ObjectivesTo comparatively analyze maternal and fetal factors and quality markers of blood samples in a public umbilical cord blood bank.MethodThis is a cross-sectional descriptive study that revisited 458 records of donations from September 2009 to March 2013 at the Hemocentro de Santa Catarina. The means of markers were used to define cutoff points for the quality of cord blood.ResultsMost donations came from women with ages between 18 and 29 years (62.8%), gestational age ≥ 40 weeks (55.2%), vaginal delivery (51.3%), primiparous (41.4%), and with male newborns (54.4%) weighing between 3000 and 3499 g (41.8%). The volume of the donations ranged from 71.6 to 275.2 mL, the total nucleated cell count ranged from 4.77 × 108 to 31.0 × 108 cells and CD34+ cells ranged from 0.05 to 1.23%. There were statistically significant differences in the volume with respect to gestation age > 38 weeks (p-value = 0.001), cesarean section (p-value < 0.001) and birth weight > 3500 g (p-value < 0.001). The total nucleated cell count was positively affected by cesarean section (p-value = 0.022) and birth weight > 3500 g (p-value < 0.001). There was no statistically significant difference between the variables and the percentage of CD34+ cells.ConclusionsDelivery route and birth weight influence the volume of cord blood and the total nucleated cell count. Gestational age influences only the volume of cord blood.
The aim of this report is to describe general and methodological characteristics of a cohort study in southern Brazil (Coorte Brasil Sul), aimed at understanding the impact of the first 1,000 days of life on children's health. It is a cohort study involving all children born in 2009 and their families living in the municipality of Palhoça, State of Santa Catarina, Brazil. Face-to-face interviews with parents at home using a structured questionnaire and children's physical and clinical examinations at schools have been carried out. Crosssectional analyzes, longitudinal comparisons and hierarquical regression analysis will allow understanding if the first 1,000 days of life can influence on 6-year-old children's health. The Coorte Brasil Sul is in its retrospective phase together with the children's physical data collection. Preliminary data (n=1270) related to nutritional status point to a high prevalence of overweight (16.4%) and obesity (15.5%). With the continuity of the study, it is expected to evaluate if the first phases of life can influence health during adolescence and in adult life, mainly in relation to chronic diseases.
Objective: To determine maternal and fetal risk factors associated with the birth of late preterm infants in comparison to those born at term. Methods: A case-control study was carried out in a tertiary center for high-risk pregnancies. For the cases, the study enrolled post-partum mothers and their respective newborns with gestational ages equal or greater than 34 weeks and less than 37 weeks. As controls, the post-partum mothers and their newborns with gestational ages of 37 weeks or greater were selected. The sample was calculated with a ratio of two controls for each case, resulting in 423 patients. Association studies were performed using the chi-square test or Fisher’s exact test and logistic regression analysis. Results: The variables associated with late prematurity were inadequate prenatal (Odds Ratio [OR] 1.23; confidence interval of 95% [95%CI] 1.12-1.34; p≤0.001), premature rupture of membranes (OR 4.98; 95%CI 2.66-9.31; p≤0.001), length of hospital stay ≥24 hours until birth (OR 0.18; 95%CI 0.06-0.52; p≤0.001), cesarean section (OR 2.74; 95%CI 1.69-4.44; p≤0.001) and small for gestational age newborn (OR 3.02; 95%CI 1.80-5.05; p≤0.001). Conclusions: Inadequate prenatal care and membranes’ premature rupture were found as factors associated with the late preterm birth. It is important to identify the factors that allow intervention with adequate prenatal care in order to reduce poor outcomes due to late prematurity.
SUMMARY OBJECTIVE: To analyze the temporal trend and factors associated with adolescent pregnancy during ten years in Brasil. METHODS: Data were extracted from the Live Births Information System, of the Brazilian Health System and included information regarding live births from adolescent mothers from 2006 to 2015. The overall proportion of gestation in adolescence and the specific proportions according to the characteristics analyzed were calculated using the standardized reporting coefficients and the simple linear regression method. The study was approved by the ethics committee of the University of Southern Santa Catarina (UNISUL). RESULTS: The general proportion of Live Births from adolescent mothers varied from 21.4% in 2006 to 18.1% in 2015. This reduction occurred because of the negative variation observed among mothers aged 15 to 19 years. The indigenous group was the only that did not present a reduction. There was an increase in the proportion of adolescents with between four and seven years of formal education and in the proportion of adolescents living with partners. There was a reduction in all Brazilian Regions and in large part of the Federation Units. CONCLUSION: The analysis of the temporal trend identifies a reduction in the proportion of live births among adolescent mothers in Brasil. However, there is a growing trend among some specific groups.
Objetivo: O estudo objetiva conhecer os fatores associados à realização da episiotomia no parto vaginal. Metodologia: Estudo transversal envolvendo 330 partos, de janeiro/2012 a dezembro/2013. Estudou-se a associação das variáveis independentes com a realização da episiotomia por meio de cálculo de razões de prevalência e seus respectivos intervalos de confiança, com nível de significância p < 0,05. Resultados: Na amostra estudada, 224 (67,9%) pacientes não realizaram episiotomia e 106 (32,1%) realizaram o procedimento. A idade média foi de 22,9 (± 5,9) anos e a episiotomia foi mais realizada naquelas com idade inferior a 20 anos [RP 1,61 (IC 95% 1,15; 2,25)] (p = 0,005). As mulheres com mais de 8 anos completos de estudo foram significativamente mais submetidas à episiotomia que as demais [RP 2,34 (IC 95% 1,70; 3,22)] (p < 0,001). The mean age was 22.9 ± 5.9 years and the episiotomy was more frequently performed in those younger than 20 years old [PR 1.61 (95% CI 1.15; 2.25)] (p = 0.005). Women with more than 8 years of education were significantly more likely to undergo episiotomy than the others [PR 2.34 (95% CI 1.70; 3.22)] (p < 0.001). Objective To evaluate the association between hormonal contraception and the appearance of human papillomavirus HPV-induced lesions in the uterine cervix of patients assisted at a school outpatient clinic -ObGyn outpatient service of the Universidade do Sul de Santa Catarina. Methods A case-control study, with women in fertile age, performed between 2012 and 2015. A total of 101 patients with cervical lesions secondary to HPV were included in the case group, and 101 patients with normal oncotic colpocytology, in the control group. The data were analyzed through the Statistical Package for the Social Sciences (SPSS, IBM Corp. Armonk, NY, US) software, version 24.0, using the 95% confidence interval. To test the homogeneity of the proportions, the chi-square (χ 2 ) test was used for the qualitative variables, and the Student t-test, for the quantitative variables.Results When comparing the occurrence of HPV lesions in users and non-users of combined oral contraceptives (COCs), the association with doses of 0.03 mg or higher of ethinylestradiol (EE) was observed. Thus, a higher probability of developing cervical lesions induced by HPV was identified (odds ratio [OR]: 1.9 p ¼ 0.039); and when these cases were separated by the degree of the lesion, the probability of these patients presenting with lowgrade squamous intraepithelial lesion was 2.1 times higher (p ¼ 0.036), but with no impact on high-grade squamous intraepithelial lesions and the occurrence of invasive cancer. No significant differences were found in the other variables analyzed.Conclusion Although the results found in the present study suggest a higher probability of the users of combined hormonal contraceptives with a concentration higher than 0.03 mg of EE to develop low-grade intraepithelial lesions, more studies are needed to conclude causality. Malocclusions are changes in the development of craniofacial structures of high prevalence. Associated risk factors are diverse and nutritional and non-nutritional suction habits are the most reported. The aim of this study was to estimate the prevalence of malocclusions in the mixed dentition and to study possible association with practices of breastfeeding and suction habits among Brazilian schoolchildren. A cross-sectional study involving a sample of 664 6-year-old children and their families was carried out. Data collection was done through interviews with mothers in the homes and oral examinations of children in schools. Multivariate analyses were performed using the Poisson regression with a robust estimator. The prevalence of overjet over 4 mm was 21.1% and of posterior crossbite was 12.2%; 91.9% of the children were breastfed, 79.0% used a nursing bottle and 49.4% used a pacifier. Significant and independent associations were observed between father's unemployment, private school, interruption of breastfeeding before the fourth month and pacifier use with certain malocclusions. High rates of malocclusion were found in the studied sample. Socioeconomic conditions, breastfeeding practices and suction habits were shown to be statistically associated with their occurrence.
RESUMO Objetivo Identificar a prevalência e fatores associados à síndrome de Burnout nos estudantes de Medicina. Métodos Estudo transversal com estudantes de Medicina de uma instituição privada de ensino na região Sul do Brasil. Utilizou-se o Maslach Burnout Inventory – Student Survey para a identificação da síndrome e um instrumento para a coleta de informações sociodemográficas, hábitos, rotina e situação acadêmica. Realizou-se análise descritiva dos dados e avaliaram-se associações entre as informações obtidas a partir do instrumento de coleta com a presença da síndrome de Burnout, utilizando-se razões de prevalência por meio do teste qui-quadrado ou teste de Fisher. Associações com valor de p < 0,25 foram inseridas em um modelo multivariado, em formato step-forward, utilizando-se regressão de Poisson. Foram consideradas significativas associações com valor de p < 0,05. Resultado Quinhentos e vinte e dois (88,2%) alunos participaram da pesquisa. A prevalência da síndrome de Burnout foi de 12,3%. As associações com síndrome de Burnout que possuíram valor de p < 0,25 foram “não viver com parceiro”, “residir sozinho”, “dormir até cinco horas por dia”, “ter até cinco horas semanais de lazer”, “possuir pensamento suicida” e “realizar acompanhamento psicológico”. Após inserir essas variáveis no modelo multivariado, identificou-se apenas 10% de maior probabilidade de síndrome de Burnout no estudante que relatou pensamento suicida, sendo esse o único fator associado encontrado à ocorrência de burnout (p < 0,001). Conclusão A prevalência de síndrome de Burnout entre os acadêmicos de Medicina foi de 12,3%, e apenas a presença de pensamentos suicidas durante o curso foi associada à sua ocorrência.
