Circumcision is one of the most frequent procedures performed by pediatric surgeons. A dorsal penile nerve block (DPNB) is commonly used for perioperative analgesia. We report the case of a 16-year-old boy with ischemia of the glans who presented on the first postoperative day after circumcision under DPNB (0.25% bupivacaine) at an outside hospital. The patient underwent immediate angiography under sedation. An intra-arterial spasmolysis was performed with alprostadil and nitroglycerine and a sufficient perfusion of the glans penis was confirmed. Subsequently, systemic sildenafil, arginine, and heparin were given. Following this 3-day medical treatment, ischemia resolved completely. Our case emphasizes the role of invasive angiography in the diagnostic workup and the therapeutic possibilities of local spasmolysis, systemic vasodilatation, and anticoagulation.
We established a small animal model for neonatal laparoscopy. A pressure of 2 mmHg and flow of 0.5 L/minutes induced physiologic alterations but was well tolerated by the animals. These settings can be used in future studies on the impact of the capnoperitoneum in neonatal MIS.
This study showed an increase of perioperative blood glucose (normoglycemic ranges) after single dose of dexamethasone, but no BMI-dependent effect was observed in children. Therefore, low-dose dexamethasone may be used in obese children for PONV prophylaxis.
ZusammenfassungDie ulzeroglanduläre Tularämie ist eine sehr seltene Erkrankung, die sich häufig mit unspezifischen Symptomen wie Fieber, Abgeschlagenheit und einer lokalen Lymphadenopathie manifestiert. Unbehandelt geht sie mit einer Mortalität von 30 % einher. Histopathologisch zeigt sich eine nekrotisierende granulomatösepitheloidzellige Lymphadenitis. Wir berichten über drei Schulkinder aus Mitteldeutschland, die sich sowohl akut krank als auch wenig beeinträchtigt mit einer inguinalen beziehungsweise axillären Lymphadenopathie vorstellten und operativ versorgt werden mussten. Post-operativ bestätigte sich eine akute Infektion mit Francisella tularensis durch Arthropoden. Unter antibiotischer Therapie mit Ciprofloxacin kam es bei allen Patienten zur vollständigen Ausheilung.Anhand der Fallbeispiele werden die klinischen und diagnostischen Herausforderungen der Tularämie im Kindesalter erläutert und ihre oft unterschätzte Rolle in nicht-endemischen Ländern wie Deutschland diskutiert. Bei einer Lymphadenopathie mit protrahiertem Krankheitsverlauf – insbesondere nach Arthropodenstichen – sollte immer an eine Tularämie als seltene Ursache für eine Lymphadenopathie gedacht werden.
