Erro de medicação é qualquer evento evitável capaz de causar ou levar ao uso inapropriado da medicação que está no controle do profissional de saúde, paciente ou consumidor. Estima-se que 46% dos erros de medicação ocorrem na admissão hospitalar e uma das formas de reduzi-los é adotando a reconciliação medicamentosa em unidade hospitalar. O trabalho objetivou a criação de um método para a prática de conciliação medicamentosa de forma estruturada. Foi realizado um estudo exploratório, em um hospital oncológico de grande porte no Rio de Janeiro/Brasil, aplicando-se o método na internação de pacientes nos Serviços de Ginecologia e Oncologia do hospital. O método foi aplicado a 201 pacientes internados, permitindo a mensuração da frequência da conciliação medicamentosa instituída. Do total de pacientes entrevistados, 89,6% eram do sexo feminino; 28,2% tinham entre 61 a 70 anos e 25,2% tinham entre 51 a 60 anos (25,2%). O principal motivo de internação foi o procedimento cirúrgico (69,3%). Por meio do método proposto, foram identificadas discrepâncias; essas foram registradas de acordo com a classificação de Otero. A partir do método proposto, os medicamentos utilizados anteriormente à internação podem ser comparados com a prescrição médica de internação e as discrepâncias analisadas, aumentando a qualidade do cuidado na rotina hospitalar.
