Asthma and obesity, both of which are considered global health issues, affect approximately 300 million individuals worldwide. The observation that asthma and obesity tend to increase in parallel may indicate a potential link between these two conditions. The aim of this systematic review of the literature was to investigate the relationship between childhood nutritional status and incidence or persistence of asthma during adolescence. The principal eligibility criteria for selection of articles were including longitudinal analysis, and which measured of exposure during childhood or adolescence and outcome during adolescence. The systematic search initially returned 1563 unique articles. After inspection of titles and reading the abstracts of these articles, we chose 27 for full reading. Of these, 10 were selected to be included in the present review. Of the 10 selected studies, eight showed positive associations between overweight/obesity and asthma. Of these, two were independent of sex, three were found only among men and another three only among women. The present study shows strong evidence that, although the role of sex is not clear, obesity precedes, and is associated with, the persistence and intensity of symptoms of asthma.
PurposeTo evaluate the association between physical activity changes in those aged 11 to 15 years and lung function at age 15 years.MethodsThe original cohort comprised 5,249 hospital-born children during the calendar year of 1993 in Pelotas, Brazil. In 2004–2005 and 2008–2009, all cohort members were sought for follow-up visits. Self-reported physical activity was measured at ages 11 and 15 years. At the 2008–2009 visit, when participants were 15 years old, spirometry was performed. Linear regression was used, and all analyses were stratified by sex.ResultsOf the 5,249 original members of the cohort, 4,325 were located at 15 years of age, and spirometry was performed on 4,010 members. Forced expiratory volume in 1 second was not associated with physical activity. In girls, those who were active in leisure time in both periods have better percent-predicted forced vital capacity (β = 3.573 [95% confidence interval {CI}: 1.015, 6.130]) and forced expiratory volume in the 6 seconds (β = .095 [95% CI: .021, .168]) than those who were inactive in the two periods. Also in girls, those who became active at 15 years of age had higher peak expiratory flow than those who were inactive at 11 and 15 years of age. In boys, only those who became inactive in leisure time had worse peak expiratory flow (β = −.180 [95% CI: −.339, −.021]) than boys who were inactive at ages 11 and 15 years.ConclusionsSelf-reported leisure-time physical activity was associated with better effort-dependent lung function parameters, particularly among girls.
PurposeTo investigate the effect of obesity at the start of adolescence on the prevalence, incidence and maintenance of chest wheezing among individuals aged 11–15 years in a birth cohort in a developing country.MethodsThe seventh follow-up of the 1993 Pelotas birth cohort occurred in 2004 (individuals aged 10–11 years). Between January and August 2008, the eighth follow-up of the cohort was conducted. All the individuals of the original cohort who were alive (who were then adolescents aged between 14 and 15 years) were targets for the study. The International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire was used to define wheezing. In addition to the body mass index (BMI), used to define obesity by the World Health Organization (WHO) criteria, we assessed skinfold thickness.ResultsFrom the original cohort, 4,349 individuals were located (85.7% follow-up rate). The prevalence of chest wheezing at 11 and 15 years were 13.5% (95% CI: 12.5%–14.5%) and 12.1% (95% CI: 11.1%–13.1%), respectively. The prevalence of wheezing at both times was 4.5% (95% CI: 3.9%–5.1%) and the incidence of wheezing was 7.5% (95% CI: 6.7%–8.3%). Independent of the effect of various confounding variables, the prevalence of wheezing at 15 years was 50% greater among obese individuals than among eutrophic individuals at 11 years (RR 1.53; 95% CI: 1.14–2.05). The greater the skinfold tertile at 11 years, the higher the prevalence of wheezing at 15 years was (p = .011). Weight status and skinfolds did not present any association with incident wheezing. After controlling for confounding factors, the risk of persistent wheezing among obese individuals at 11 years was 1.82 (95% CI: 1.30–2.54).ConclusionsSince obesity at the start of adolescence is associated with asthma symptom persistence, prevention and treatment of obesity may reduce avoidable healthcare costs and disease burden.
OBJETIVO: Estimar a prevalência de ronco habitual e apnéia obstrutiva observada e fatores associados. MÉTODOS:Estudo transversal com amostra representativa de 3.136 adultos, com 20 anos ou mais, residentes em Pelotas, Rio Grande do Sul, em 2005. Informações sobre os desfechos e variáveis demográfi cas, socioeconômicas, comportamentais e antropométricas foram coletadas por meio de questionário. A análise estatística bruta e ajustada foi realizada utilizando-se teste exato de Fisher ou qui-quadrado de tendência linear e regressão de Poisson, respectivamente. RESULTADOS:A prevalência encontrada de ronco habitual foi de 50,5% (IC 95%: 48,1;52,8) e de apnéia obstrutiva de 9,9% (IC 95%: 8,7;11,2). Na análise ajustada, o relato de ronco foi maior nos homens (Razão de Prevalências -RP=1,25; IC 95%: 1,16;1,34), nos idosos (RP=1,62; IC 95%: 1,46;1,80), nos tabagistas (RP=1,15; IC 95%: 1,07;1,25), nos alcoolistas (RP=1,17; IC 95%: 1,03;1,31) e nos obesos (RP 1,71, IC95% 1,55;1,88). O relato de apnéia obstrutiva foi maior nos homens (RP=2,05; IC 95%: 1,67;2,52), nos idosos (RP=2,23; IC 95%: 1,64;3,03), nos tabagistas (RP=1,60; IC 95%: 1,25;2,05) e nos obesos (RP=2,61; IC 95%: 1,97;3,47). CONCLUSÕES:Ronco habitual e apnéia obstrutiva foram sintomas comuns na população estudada. Fatores de risco conhecidos como sexo masculino e idade entre quarta e quinta décadas de vida não são modifi cáveis. BackgroundSocioeconomic status (SES) has been shown to be an important contributor to lung function. The aim of this study was to evaluate the association between lung function in adolescence and (a) SES at birth; (b) SES in adolescence; (c) SES trajectory from birth to adolescence ('never poor', 'non poor-poor', 'poor-non poor' and 'always poor'). Additionally, we investigate the role of adolescent and parental variables at mediating these associations.MethodsProspective birth cohort study in Pelotas, Brazil, including 4,005 adolescents (mean age: 14.7 years) followed up from birth. Lung function was measured by spirometry. Outcome variables were forced expiratory volume in one second in liters (FEV1) and forced vital capacity also in liters (FVC).ResultsMean FEV1 was 3.46 L (95%CI 3.43-3.49) among boys and 2.93 L (95%CI 2.91-2.95) among girls. Mean FVC was 4.00 L (95%CI 3.97; 4.04) among boys and 3.30 L (95%CI 3.27; 3.32) among girls. SES at birth, in adolescence and its trajectory from birth to adolescence were inversely associated with lung function in both adolescent boys and girls. After adjustment for mediating variables, coefficients were largely reduced, particularly among boys, and the main predictor of change in coefficients was the inclusion of height in the models.ConclusionLow income adolescents from Brazil present impaired lung function as compared to the better off, and this is largely explained by height.
ResumoAproximadamente sete milhões de brasileiros acima de 40 anos são acometidos pela DPOC. Nos últimos anos, importantes avanços foram registrados no campo do tratamento medicamentoso dessa condição. Foi realizada uma revisão sistemática incluindo artigos originais sobre tratamento farmacológico da DPOC publicados entre 2005 e 2009, indexados em bases de dados nacionais e internacionais e escritos em inglês, espanhol ou português. Artigos com tamanho amostral menor de 100 indivíduos foram excluídos. Os desfechos sintomas, função pulmonar, qualidade de vida, exacerbações, mortalidade e efeitos adversos foram pesquisados. Os artigos foram classificados segundo o critério da Global Initiative for Chronic Obstructive Lung Disease para nível de evidência científica (grau de recomendação A, B e C). Dos 84 artigos selecionados, 40 (47,6%), 18 (21,4%) e 26 (31,0%) foram classificados com graus A, B e C, respectivamente. Das 420 análises oriundas desses artigos, 236 referiam-se à comparação de fármacos contra placebo nos diversos desfechos estudados. Dessas 236 análises, os fármacos mais frequentemente estudados foram anticolinérgicos de longa duração, a combinação β 2 -agonistas de longa duração + corticosteroides inalatórios e corticosteroides inalatórios isolados em 66, 48 e 42 análises, respectivamente. Nas mesmas análises, os desfechos função pulmonar, efeitos adversos e sintomas geraram 58, 54 e 35 análises, respectivamente. A maioria dos estudos mostrou que os medicamentos aliviaram os sintomas, melhoraram a qualidade de vida, a função pulmonar e preveniram as exacerbações. Poucos estudos contemplaram o desfecho mortalidade, e o papel do tratamento medicamentoso nesse desfecho ainda não está completamente definido. Pulmonary rehabilitation programs are aimed at providing benefits to COPD patients, in various aspects. Our objective was to review the literature on COPD patient rehabilitation. This systematic review involved articles written in English, Spanish, or Portuguese; published between 2005 and; and indexed in national and international databases. Articles were classified in accordance with the Global Initiative for Chronic Obstructive Lung Disease criteria for the determination of the level of scientific evidence (grade of recommendation A, B, or C). The outcome measures were exercise, quality of life, symptoms, exacerbations, mortality, and pulmonary function. Treatments were classified as standard rehabilitation, partial rehabilitation, strength exercises, and resistance exercises. Of the 40 articles selected, 4, 18, and 18 were classified as grades A, B, and C, respectively. Of the 181 analyses made in these articles, 61,50,23,23,20, and 4, respectively, were related to the outcome measures quality of life, exercise, symptoms, exacerbations, pulmonary function, and mortality. The standard rehabilitation programs showed positive effects on all of the outcomes evaluated, except for mortality (because of the small number of analyses). However, we found no differences among the various rehabilitation programs regarding their effects on the outcomes studied. Rehabilitation programs can be considered important tools for the treatment of COPD. BackgroundWe investigated the association between maternal anthropometric measurements in prepregnancy and at the end of pregnancy and their children's systolic (SBP) and diastolic (DBP) blood pressure at 11 years of age, in a prospective cohort study.MethodsAll hospital births which took place in 1993 in the city of Pelotas - Brazil, were identified (5,249 live births). In 2004, the overall proportion of follow-up was 85% and we obtained arterial blood pressure measurements of 4,452 adolescents.ResultsIndependent variables analyzed included maternal prepregnancy weight and body mass index (BMI) and maternal weight, and height at the end of pregnancy. Multiple linear regression analysis controlling for the following confounders were carried out: adolescent's skin color, family income at birth, smoking, alcohol intake during pregnancy, and gestational arterial hypertension. Mean SBP and DBP were 101.9 mmHg (SD 12.3) and 63.4 mmHg (SD 9.9), respectively. Maternal prepregnancy weight and BMI, and weight at the end of pregnancy were positively associated with both SBP and DBP in adolescent subjects of both sexes; maternal height was positively associated with SBP only among males.ConclusionsAdequate evaluation of maternal anthropometric characteristics during pregnancy may prevent high levels of blood pressure among adolescent children.
