RESUMO: Programas de treino funcional podem colaborar na redução de incapacidades, quedas, problemas emocionais e sociais em idosos. O objetivo do estudo foi verificar o efeito de um programa de oito semanas de exercícios funcionais em idosas da comunidade, avaliando o impacto nas atividades instrumentais de vida diária (AIVD) e no equilíbrio unipodálico. Deste estudo quasi-experimental participaram sete idosas da comunidade (71±8,1 anos), sem distinção de raça e/ ou condição social, excluindo-se aquelas com alterações cognitivas, doenças agudizadas ou neurológicas, quadro álgico na coluna ou articulações, labirintite, distúrbios visuais não-compensados, fratura prévia no último ano e utilização de apoio para a marcha. Todas responderam ao questionário de Lawton e se submeteram ao teste de apoio unipodálico antes e depois do programa, administrado três vezes por semana. O programa consistia em exercícios de marcha em flexão plantar, dorsiflexão, permanência em alternância de apoio unipodálico, marcha lateral com flexão de quadril aumentada e marcha tandem. Os resultados indicam melhora (p=0,042) no nível funcional (índice de Lawton) após o programa e uma tendência à melhora no equilíbrio unipodálico, embora não-significante (p>0,105). O programa de exercícios proposto produziu pois melhora no desempenho das AIVD e uma tendência à melhora do equilíbrio estático, sugerindo ser relevante para aprimorar a autonomia das idosas. The aim of this study was to assess the impact of an eight-week functional exercise program on aged women's instrumental activities of daily living (IADL) and one-foot stance balance. Irrespective of race and/or social condition, 7 community-dwelling elderly women (aged 71±8.1) were selected, being excluded those with cognitive impairment, acute or neurological diseases, spine or joint pain, labyrinth inflammatory disease, visual impairment, fractures in the previous year, and use of gait aid. All subjects were assessed by the Lawton IADL scale and the one-foot stance balance test, before and after the program. Exercises were performed three times a week and consisted of gait exercises in plantar flexion, dorsiflexion, alternation of sustained one-foot stance, sideways gait, gait with increased hip flexion, and tandem gait. Results show a functional improvement in IADL (p=0.042) as measured by the Lawton scale, and a trend towards better static one-foot stance balance, though not significant (p>0.105). The proposed program of functional exercises may thus be said to bring about improvement in older women's functional performance and a trend towards improvement on static balance, suggesting its relevance to increase elderly women's autonomy.
ResumoO objetivo do estudo foi verificar a prevalência da síndrome de fragilidade e sua relação com a capacidade e o desempenho funcional em idosos frequentadores de grupos de convivência. O perfil de fragilidade foi determinado por meio dos critérios perda de peso não intencional; exaustão; baixa velocidade de marcha; baixa força de preensão manual e baixo consumo calórico. A capacidade funcional foi avaliada pelo Timed Up and Go (TUG) e o desempenho funcional, pelo índice de Lawton. O perfil de fragilidade foi apresentado em frequências e a associação foi analisada pelo teste de correlação de Spearman (α = 5%). Participaram 117 idosos (70,1 ± 7,3 anos). A maioria foi classificada como pré-frágil (51,3%). A média do TUG para os não-frágeis (NF) foi 11,3 s (± 1,9), pré-frágeis (PF) 12,7 s (± 3,4) e frágeis (FF) 16,7 s (± 3,3). O escore médio do Lawton em NF foi 29,8 (± 0,6), PF 28,4 (± 3,3) e FF 27,4 (± 2,8). Houve associação das classes de fragilidade com a capacidade e o desempenho funcional (p= 0,001). Houve maior prevalência de idosos pré-frágeis na amostra pesquisada, e idosos frágeis e pré-frágeis apresentaram piores desempenhos nos testes funcionais. Esses resultados reforçam o pressuposto de que a fragilidade compromete a funcionalidade em idosos. The frailty phenotype was identified with the weight loss; decrease of gait speed; decrease of physical activity; decrease of manual force and exhaustion. This article aims to categorize elderly non-frail (NF), pre-frail (PF) and frail (FF) as to fast and slow gait speed. Compare NF, PF and FF, and analyze associations between fast or slow gait speed with clinical, functional and mental factors. 5,501 elderly (65 years or over; to the Frailty in Brazilian Older People Study), classified as NF, PF and FF (Fried´s frailty phenotype) and, in relation to fast gait speed (≥ 0.8m/s) and slow (< 0.8m/s). Age, sex, body mass index, muscular strength, advanced, instrumental and basic activities of daily living, falls, fear of falling and depressive symptoms were evaluated. Logistic regression analysis investigated associations between variables. The proportion of the slow elderly increased with fragility (NF = 12.39%, PF = 37.56%, FF = 88.83%, p < 0.01). Be woman, performance in activities of daily living, muscle strength and fall were associated with fragility syndrome. The association between frailty and adverse health outcomes reinforces its primacy as an indicator of the functional health of the elderly. Functional capacity, muscular strength, and falls should be evaluated considering their potential for reversibility.
RESUMO A velocidade da marcha (VM) tem sido considerada um marcador de saúde em idosos capaz de predizer desfechos adversos de saúde, mas a compreensão de fatores associados a ela ainda é limitada e controversa. O objetivo deste trabalho é identificar desfechos adversos de saúde relacionados ao declínio da velocidade de marcha em idosos comunitários. Trata-se de estudo transversal e multicêntrico, que avaliou o autorrelato de doenças crônicas e de hospitalização no último ano, polifarmácia e velocidade de marcha. Utilizou-se análise de regressão logística para estimar os efeitos de cada variável independente na chance de os idosos apresentarem declínio na velocidade de marcha inferior (VM<0,8m/s) (α=0,05). Participaram da pesquisa 5.501 idosos. A menor velocidade da marcha mostrou-se associada a portadores de doenças cardíacas (OR=2,06; IC: 1,67-2,54), respiratórias (OR=3,25; IC: 2,02-5,29), reumáticas (OR=2,16; IC: 1,79-2,52) e/ou depressão (OR=2,51; IC: 2,10-3,14), hospitalizados no último ano (OR=1,51; IC: 1,21-1,85) e polifarmácia (OR=2,14; IC: 1,80-2,54). Assim, os resultados indicaram que idosos com velocidade de marcha menor que 0,8m/s apresentam maior risco de eventos adversos de saúde. Dessa forma, sugere-se que a velocidade de marcha não seja negligenciada na avaliação de idosos comunitários, inclusive na atenção básica.
ResumoObjetivo: Avaliar a capacidade funcional de idosos com e sem medo de cair. Materiais e métodos: Sessenta e um idosos foram divididos em dois grupos segundo os escores obtidos na Falls Efficacy Scale International (FES-I-Brasil) e foram avaliados quando à capacidade funcional ao realizar o teste Short Physical Performance Battery (SPPB), que avalia força, equilíbrio e velocidade da marcha. Para as comparações entre os grupos, foi utilizado o teste t para amostras independentes e o teste de Pearson para investigar possíveis correlações entre os escores da FESI-Brasil e demais variáveis (p ≤ 0,05). Resultados: Trinta e sete idosos atingiram nota ≥ a 23 na FES-I-Brasil e foram alocados no Grupo 1 (G1, n=37; idade= 70,88 ± 0,88 anos), e 24 idosos foram classificados como sem medo de cair e foram alocados no Grupo 2 (G2, n= 24; idade= 69,33 ± 0,90). Os grupos foram diferentes em relação à velocidade da marcha (G1: VM=3,10 ± 0,09 m/s e G2: VM= 0,93 ± 0,03; p 0,01) e ao número de doenças (G1= 4,13 ± 0,21 doenças e G2=3,00 ± 0,44 doenças; p 0,01). Houve correlação entre a FESI e número de doenças (FESI x VM p 0,03 r -0,61 e FESI x doenças p 0,01 r 0,58).
