Objective To describe nursing workload in Intensive Care Units (ICU) in different countries according to the scores obtained with Nursing Activities Score (NAS) and to verify the agreement among countries on the NAS guideline interpretation. Method This cross-sectional study considered 1-day measure of NAS (November 2012) obtained from 758 patients in 19 ICUs of seven countries (Norway, the Netherlands, Spain, Poland, Egypt, Greece and Brazil). The Delphi technique was used in expertise meetings and consensus. Results The NAS score was 72.8% in average, ranging from 44.5% (Spain) to 101.8% (Norway). The mean NAS score from Poland, Greece and Egypt was 83.0%, 64.6% and 57.1%, respectively. The NAS score was similar in Brazil (54.0%) and in the Netherlands (51.0%). There were doubts in the understanding of five out 23 items of the NAS (21.7%) which were discussed until researchers’ consensus. Conclusion NAS score were different in the seven countries. Future studies must verify if the fine standardization of the guideline can have a impact on differences in the NAS results.
RESUMO Objetivo Investigar o estresse emocional, o coping e burnout da equipe de enfermagem e a associação com fatores biossociais e do trabalho em Unidade de Terapia Intensiva (UTI). Método Estudo transversal, realizado em oito UTI de hospital-escola, do município de São Paulo, em 2012. Coletaram-se dados biossociais e de trabalho dos profissionais, juntamente com Escalas de Estresse no Trabalho, Coping Ocupacional, Lista de Sinais e Sintomas e Inventário Maslach de Burnout. Resultados Participaram da pesquisa 287 sujeitos, predominantemente mulheres, com companheiro e filhos. O nível médio de estresse e coping controle foram prevalentes (74,47% e 79,93%, respectivamente) e a presença de burnout em 12,54%. Fatores associados ao estresse referiram-se às condições de trabalho. Ter companheiro, atuar em UTI Clínica e gostar do trabalho foram fatores de proteção para coping prevalente, enquanto que horas de sono adequadas foi fator de proteção para burnout. Conclusão O controle do ambiente de trabalho e o sono adequado são fatores decisivos e protetores para enfrentamento das situações de estresse ocupacional.
Objective: to analyze the influence of workload, stress, Burnout, work satisfaction, the nursing team's perception of the care environment, and the presence of adverse events in a Trauma Intensive Care Units.Method: an observational study conducted at the Intensive Trauma Therapy Unit 195 patients were prospectively followed for the collection of incidents, and instruments were used to measure Nursing workload, stress, Burnout, job satisfaction and work environment assessment by the nursing team. Data were analyzed using appropriate statistics for the study.Results: we observed 1,586 incidents, predominantly no harm incidents (78.44%). Among the nursing staff, 77.40% had average levels of stress; 17.00% presented Burnout; 56.6% were dissatisfied and considered the environmental characteristics to be inadequate. Nursing workload was high (73.24%). An association between incident and length of stay was found. No harm incidents were associated with nursing workload. Método: estudio observacional en la Unidad de Cuidados Intensivos de Trauma. Para la recolección de los incidentes fueron seguidos prospectivamente 195 pacientes siendo utilizados instrumentos para medir la carga de trabajo de enfermería, el estrés, el Burnout, la satisfacción laboral y el trabajo de evaluación del entorno de trabajo por parte del personal de enfermería. Los datos se analizaron usando el estudio estadístico apropiado. ConclusionResultados: ocurrieron 1.586 incidentes predominantemente incidentes sin daño (78,44%). Entre el personal de enfermería, 77.40% tenían niveles medios de estrés; 17,00% mostró Burnout; y el 56,6% estaban insatisfechos y consideraron inadecuadas las características ambientales. La carga de trabajo de enfermería fue alta (73,24%). Se observó una asociación entre la incidencia y la duración de la estancia. Incidentes sin lesiones se asociaron con la carga de trabajo de enfermería.Conclusión: La identificación de factores asociados puede prevenir la aparición de incidentes. The study aimed to describe the reports of heart failure patients on the factors that cause difficulties to sleep and the association of these factors with the quality of sleep. This crosssectional study involved a non-probabilistic sample of 400 patients (mean age 57.8 years, 64.8% were men, average education of 6.1 years, 82.5% in functional class II or III) with heart failure. The main factors associated with sleeping difficulty were: nocturia, interrupted sleep at night and breathing difficulty. Sleeping difficulties in heart failure patients are diverse and there is an association between these difficulties and quality of sleep. Most of these disorders warrant professional nursing interventions.Descriptors: Nursing; Sleep; Heart Failure. Dificultades para dormir por los pacientes con insuficiencia cardíacaEl objetivo del estudio fue describir los informes de los pacientes con insuficiencia cardíaca en los factores que causan dificultades para dormir y la asociación de estos factores con la calidad del sueño. Se trata de un estudio transversal con muestra no probabilística de 400 pacientes (edad media de 57,8 años, 64,8% eran hombres, el promedio de escolaridad de 6,1 años, 82,5% en clase funcional II o III) con insuficiencia cardíaca. Los principales factores asociados con dificultad para dormir son: nicturia, interrupción del sueño en la noche y dificultad para respirar. Las dificultades en el sueño de los pacientes con insuficiencia cardíaca son diversas y existe una asociación entre estas dificultades y la calidad del sueño. Estos trastornos, la mayoría son susceptibles de intervención por el personal de enfermería.
A segurança do paciente representa um desafio para a excelência da qualidade no setor saúde. Este estudo objetivou: verificar a adequação entre a alocação da equipe de enfermagem e as horas de cuidado requeridas pelos pacientes, bem como identificar a relação entre essa alocação com eventos adversos/incidentes (EA/I). Trata-se de pesquisa observacional, descritiva e prospectiva, desenvolvida nas Unidades de Terapia Intensiva Clínicas do 4º andar e 6º andar de um Hospital Universitário, do município de São Paulo, Brasil, no período de 01/11/07 a 10/12/07, com 46 pacientes. Nas UTIs 4º andar e 6º andar, respectivamente, 43,3% e 10,3% das alocações foram inadequadas (p = 0,000). Houve diferença na frequência de EA/I nas alocações adequadas e inadequadas da equipe de enfermagem da UTI 4º andar e UTI 6º andar, p = 0,0004 e p = 0,000, respectivamente. Concluiu-se que, quanto maior a diferença entre as horas disponíveis e requeridas de cuidado nas alocações de enfermagem, menor a frequência de EA/I.
Objective: to analyze the association between the environment of nursing practices and work satisfaction in Intensive Care Units (ICU). Method: a cross-sectional study was performed in eight adult ICUs of a public university hospital between 2012 and 2015. The Nursing Work Index-Revised (NWI-R), in their short forms, and the Index of Work Satisfaction (IWS) were applied to investigate the environment of nursing practices and work satisfaction, respectively. Results: a total of 100 (34.84%) nurses and 187 (65.15%) nursing assistants/ technicians participated in this study. The environment was favorable for autonomy and relationships and it showed vulnerability for control of practices and organizational support. The IWS score indicated low work satisfaction. "Environment of practices", "length of work in the ICU" and "willingness to work" were associated with work satisfaction. Conclusion: to invest in the environment of practices, in factors that promote willingness to work and length of experience in the ICU increases nursing work satisfaction.
Objective: to identify the factors associated with the occurrence of adverse events in critical elderly patients admitted to intensive care unit according to demographic and clinical characteristics. Method: a retrospective cohort study was conducted in nine units of a teaching hospital. Data were collected from medical records and from monitoring of nursing shift change. We used the t-test/Mann-Whitney, chi-square and logistic regression to test associations. Signifi cance level of 5% was used. Results: out of the 315 elderly, 94 experienced events. Those who experienced events were men (60.6%) with mean age of 70.7 years, length of hospital stay of 10.6 days and survivors (61.7%). Most of the 183 events were clinical processes and procedures (37.1%). There was an association between adverse event and length of hospital stay in the unit (p=0.000; OR=1.10, 95% CI [1.06, 1.14]). Conclusion: the identifi cation of associated events and factors in the elderly subsidize the prevention of these occurrences before the vulnerability of this age group. Método: estudo de coorte retrospectivo realizado em nove unidades de um hospital universitário. Os dados foram coletados dos prontuários e do acompanhamento de passagens de plantão de enfermagem. Utilizou-se o Teste-t/Mann-Whitney, Qui-quadrado e Regressão Logística para verifi car associações. Nível de signifi cância de 5%. Resultados: do total de 315 idosos, 94 sofreram eventos. Os que sofreram eventos eram homens (60,6%), com média de idade de 70,7 anos, permanência de 10,6 dias e sobreviventes (61,7%). Dos 183 eventos, houve predomínio do tipo processo clínico e procedimento (37,1%).
