Posttraumatic injury pain is commonly treated with oral nonsteroidal anti-inflammatory drugs. However, oral nonsteroidal anti-inflammatory drugs cause several adverse events, with topical formulations arising as an important alternative. Therefore, we aimed at evaluating the efficacy and safety of loxoprofen patch (LX-P) in the treatment of patients with posttraumatic pain. This phase III, randomized, double-blind, noninferiority study enrolled Brazilian patients aged 18 to 65 years diagnosed with lower and upper limb posttraumatic injury who were experiencing moderate or severe pain. Patients were assigned to active LX-P or to loxoprofen tablet (LX-T), and pain intensity was measured based on a visual analog scale score variation after 7 days of treatment. Data on clinical symptoms, rescue medication use, and adverse events were also collected. Visual analog scale score variation was compared using a 10% noninferiority margin. Two hundred forty-two patients were randomly assigned to LX-P (n = 123) or to LX-T (n = 119). The results showed a reduction in pain after 7 days of treatment: −49.96 (n = 118; SE 1.7) in the LX-P and −47.71 (n = 117; SE 1.6) in the LX-T groups (difference of −2.25; 95% CI: −5.97 to 1.47; P = 0.23). On the safety analysis, the LX-T group presented twice as many patients with treatment-emergent adverse events as the LX-P group (30.8% and 14.2%, respectively). A sensitivity analysis demonstrated that rescue medication use has not affected the primary end point. This study showed that LX-P has a comparable efficacy to LX-T, but with a better safety profile, being a therapeutic option for the treatment of posttraumatic injury pain.
OBJETIVO: Apresentar o perfil epidemiológico dos pacientes portadores de doença vertebral metastática sintomática de serviço público de atenção terciária na região do ABC. MÉTODO: Avaliamos de Janeiro de 2008 a Janeiro de 2011, 55 pacientes com diagnóstico de metástase vertebral e coletamos dados de idade no diagnóstico de lesão metastática, sexo, tipo de tumor, topografia vertebral e sintomatologia. A evolução da doença foi observado nas datas: biópsia da neoplasia primária; início de sintomas vertebrais; diagnóstico de doença vertebral metastática (imagem) e data da cirurgia. RESULTADOS: A idade dos pacientes variou de 28 a 85 anos; 40% homens e 60% mulheres. Os principais tumores foram carcinoma de mama (32,7%), mieloma múltiplo (25,4%) e carcinoma de próstata (14,5%). Observamos 25 pacientes (45,4%) com lesões na coluna torácica; 13 pacientes (23,6%) lombares ou sacrais; 11 pacientes (20%) difusas e 6 pacientes (10,9%) cervicais. 34 pacientes (61,8%) apresentavam somente dor, os demais (38,2%) apresentavam também alteração neurológica. O intervalo entre a lesão primária e a manifestação clínica de lesão vertebral apresentou mediana de 190 dias; entre a sintomatologia na coluna e o diagnóstico por imagem teve mediana de 70 dias; aos submetidos a cirurgia, entre o diagnóstico e o procedimento foi de 288 dias. CONCLUSÃO: Observamos os dados epidemiológicas, compatíveis a literatura: predomínio do sexo feminino (60%), com idade média de 55 anos; metástases predominantemente toracolombares (69%) por neoplasia de mama, mieloma múltiplo e próstata (72%). Observamos ampla variação no intervalo de tempo na descrição cronológica dos eventos clínico-diagnósticos e cirúrgicos.
ResumoOs autores apresentam dois irmãos com diagnóstico de analgesia congênita, com suas características clínicas e evolução. Essa doença é rara, apresenta alta morbidade e gera complicações osteoarticulares de difícil solução. O objetivo dos autores foi ressaltar a importância do diagnóstico tanto para o tratamento de suas afecções secundárias, quanto para seu aspecto jurídico. AbstractThe authors present two brothers with congenital pain insensitivity, with their clinical characteristics and evolution. This disease is rare, has high morbidity and originates complex osteoarticular complications. The aim of the authors was to emphasize the value of the diagnosis for a better treatment and to avoid legal problems to the parents.
