The laparoscopic approach appears to be as effective as open repairs in the treatment of ventral hernias. Advanced surgical skill, laparoscopic experience and high technology are mandatory factors for successful ventral hernia repair.
Oxidative stress parameters and nitric oxide (NO) values were determined in 27 newly diagnosed Basedow patients before and after 1 mo of propylthiouracil (PTU) therapy and in 15 healthy controls. Basedow patients exhibited increased triiodothyronine (T3) and thyroxine (T4) and decreased thyroid-stimulating hormone (TSH) values compared to controls. Significantly higher thiobarbituric acid-reactive substances (TBARS), NO and glutathione (GSH) levels, and CuZn superoxide dismutase (CuZn SOD) activity were found in Basedow patients in comparison to controls, regardless of sex. Treatment with PTU (3 x 100 mg/d for 30 d) was effective in decreasing T1 and T4 and increasing TSH levels. Significantly decreased NO and TBARS and increased GSH and CuZn SOD levels were observed in PTU-treated Basedow patients compared to pre-PTU administration. PTU-treated patients compared to controls still exhibited significantly higher T3 and lower TSH values and higher NO, TBARS, GSH, and CuZn SOD levels. The induced antioxidant defense and decrease in NO) values in response to PTU therapy emphasizes the role of PTU as an antithyroid drug, where the ability to diminish hyperthyroidism results in decreased catabolism and lower oxidant generation.
AMAÇBu çalışmada, antioksidan özelliği bilinen glutaminin kullanımının iskemi-reperfüzyon sonrası oluşabilecek oksidasyon hasarı üzerindeki etkilerini araştırmayı amaçladık.
GEREÇ VE YÖNTEMÇalışma grubu Wistar Albino sıçanlarla oluşturuldu. Grup 1, abdominal kompartman grubuydu. Anestezi altındaki sı-çanların karnına basınç uygulandı. Gaz boşaltıldıktan 3 gün sonra sıçanlar sakrifiye edildi, karaciğer, bağırsak ve akciğer doku parçalarında oksidatif hasar parametresi olarak malonildialdehit (MDA) ve glutatyon (GSH) seviyeleri ile enflamatuvar parametre olarak miyeloperoksidaz (MPO) aktivitesi ölçüldü. Alınan kan örneklerinde serum alaninaminoasit transferaz (ALT) ve aspartat-aminoasit transferaz (AST) seviyeleri ölçüldü. Grup 2, abdominal kompartman ve glutamin uygulanan gruptu. Sıçanlara 1 hafta boyunca yüksek doz glutamin mide gavajı yolu ile verildi. Gavaj işlemi sonrası ilk gruptaki batın içi basınç uygulaması yapıldı, 3 gün daha glutamin verilen sıçanlar sakrifiye edildi, Grup 1'deki işlemlerin aynısı yapıldı. Grup 3 glutamin grubuydu. Grup 4 sham kontrol grubuydu.
BULGULARAbdominal kompartman yaratılan sıçanlarda MDA düzeyi ve MPO aktivitesi kontrol grubuna göre anlamlı oranda artmıştı. Glutamin ile MDA düzeyi ve MPO aktivitesi azaldı, GSH seviyesi arttı.
SONUÇOksidatif hasar parametrelerine bakıldığında, glutaminin, reperfüzyon hasarını azaltıcı etkisinin antienflamatuvar ve antioksidan etkilerine bağlı olduğu düşünülmüştür.
AIM:Single-port laparoscopic surgery (SILS) has become increasingly popular during the last decades. This prospective study was undertaken to evaluate the feasibility of single-port laparoscopic splenectomy compared with conventional multiport laparoscopic splenectomy.MATERIALS AND METHODS:Between February 2, 2009 and August 29, 2011, a total of 40 patients with the diagnosis of immune thrombocytopenic purpura were included to study. Patients were alienated into two groups according to the procedure type including SILS and conventional multiport splenectomy.RESULTS:There were 19 patients in group 1, and 21 in group 2. Operative time was significantly shorter in group 1 versus group 2 (112.4 ± 13.56 vs 71.2 ±18.1 minutes, respectively, P < 0.05). One patient in group 1 had converted to laparatomy due to preoperative bleeding. Postoperative pain analyses (VAS Score) revealed superiority of SILS in the early post-operative days (P < 0.05).CONCLUSIONS:SILS splenectomy is a safe and effective alternative to standard laparoscopic splenectomy.
