O uso da Tomografia Computadorizada Cone Beam (TCCB) na Odontologia tem aumentado nos últimos tempos. Entre as suas utilizações se encontra a medição da largura e profundidade da arcada dental. Este trabalho realizou uma revisão da literatura sobre o tema e verificou que a TCCB pode ser empregada para tais propósitos, bem como foi observada uma variação nos pontos de referências para obter tais medidas. IntroduçãoA expansão da arcada dentária tem sido tanto incentiva (1), como criticada (2), e muito tem sido pesquisada nesta área, avaliando-se a os efeitos e a estabilidade dos mais diversos aparelhos e do próprio crescimento nas medidas de largura, comprimento e profundidade da arcada dental (3, 4).A determinação das medidas de largura, comprimento e profundidade do arco dental tem sido feita geralmente em modelos de gesso ou em radiografias cefalométricas 2D (lateral de crânio e antero-posterior). Com a tomografia computadorizada de feixe cônico de baixa dose de radiação (Cone Beam) é possível se obter imagens radiográficas acuradas que permitem aos clínicos e pesquisadores avaliar estas medidas em uma imagem radiográfica de 3D (5,6).O objetivo deste trabalho foi realizar uma revisão de literatura sobre as principais medidas utilizadas na avaliação da largura do arco dental bem como da utilização da tomografia computadorizada de baixa dose por feixe cônico (Cone Beam) na obtenção destas medidas. Revisão de LiteraturaAlterações que ocorrem no arco dental com o crescimento e com o tratamento são de interesse dos ortodontistas e devem ser criteriosamente consideradas durante o planejamento e execução do tratamento ortodôntico. Estas alterações podem estar relacionadas com a estabilidade ou não do tratamento ortodôntico e tal situação é de extrema importância tanto para o ortodontista como para o paciente (3). Bishara et al.(3) estudaram as alterações na largura da arcada ocorrida das 6 semanas até os 45 anos de idade e verificaram que a largura inter-caninos e a largura inter-molares aumenta significativamente entre os 3 e os 13 anos de idade em ambas arcadas. Após a erupção completa da dentição permanente, ocorria uma ligeira diminuição nas larguras dos arcos, mais na largura inter-caninos do que na inter-molares. A largura inter-caninos inferiores geralmente se estabelecia em volta dos oito anos de idade, ou seja, geralmente após a erupção dos quatro incisivos inferiores.
Ponticulus Posticus (PP) significa “pequena ponte posterior” em latim. É definida como uma pequena ponte óssea anormal, que é gerada entre a parte posterior do processo articular superior do osso occipital e a porção posterolateral da margem superior do arco posterior do Atlas. A associação entre PP e algumas anomalias dentárias já foi demonstrada, sendo a impacção palatina de caninos superiores uma delas. Objetivos: o presente estudo avaliou os efeitos das variáveis gênero, idade e padrão vertical, comparado na frequência e tipo de PP (forma parcial ou completa) em uma amostra retrospectiva transversal de radiografias cefalométricas laterais de pacientes saudáveis, ortodonticamente não tratados. Materiais e métodos: a amostra incluiu radiografias laterais de 300 pacientes (n = 300) - (124 homens e 176 mulheres) de uma clínica odontológica da cidade de Lages/SC. A presença de Ponticulus Posticus (parcial ou completa) foi avaliada, levando-se em consideração gênero, idade e padrão vertical. Após análise descritiva dos dados, os mesmos foram submetidos ao teste chi quadrado (� = 0,05). Resultados: a prevalência geral de Ponticulus Posticus foi de 11% (30 de 300) com as formas parcial e completa verificadas, respectivamente, em 6,3% (19 de 300) e 4,7% (11 de 300) da amostra. Não foram encontradas associações com gênero (p = 0,611) e idade (p = 0,761). O padrão vertical teve relevância significativa com a presença de PP (p = 0,007). Conclusão: há uma associação positiva entre o padrão vertical do tipo braquifacial com uma maior prevalência de Ponticulus Posticus em suas formas parcial e completa.
INTRODUÇÃO: As anomalias dentárias são um desafio a ser administrado pelos ortodontistas em um grande número de pacientes. Os dentes supranumerários correspondem a 8-9% das anomalias, e não parecem estar associados a outras alterações de forma, estrutura, tamanho ou posição. São de etiologia genético-ambiental ou causados por hiperatividade da lâmina dentária, geralmente de manifestação única e, quando múltiplos, estão comumente relacionados à disostose cleidocraniana, síndrome de Gardner ou fissuras labiopalatinas. Muitos pré-molares supranumerários têm formação tardia, aproximada- mente 7 a 11 anos após a formação da série numerária, o que coincide com o momento do tratamento ortodôntico para alguns pacientes, com a possibilidade de formação recorrente, razão pela qual é sugerido o monitoramento radiográfico durante e após a Ortodontia. A presença de supranumerários pode dificultar a mecânica ortodôntica, provocar reabsorção das raízes dos dentes vizinhos e causar diastemas, mas a degeneração cística é relativamente rara. OBJETIVOS: Revisar a literatura sobre etiologia, diagnóstico e tratamento de supranumerários com formação tardia, ilustrando com três casos clínicos. CONCLUSÃO: Não há um consenso em relação ao melhor momento de intervenção. A decisão entre a remoção cirúrgica ou o acompanhamento clínico e radiográfico deve ser tomada individualmente, levando em conta a possibilidade de intercorrência cirúrgica e a real necessidade de exodontia.
