Com o objetivo de determinar o perfil clínico-epidemiológico dos acidentes ofídicos, em hospital do Estado do Acre, foram estudados prospectivamente 144 pacientes admitidos no período de janeiro a dezembro de 2002. Desses, 113 (78,5%) foram classificados como vítimas de acidente com envenenamento. Os gêneros Bothrops, Lachesis e Micrurus foram responsáveis por, respectivamente, 75,7%, 2,1% e 0,7% dos casos. Os acidentes predominaram em pessoas do sexo masculino (78,5%), trabalhadores rurais (51,4%) e com idades entre 10 e 29 anos (43,8%). Nos acidentes botrópicos, os envenenamentos considerados moderados (48,6%) prevaleceram sobre os leves (31,2%) e graves (20,2%). Dois casos envolvendo o gênero Bothrops não receberam terapia antipeçonha. Entretanto, soro heterólogo foi administrado em 23 vítimas de acidente sem envenenamento. Concluindo, os resultados obtidos neste estudo diferiram dos observados por outros autores quanto à gravidade dos casos e adequação do tratamento, indicando a necessidade de treinamento da equipe.

The state of Bahia's main climatic characteristic is the high spatial and chronological variability of precipitation. This heterogeneity may be used to determine of pluviometrically homogeneous areas that can define mesoregions in the state, since they allow better management of water resources and help in the elaboration of agricultural studies. The mesoregions already proposed by the scientific community for the state were based only on the annual precipitation in the proximity of the pluviometric stations. In this paper, besides these parameters, spatial and chronological rainfall distribution was considered, i.e., the Precipitation Concentration Degree (PCD) and Precipitation Concentration Period (PCP). The new zoning is based on an update of a study defined in 2000 that divided Bahia into eight mesoregions. Thus, 180 pluviometric stations were distributed throughout the state and grouped conforming to the division previously described. It was concluded that some stations of the same mesoregion had presented conflicting values for the analyzed parameters and, therefore, should not belong to the same area. Starting from an arrangement of the collection stations, considering their proximity, annual precipitation and statistical parameters, a new zoning for Bahia with 10 clusters was defined and validated through the statistical treatment of data. Essa heterogeneidade conduz à determinação de áreas pluviometricamente homogêneas, uma vez que estas permitem uma melhor gestão dos recursos hídricos e auxiliam na elaboração de estudos agrícolas. This study investigated the correlation between the incidence of snakebite and indicators of agricultural development in municipal districts of the State of Bahia, Brazil. An ecological study was conducted with the 27,347 cases of snakebite poisoning reported to the Reportable Diseases National Information System in municipalities from the State of Bahia, Brazil, 2000-2009. The unit of analysis was each one of the 417 State municipalities, and the outcome variable was the average annual incidence of snakebite. Data were analyzed by multiple linear regression technique. The average annual incidence of snakebite ranged from zero to 221.96 per 100,000 inhabitants, according to the municipalities. The incidence of snakebite was positively and strongly associated with municipality characteristics: planted areas of cocoa and coffee, and the size of domestic bred chicken and bovine livestocks. It was concluded that several characteristics related to municipal agricultural profile were strongly associated with the incidence of snakebite.
Resumo A presente investigação é um estudo descritivo dos aspectos clínicos dos acidentes causados pelo escorpião Tityus stigmurus no Estado da Bahia, Brasil. Foram analisados 237 casos confirmados, tratados pelo Centro de Informações Antiveneno da Bahia (CIAVE), no período de 1982-1995. O envenenamento por T. stigmurus caracterizou-se por manifestações locais: dor (94,4%), dormência (30%), edema (17,8%), eritema (17,8%) e parestesia (15,6%) e gerais: cefaléia (14%), vômitos (4,4%) e sudorese (3,3%). A maioria dos envenenamentos (94%) foi leve e todos evoluíram para cura. A ausência de letalidade, com o restabelecimento dos pacientes, inclusive casos graves, sugere a eficácia do tratamento com o antiveneno específico, apesar do veneno desta espécie não estar presente no pool de produção nacional do soro. Há necessidade de revisão dos critérios regionais nos esquemas atuais de soroterapia. Os dados apontam para a semelhança da gravidade do envenenamento por T. serrulatus, com exceção da ocorrência de óbitos e complicações sistêmicas. Palavras-chaves: Tityus stigmurus. Envenenamento. Escorpionismo. Bahia. Brasil. AbstractThe present investigation is a descriptive study regarding the clinical aspects of accidents caused by the scorpion Tityus stigmurus in Bahia, Brazil. We analyzed 237 confirmed cases treated by the Antivenom Information Centre (CIAVE) from 1982 to 1995. Envenomation by T. stigmurus was mainly characterized by local symptoms: pain (94.4%), dormancy (30.0%), edema (17.8%), erythema (17.8), paresthesia (15.6%) and general manifestations such as headache (4.4%), vomiting (4.4%) and sudoresis (3.3%). Most of the envenomation cases were mild (94%) and all were successfully cured. Although T. stigmurus venom is not in the pool of anti-venom serum (SAE), the absence of lethality and benign nature of the cases suggest the efficiency of SAE. With the exception of deaths and systemic complications, envenoming gravity was similar to those of Tityus serrulatus.
Objective To analyze the relationship between time to treatment and severity of snakebite envenomation in Brazil. Methods This case-series retrospective study analyzed 144 251 snakebite cases in Brazil between 2007 and 2015, as reported to the Brazilian Notifiable Diseases Information System. The main dependent variable was snakebite envenomation severity (mild/moderate/severe). The main predictor was time to treatment (early (< 6 hours) vs. delayed (≥ 6 hours)). Covariables were snake type ( Bothrops/Crotalus/Micrurus/Lachesis ), patient's age and sex, bite site, and treatment at a specialized care center (center/noncenter). Polytomous logistic regression techniques were used to control the covariates and assess confounding and effect modification. Results The time to treatment variable was strongly associated with the severity of snakebite envenomation. Snake type and treatment at specialized care center modified the main association effect. The association between delayed time to treatment and envenomation severity was consistently stronger among patients treated at specialized care centers than among those who were not treated at such centers. Odds ratios tended to increase significantly from moderate to severe envenomation for cases within the subgroups “Center + Bothrops ” (1.37 to 2.05), “No center + Bothrops ” (1.25 to 1.47), “Center + Crotalus ” (1.35 to 3.03), “No center + Crotalus ” (0.97 to 2.72), and “Center + Lachesis ” (1.22 to 1.89). Discussion This study confirmed the classical hypothesis that the time between snakebite and initiation of medical care is associated with severity of snakebite envenomation. It is therefore necessary to provide snakebite victims early access to specialized medical care, particularly to antivenom therapy.
Um estudo epidemiológico mediu a prevalência de pessoas que referiram acidentes por escorpião em uma amostra populacional do Areal, bairro Nordeste de Amaralina, Salvador, Bahia. Examinou-se uma amostra aleatória sistemática de 1367 indivíduos, correspondendo a 44,4% da população da área. Oitenta e dois indivíduos referiram haver sido picados por escorpião desde que residiam no Areal, resultando numa prevalência de 6% (IC 95% 4,7 - 7,3). A prevalência de pessoas picadas por escorpião aumentou nos grupos com maior tempo de residência no domicílio e com maior idade atual. Chamou atenção que 92,7% dos acidentes aconteceram dentro do domicílio. A incidência estimada para o período mais recente (janeiro a julho de 2000) foi de 1,15 casos/1.000 habitantes por mês. Este coeficiente compara-se ao mais elevado já referido na literatura especializada para uma área epidêmica para acidentes escorpiônicos.
This work is a retrospective study of latrodectism in the State of Bahia, Brazil, from August 1980 to July 1990. The data concerning the accidents were obtained from file cards at the Antivenom Information Center of Bahia (AVICB). Latrodectus curacavienis was the ethiologic agent identified in 28% of the arachnid accidents. The major incidence was registered in urban area (57%) affecting men (70%) more than women, with 10 to 29 year-old age group (58%). Local pain (56%), erythematous papula (29%) and light oedema (17%) were the principal local symptoms. Pain in the limbs (29%), tremor and rigidities (29%), sweating (28%), limbs and arms paresthesia (21%) and abdominal pain (17%) were systemic ones. The treatment was mainly symptomatic (67%) and antivenin serum was used in 21% of the cases. After serotherapy, 64% of the patients left the hospital within less than 24 hours.
