The B subtype is predominant in Costa Rican HIV-positive patients. There is high variability within sequences with potential recombination between B and F or D subtypes. The BD recombinant has not been previously reported. This high variability is likely the result of possible recombinant events, nonadherence to antiretroviral therapy, sexual intercourse without protection, and many sexual partners. Similar studies should be done in other countries in the Region, in particular in those places with extensive immigration, in order to decrease the possibility of virus variability as well as the cost of antiretroviral therapy.
Justificación y Objetivos: los enterococos son cocos Gram positivos y las especies de importancia médica son Enterococcus faecalis y E. faecium como productores de la mayoría de las infecciones en el ser humano y E.casseliflavus, E. gallinarum, E. durans, E.hirae, E.raffinosus, y E.avium raramente aislados de muestras clínicas pero que pueden poseer genes extracromosómicos que los hacen intrínsicamente resistentes a los glucopéptidos, los que podrían ser transferidos a E. faecalis y E. faecium. La colonización por estas bacterias es frecuente en pacientes gravemente enfermos ingresados en unidades de cuidado intensivo (UCI) y es un factor predisponente para septicemia. Los objetivos de este trabajo son la identificación de pacientes colonizados por enterococos vancomicina resistentes (EVR) en medios hospitalarios nacionales, el análisis de la sensibilidad a antibióticos de las bacterias aisladas, el estudio de los factores de riesgo para adquirirlas y de los aislamientos con alta resistencia a la vancomicina, con el fin de identificar determinantes genéticos de resistencia.Pacientes y métodos: estudio prospectivo, que se desarrolló de mayo a agosto del año 2001. Se incluyeron 106 pacientes internados en las unidades de cuidado intensivo del Hospital México, Hospital San Juan de Dios y Servicio de emergencias médicas de ese último nosocomio, a los que se les tomaron hisopados rectales, los que fueron cultivados para identificar la presencia de EVR. Se determinaron las concentraciones mínimas inhibitorias (CIM) para vancomicina, y se realizó el estudio genético de las cepas con CIMs más altas. Resultados: la tasa de colonización del tracto gastrointestinal de los pacientes fue de 52%. Empleando las pruebas de chi cuadrado y de regresión logística, se identificaron factores que intervienen en la colonización por estas bacterias, siendo el servicio de procedencia antes del ingreso a las respectivas UCI, los días de estancia en UCI y el uso previo de cefalosporinas de tercera generación los principales. El 29.6% de los aislamientos de enterococos resistentes a vancomicina tenían CIM ≥512 ug/ml, todos poseían el gen van A y correspondieron a E.gallinarum, E.faecium, E casseliflavus y E.hirae Todos los aislamientos de Enterococcus faecalis mostraron CIM £ 16ug/ml a la vancomicina. Conclusiones: se demostró 52% de prevalencia de colonización en pacientes con EVR, superior a lo encontrado en otros estudios publicados en la literatura médica. Se establece la importancia de conocer estos hallazgos para el manejo clínico y epidemiológico de las infecciones asociadas a estas bacterias. Pocos estudios en la literatura mundial han logrado identificar este tipo de determinantes genéticos de resistencia (vanA) en enterococos no patogénicos. Se discute la relevancia de estos datos.
Justificación y objetivos: La producción de ß-lactamasas es uno de los principales mecanismos de resistencia a antibióticos utilizados por las bacterias Gram negativas. A partir de los años 80 se describe en Escherichia coli y Klebsiella pneumoniae un nuevo tipo de estas enzimas, las ßlactamasas de espectro ampliado (BLEA) que inactivan todos los antibióticos ß-lactámicos, con excepción de los carbapenémicos. Su diseminación global ha sido muy rápida, lo que ha creado problemas terapéuticos importantes. Este trabajo tiene como objetivos el análisis de la frecuencia de las infecciones producidas por estas bacterias en pacientes internados en el Hospital San Juan de Dios (HSJD), los factores de riesgo para su adquisición y el análisis molecular de las enzimas identificadas.Metodología: Se revisaron los archivos del laboratorio de bacteriología del HSJD de 2004 y 2005, con el fin de identificar cepas de K. pneumoniae y E. coli productoras de BLEA. La muestra control fue seleccionada al azar entre los cultivos negativos de E. coli y K. pneumoniae por BLEA obtenidos durante mismo período en las bacterias de interés. Solo se incluyeron los reportes positivos por muestra intrahospitalarias. Se analizaron los factores de riesgo para adquirir estas infecciones en pacientes internados entre marzo y julio de 2004. Para la identificación de las bacterias, la sensibilidad a diversos antibióticos, así como la producción de BLEA, se utilizó el equipo automatizado VITEKR BioMérieux. La comprobación de la producción de estas enzimas se efectuó por medio de la prueba de disco empleando cefotaxima, cefotaxima + clavulanato, ceftazidima y ceftazidima + clavulanato. La identificación de las secuencias de los tipos de ß-lactamasas SHV-5, CTX-M y TEM se efectuó por medio de la prueba de reacción en cadena de la polimerasa.Resultados: En 2004, en el 12% de los aislamientos de E. coli y el 16% de K. pneumoniae se demostró la producción de BLEA. En 2005, el 18% y el 40% de esas bacterias, respectivamente, fueron BLEA+. Justificación: La resistencia a antibióticos por parte de bacterias patógenas es un grave problema mundial. Su control ha sido difícil pero deben hacerse todos los esfuerzos por realizarlo. Conocer la epidemiología en las diferentes regiones y países se constituye en parte importante de este control. Este estudio se llevó a cabo con el fin de analizar el comportamiento de la resistencia en el hospital San Juan de Dios, centro de atención nacional de adultos de aproximadamente 700 camas.Materiales y métodos: El estudio se basó en los datos obtenidos en los años 1995-1999 ya que en 1995 se inició el empleo de equipo automatizado VITEK® en este hospital. Se analizaron 2817 aislamientos de cocos Gram positivos (CGP) y 7626 de bacilos Gram negativos (BGN) obtenidos de todos los sitios anatómicos y fluidos corporales. Por ser un estudio retrospectivo, no se pudo definir cuáles aislamientos provenían de infecciones intra o extrahospitalarias.Resultados: Staphylococcus aureus fue el CGP más frecuentemente aislado, mientras que el BGN más frecuente fue Escherichia coli. En más de 90% de los estafilococos aislados se demostró resistencia a la penicilina. La resistencia a la oxacilina en S.aureus se incrementó de 35% en 1995 a 52% en 1999 mientras que en los estafilococos coagulasa negativa, pasó de 70 a 77% en el mismo periodo. La resistencia a la cefalotina en S.aureus pasó de 35 a 50% mientras que en los estafilococos coagulasa negativa pasó de 65 a 76%. La resistencia a la clindamicina en ambos grupos bacterianos se mantuvo relativamente estable. No se demostró resistencia a la vancomicina. La mayoría de los enterococos aislados correspondieron a E. faecalis, 75% de ellos fueron sensibles a la penicilina y 50% no mostraron sinergismo de la gentamicina con los antibióticos que actúan sobre la pared. No se encontraron enterococos vancomicina resistentes. En las enterobacterias se observó un aumento importante de la resistencia a cefalosporinas de tercera generación. E. coli mostró un aumento de la resistencia a ceftazidima de 10% en 1995 a 35% en 1999, mientras que Klebsiella pmeumoniae pasó de 0% a 52% para el mismo antibiótico en ese periodo. Con respecto a cefotaxima, mientras que en E.coli se incrementó la resistencia de 5 a 12%, en K. pneumoniae se modificó de 5 a 24%. Esto podría deberse a la presencia de ß lactamasas de espectro ampliado. La sensibilidad a ciprofloxacina en E. coli al igual que en K. pneumoniae, se mantuvo estable. En cuanto a amicacina, E.coli pasó de 0% de resistencia en 1995 a 15% en 1999 y K. pneumoniae de 20 a 32%. En Enterobacter cloacae se demostró aumento importante de la resistencia a cefalosporinas de tercera generación y a amicacina no así a la ciprofloxacina. En todas las enterobacterias se mantuvo una baja resistencia al imipenem. En cuantoa BGN no fermentadores, se demostró en Pseudomonas aeruginosa un aumento de la resistencia a la mayoría de los antibióticos con excepción del imipenem y ceftazidima. No obstanteAcinetobacter calcoaceticus presentó un incremento importante de la resistencia a amicacina (de 20 a 72%), a cefalosporinas de tercera generación (de 0 a 61%) y ciprofloxacina (57%), manteniendo alta sensibilidad sólo a imipenem (95%).
