Introduction:The high demand for orthodontic treatment, evidenced over the last few decades, has been justified mainly by the greater importance given to facial esthetics, influencing individual's self esteem. However, the professional frequently does not meet all the patient's expectations, for not establishing good communication and not knowing about the critical points during orthodontic treatment. Objective: The aim of this study was to elucidate patients' desires and doubts regarding orthodontic treatment, by means of a survey applied to 60 adult patients. Results: The analysis of results revealed that most individuals (38.3 %) noticed treatment success after its conclusion. Occlusion deviation was pointed out by 66.7 % as the main reason for seeking treatment, and esthetics ranked as second (with 48.3 %). Treatment time was considered within the prediction by 46.7% of the interviewees and the results were judged as very good by 43.3 %. The social relations of most participants were not affected by treatment (73.3 %). Also, 58.3 % of the interviewees reported pain as the main complaint and 53.3 % found it difficult to use dental floss. Most participants saw the orthodontist as a professional who was concerned about their health (76.7 %), and believed that he/she was more able to treat them (96.6 %) when compared with the general practitioner. Resultados: a análise dos resultados revelou que a maioria dos indivíduos (38,3%) percebeu êxito após a finalização da terapêutica. O desvio da oclusão foi apontado por 66,7% como principal motivo de procura pelo tratamento, e, em segundo lugar, 48,3% pela estética. O tempo de tratamento foi considerado dentro do previsto por 46,7% dos entrevistados e os resultados foram julgados como muito bons para 43,3%. As relações sociais da maioria dos participantes não foram afetadas pelo tratamento (73,3%), sendo que 58,3% dos entrevistados relataram a dor como queixa principal, e 53,3% encontraram dificuldades no uso do fio dental. Objective: To test the null hypothesis that enamel deproteinization with 10% papain gel does not increase the shear bond strength of orthodontic brackets bonded with resin-modified glass ionomer cement (RMGIC). Materials and Methods: One hundred and twenty bovine incisors were used and divided into eight groups: 1) Transbond XT according to the manufacturer's recommendations, 2) Transbond XT deproteinized with 10% papain gel, 3) RMGIC without enamel deproteinization and without etching, 4) RMGIC without enamel etching and with deproteinization with 10% papain gel, 5) RMGIC deproteinized with 10% papain gel and etched with polyacrylic acid, 6) RMGIC deproteinized with 10% papain gel and etched with phosphoric acid, 7) RMGIC deproteinized with 2.5% sodium hypochlorite, and 8) RMGIC etched with polyacrylic acid. After bonding, the mechanical tests were performed in a Universal mechanical test machine. The values obtained were submitted to an analysis of variance and afterward to the Tukey test (P , .05). Results: It was demonstrated that group 2 presented the highest shear bond strength value, and this differed statistically from the others; group 3 presented the lowest value and showed no differences from groups 4, 5, 7, and 8. Regarding the Adhesive Remnant Index, groups 2 and 6 presented the best results and groups 3 and 8 the worst. It could be concluded that enamel deproteinization with 10% papain gel increases the shear bond strength, irrespective of the etching agent. Conclusions: The hypothesis is rejected. Papain gel was shown to be a new ally in the orthodontic clinic. (Angle Orthod. 2012;82:541-545.)
Objective: To evaluate the degree of perception of laypersons, dental professionals, and dental students regarding dental esthetics in cases with mandibular central incisor extraction. Materials and Methods: Using a smile photograph of a person with normal occlusion and all teeth, modifications were made to reflect the extraction of a mandibular incisor of various compositions and sizes. For this purpose a program specifically for image manipulation (Adobe Photoshop CS3, Adobe Systems Inc) was used. After manipulation the images were printed on photographic paper, attached to a questionnaire and distributed to laypersons, dental professionals, and dental students (n 5 90) to evaluate the degree of perception and esthetic using a scale of attractiveness, where 0 5 hardly attractive, 5 5 attractive, and 10 5 very attractive. The differences between examiners were checked by the Mann-Whitney test. All the statistics were performed with a confidence level of 95%. Results: The results demonstrated the skill of the dental professionals and dental students in perceiving the difference between cases of normal occlusion and cases where an incisor was lacking (P , .05). The photograph in which the lateral incisors were shown to be larger than the central incisor was the one that obtained the highest value among the cases of extraction in all groups of evaluators. Conclusions: It can be concluded that dental professionals and dental students are more skillful at identifying deviation from normality. In addition, central incisor extraction should always be discarded when there are other treatment options available. (Angle Orthod. 2012;82:732-738.)
Objective: The aim of this study was to evaluate the degree of perception of attractiveness of the smile among dentists, dental students, and lay persons in cases of agenesis of the maxillary lateral incisors replaced by canines for space closure. Methods: A smiling front view extraoral photograph of a 20-year-old woman was digitally altered simulating agenesis and its treatment, by means of: repositioning, reshaping or bleaching the canine, and gingival contour. A questionnaire was distributed to individuals of the three groups (n= 150), with a view to evaluating their degree of esthetic perception. An attractiveness scale was also used, with ‘0’ representing unattractive and ‘10’, very attractive. Results: In the comparative evaluation among all the photographs, the original image obtained the highest level of acceptance. Photograph ‘i’ (agenesis of both lateral incisors treated with reposition and reshaping of the canines) was ranked as the least attractive by the dentists, whereas the student and lay persons ranked photograph ‘f’ (agenesis of both lateral incisors treated with reposition of the canines, gingival contour, bleaching and reshaping) as the worst. Conclusion: The methods of treatment most accepted among the dentists and students were those that involved changes in the gingival contour, whereas among lay persons, they were those that involved only reshaping.
Background The use of skeletal anchorage devices for maxillary protraction in patients with class III malocclusion due to deficiency in the middle third of the face has been shown to be a promising approach to treatment of these patients. The aim of this study was to evaluate the treatment of class III patients with maxillary retrusion, using orthodontic mini-implants (MI) associated with intermaxillary elastics in comparison with the rapid maxillary expansion and facemask protocol (RME/FM). Methods In this prospective non-randomized clinical trial, the sample of 24 participants between 7 and 12 years of age (median age of 10.0 years and interquartile range = 3.0 years), at the stage prior to the pre-pubertal growth spurt, was divided in two groups. In group facemask (FM) ( n = 12), the individuals received orthopedic treatment with RME/FM. In group MI ( n = 12), two mini-implants were inserted in the region close to the maxillary first molar roots, and the other two in the region of the mandibular canines. Initial and final lateral teleradiographs were taken for cephalometric evaluation of all the cases. Statistical analysis included the Mann-Whitney, Wilcoxon, and Fisher’s exact tests. The level of significance was 5% (α = 0.05). Results Improvement was verified in the facial profile and occlusion of the participants, showing advancement of the maxilla in the two groups, with significant differences ( P ≤ 0.05) between T0 and T1 in the following measurements: SNA, ANB, Wits, Co-A, Co-Gn, NAP, A-Npog, overjet, and molar relationship. There was no statistically significant intergroup difference ( P > 0.05) in the cephalometric measurements evaluated, but the time of treatment was significant, and was faster for group MI. Conclusions The protocol with mini-implants may be an option for the correction of Class III due to maxillary deficiency.
Purpose This article evaluates the inter-relationship between the feeding time, presence of deleterious oral habits and the development of malocclusion. Materials and methods For this purpose, a document focused on the quantitative aspects and was prepared using forms filled out according to the information available in the medical records of 93 children of both genders, aged between 4 and 12 years old, who were treated at the Dental Clinic of UESB (University of the Southwest State of Bahia), Jequié Campus, in the period from 2006 to 2009. The Chi-square statistical test was applied. Results We observed a higher frequency of children who were naturally breast-fed for a period longer than 6 months (47%). There was a significant relationship between the shorter duration of breastfeeding leading to deleterious oral habits, and consequently to the development of malocclusion (p < 0.001). The existence of one or more deleterious oral habits also favored the development of malocclusion, even in the group that was breast-fed for longer than 6 months (p < 0.001). The habit that appeared most frequently was onychophagy (nail biting; 26%) followed by lingual interposition (17%) and pacifier sucking (14%). The highest prevalence of crowding was found among children with malocclusion (33%). Conclusion Mothers should be encouraged to extend the time of breastfeeding and motivate children not to develop deleterious oral habits, as these are crucial factors in diminishing the development of malocclusion.
