A via aérea humana é um sistema composto de importantes estruturas anatômicas que protegem e mantêm a permeabilidade. Este trabalho objetiva apresentar as manobras de desobstrução e acesso às vias aéreas em situações de emergência. Realizou-se uma revisão de literatura com busca nas bases de dados Scielo e Embase. Para isto, utilizou-se descritores "Airway Obstruction" e "Emergency" e “Dentistry” com 40 artigos triados. Nas manobras básicas, a manobra de Heimlich consiste na compressão infradiafragmática em “L”. A manobra de Jaw Thrust é caracterizada pela protrusão da mandíbula, enquanto que Chin Lift realiza-se a hiperextensão cervical e elevação mentual. Nas manobras avançadas a Intubação Endotraqueal garante o fluxo do ar nas vias aéreas por meio da aposição de uma cânula por via oral ou nasal, a Cricotireoidostomia é um procedimento invasivo que cria uma abertura através da membrana cricotireoidea, enquanto que a Traqueostomia consiste em um procedimento cirúrgico para a criação de uma abertura entre o 2⁰ e o 3⁰ anel traqueal. Por isso, a manobra de Heimlich é eleita como primeira alternativa nas situações de obstrução aguda seguida das manobras de Jaw Thrust, Chin Lift e a Intubação Endotraqueal. A Cricotireoidostomia é o procedimento invasivo de primeira escolha para o reestabelecimento da ventilação em situações de emergência, enquanto que a Traqueostomia, de caráter eletivo, demanda maior tempo de procedimento. Portanto, a obstrução aguda das vias aéreas superiores é uma complicação grave e de caráter emergencial, por isso deve o Cirurgião-Dentista estar preparado para reverter esse quadro rapidamente nessas situações.
Low-grade polymorphous adenocarcinoma is a malignant neoplasm of salivary glands with uncommon occurrence in the head and neck region, almost exclusively affecting minor salivary glands. While this condition has typical clinical pathological signs, it commonly presents low biological damage potential. The lesions occur more frequently among elderly females between the sixth and eighth decades of life, with a higher prevalence for the hard palate, soft palate and, most regularly, the upper lip and jugal mucosa. The clinical and histological differential diagnosis of this condition is between pleomorphic adenoma and adenoid cystic carcinoma. The most indicated surgical treatment is extensive surgical excision, eventually including resection of the underlying bone. Although infrequent, there may be metastasis for regional lymph nodes. Radical dissection of the neck is not indicated unless there is clinical evidence of cervical metastasis. The present study reports the case of surgical excision of polymorphous adenocarcinoma in the left maxillary tuberosity region. A 63-year-old white male patient sought the Maxillofacial Surgery and Traumatology Service at the Federal University of Pernambuco Clinics Hospital, complaining of a tumor-like lesion in his left maxilla, which gradually increased in volume. The patient further reported a history of surgery ten years before for removal of a tooth in the same region, upper left arch. Given the extent and complexity of the lesion, the surgical treatment in this case consisted of hemimaxillectomy.
Background: Intramedullary nailing of the fibula (FN) is a method of fixation that has proven to be useful for treating distal fibular fractures (DFs). FN minimizes soft tissue complications and provides similar stability to plating, with fewer hardware-related symptoms. Nevertheless, FN has been associated with syndesmotic malreduction and the incapacity of restoring length and rotation of the fibula. We aimed to evaluate the fibular position and syndesmotic reduction after fixation with FN compared with the uninjured ankle in the immediate postoperative period. Methods: Prospective cohort study. Patients with DF fractures treated with IN between January 2017 and January 2020 were included. Immediate postoperative bilateral ankle CT was obtained in all cases. Fibular rotation, length, and translation as well as syndesmotic diastasis were measured on both ankles and compared by 3 independent observers. Results: Twenty-eight patients were included (16 women). The mean age was 46 years (range 16-91). Fracture type distribution according to AO/ASIF classification included 19 patients with 44.B (67.9%), 8 patients with 44.C (28.6%), and 1 patient with a 44.A fracture (3.6%). No significant differences were identified considering fibular rotation ( P = .661), syndesmotic diastasis ( P = .147), and fibular length ( P = .115) between the injured and uninjured ankle. Fibular translation had statistical differences ( P = .01) compared with the uninjured ankle. The intraclass correlation coefficient showed an excellent concordance between observers except for fibular translation on the injured ankle. Conclusion: In this cohort, fixation of DF fractures with FN allows restoration of anatomical parameters of the ankle in terms of fibular rotation, length, and syndesmotic diastasis. However, fibular translation had significant differences compared with the uninjured ankle based on bilateral CT scan evaluation. Level of Evidence: Level II, prospective cohort study.
Lipoma é um tumor benigno de origem mesenquimal com etiologia incerta, possui predileção por homens, entre os 50 e 70 anos de idade, sendo a região de tronco e extremidades os sítios mais comuns. Clinicamente, apresenta-se como lesão bem circunscrita, móvel, macia à palpação, indolor e de base séssil ou pedunculada. O diagnóstico é majoritariamente clínico com a confirmação por meio de análise histopatológica e por exames de imagens, ao qual o tratamento de escolha é a ressecção cirúrgica conservadora. Este trabalho tem por objetivo relatar o caso de tratamento cirúrgico de lipoma extenso na região submandibular em uma paciente do sexo feminino, 67 anos de idade, que apresentava aumento de volume na região submandibular direita. Ao exame clínico, apresentou lesão bem delimitada, móvel, macia à palpação e indolor. Foi preconizada a realização de uma biopsia excisional. Sob anestesia geral, realizou-se a incisão submandibular de Risdon, divulsão dos tecidos subcutâneos e hemostasia dos vasos sangrantes. A cirurgia seguiu com a ressecção da lesão, toalete da cavidade e sutura por planos. A peça patológica foi encaminhada para o Serviço de Anatomopatologia do Hospital das Clinicas da Universidade Federal de Pernambuco, medindo 14cm x 6,5cm, com confirmação da hipótese de Lipoma. O caso foi acompanhado por seis anos apresentando bom resultado estético e funcional sem recidiva. Portanto, mesmo que não muito frequentes na região de cabeça e pescoço, cabe ao Cirurgião Buco Maxilo Facial conhecer as características desse tipo de patologia, bem como a forma adequada de tratamento para prevenir recidiva.
