar próximo ao óstio e desenvolve-se, por estímulo desconhecido, através do óstio do seio maxilar para a cavidade nasal e em direção a coana e parte posterior da nasofaringe, podendo estender-se até a orofaringe. OBJETIVO: Avaliar o resultado do tratamento cirúrgico em pacientes atendidos no Serviço de Otorrinolaringologia do Hospital Universitário Walter Cantídio da Faculdade de Medicina da Universidade Federal do Ceará, principalmente em relação à técnica cirúrgica empregada e a eficácia de cada técnica no controle da doença. MATERIAL E MÉTODO: Trata-se de um trabalho retrospectivo, realizado através da análise dos prontuários dos pacientes submetidos à polipectomia por pólipos de Killian ou por outros pólipos nasais, atendidos no período de 01.03.1991 a 30.04.2001, no Setor de Otorrinolaringologia do Hospital Universitário Walter Cantídio da Universidade Federal do Ceará. RESULTADOS: Nove pacientes (56,6%) eram do sexo masculino e 7 (43,8%) do sexo feminino. Onze (68,75%) pacientes encontravam-se na faixa etária entre 8 e 20 anos de idade. Os sintomas predominantes foram obstrução nasal unilateral (81,3%) e rinorréia purulenta (43,8%). A cirurgia mais comumente empregada foi a abordagem combinada externa e endonasal em 87,5% dos casos. A cirurgia do pólipo antrocoanal correspondeu a 21,6% de todos os procedimentos cirúrgicos realizados no mesmo período para remoção de pólipos nasais. Houve uma taxa de recidiva pós-operatória de 12,5%. CONCLUSÕES: Pólipo antrocoanal foi uma afecção que predominou entre crianças e adultos jovens. O acesso combinado externo e endonasal foi a abordagem cirúrgica mais empregada. Apesar da abordagem da origem do pólipo no seio maxilar, há a possibilidade de recidiva pós-operatória.
Objectives: to identify the clinical and obstetric situation of pregnant women who required emergency care, considering the adequacy of their requirement. Methods: this is a cross-sectional study, developed in the headquarters of the Mobile Emergency Care Services from a state in the Brazilian Northeast, through the analysis of 558 reports of obstetric patients attended in 2016. The magnitude of the associations was expressed by odds ratio and confidence intervals, considering a 5% significance level. Results: more than half (50.9%) requirements for emergency care were from women who went into labor (non-expulsive), especially among third trimester pregnant women (p < 0.000). Most clinical and obstetric parameters were normal. Conclusions: the inadequate demands for emergency care services reflect the excessive medicalization of the gestational process and shows how important it is to discuss the physiological symptoms that involve pregnancy, so that a more egalitarian and efficient urgency service can be offered.
Introdução:A significância prognóstica do marcador imunológico CD 20 no linfoma de Hodgkin clássico (LHc) ainda é incerta, particularmente no que se refere à refratariedade ao tratamento inicial. Objetivos: Avaliar a influência da positividade do marcador CD 20 na refratariedade do LHc ao tratamento poliquimioterápico inicial, com o esquema doxorubicina 25 mg/m², bleomicina 10 mg/m², vinblastina 6 mg/m² e dacarbazina 375 mg/m² (ABVD), no Ceará, Brasil. Materiais e métodos: Estudo analítico incluindo 97 pacientes com diagnóstico de LHc firmado entre janeiro de 2000 e dezembro de 2004. A análise foi realizada avaliando variáveis demográficas, clínicas e laboratoriais. Resultados: Foi evidenciada uma positividade do CD 20 em 38,1% dos pacientes. Na análise bivariada, CD 20 positivo (razão de chance [RC] = 4,02; intervalo de confiança [IC] = 1,09 -8,54; p = 0,02), a presença de sintomas B (RC = 4,02; IC = 1,18-17,51; p = 0,01) e a elevação da desidrogenase lática (mediana não-refratários 248,5 [200,5 -389,5]; mediana refratários 356 [208,5 -545]; p = 0,03) apresentaram relação de pior prognóstico quanto à refratariedade. Na regressão logística, o CD 20 positivo (RC ajustada = 3,6; IC = 0,99 -13,09; p = 0,05) e a presença de sintomas B (RC ajustada = 5,41; IC = 1,16 -25,34; p = 0,03) continuaram apresentando pior prognóstico. Discussão: Esses dados coincidem com a literatura, em que a positividade do marcador CD 20 está relacionada com pior resposta ao tratamento com ABVD. Conclusão: Os nossos dados indicam que o tratamento com ABVD não é completamente adequado para a abordagem terapêutica inicial deste subgrupo de pacientes e novas pesquisas precisam ser realizadas no sentido de aperfeiçoar o tratamento destes pacientes. resumo unitermos Linfoma de Hodgkin clássico Refratariedade CD 20abstract Introduction: The prognostic value of CD20 antigen expression in classical Hodgkin lymphoma (cHL) is uncertain, particularly regarding the refractoriness to first-line treatment.
