The development of a text messaging (SMS) based intervention for the aftercare treatment of bulimia nervosa is reported. The programme is offered to bulimic patients for 6 months following discharge from inpatient psychotherapy in a German hospital for psychosomatic medicine. The intervention consists of weekly messages from the patients on their bulimic symptomatology and a corresponding weekly feedback that is a mixture of pre-programmed parts and individually tailored information. An ongoing pilot study is described, designed to investigate the acceptance, the practicability, and the effectiveness of the intervention. Preliminary results indicate that the programme is well-accepted and gives support to bulimic patients after finishing inpatient treatment. Advantages and limitations of the use of the SMS intervention programme in the treatment of bulimia nervosa are discussed.
The use of information technology (IT) for the purpose of improving psychotherapy outcomes in the context of routine clinical practice is described. IT efforts from two research programs, one in Germany and the other in the United States, are based on evidence that not all patients who enter treatment have a positive outcome and that continuous monitoring of patient treatment response with immediate feedback to therapists can be used to increase the likelihood of success for the poorly responding client. Such monitoring and feedback can best be accomplished by IT methodologies. Suitable IT systems are described, and their strengths and limitations are highlighted. Evidence is presented that supports the positive value of such efforts for improving patient treatment response.
Neglecting differences in national norms can lead to substantial misinterpretation of empirical findings in the diagnosis of eating disorders and the evaluation of treatment outcome.
The main objective of this commentary is to stimulate a debate on service allocation and the implications for clinical practice and research. Although efficacious treatments for patients with ED have been developed over the past decades, nonetheless many patients do not benefit substantially during treatment and many fail to maintain treatment gains after termination. Specific stepped care strategies have been suggested to improve this situation. These include secondary level treatments for those who do not respond to primary treatment and maintenance treatments for those who do respond. Usually, the primary as well as the secondary treatment are standardized and thus, follow an implicit service allocation strategy that is independent of the individual patient. This scenario is similar to the general situation in psychotherapy service provision. We therefore look to the psychotherapy service provision literature for guidance and propose an alternative strategy that aims for a better match between individual patient's needs and treatment duration.
Zusammenfassung. Mit dem KPD-38 steht ein neues Instrument für die Qualitätssicherung und das Ergebnismonitoring in der Psychotherapie und psychosomatischen Medizin zur Verfügung. Im vorliegenden Beitrag werden die Entwicklung des KPD-38 und erste Erfahrungen aus dem Einsatz in der Qualitätssicherung beschrieben. Das Instrument wurde einer Bevölkerungsstichprobe (N = 2066) sowie einem Patientenkollektiv aus der stationären Psychotherapie (N = 351) vorgelegt. Es wurden Prüfungen der Validität, Reliabilität, der computergestützten Vorgabe, Sensitivität und Spezifität durchgeführt. Neben testökonomischen Gesichtspunkten sprechen insbesondere die deutlichen Belege der Validität des KPD-38 für den Einsatz in diesem speziellen Anwendungsbereich. Die vielversprechenden Hinweise auf seinen Nutzen für die Behandlungs- und Versorgungsplanung durch therapiebegleitende Rückmeldungen lassen weitere Detailstudien als lohnend erscheinen.
Zusammenfassung. Theoretischer Hintergrund: Unter klinisch bedeutsamer Veränderung (KBV) wird eine reliable Verbesserung bei Erreichen eines Zielbereich verstanden. Da KBV zwar ein operationales aber kein statistisches Konzept darstellt, ist die Wahl des Zielbereiches aus statistischer Sicht arbiträr und verlangt eine inhaltliche Begründung. Fragestellung: Welche Bedeutung hat die Wahl des Zielbereiches für die Validität der KBV? Methode: Grundlage der Analysen bilden SCL-90-R (GSI) Daten einer repräsentativen Bevölkerungsstichprobe (N = 2.145) und einer Patientenstichprobe (N = 1.665). Untersucht werden die Zusammenhänge zwischen Zielbereich, KBV-Rate, Sensitivität und Spezifität sowie zwischen KBV und Suizidgedanken, Arbeitsunfähigkeit und Weiterbehandlung. Ergebnisse: Die vorgeschlagene Metdode führt zu einem Zielbereich mit einer Obergrenze zwischen 0,6 und 0,8 beim GSI der SCL-90-R, der am ehesten den Erwartungen bezüglich Fehlklassifikationsraten und Kriteriumsvalidität entspricht. Schlussfolgerungen: Der Zielbereich beeinflusst maßgeblich Fehlklassifikationsraten und Kriteriumsvalidität.
