An anomalous development of infra-renal inferior vena cava leads to circumcaval ureter or preureteral vena cava, a rare congenital anomaly with an autopsy incidence of 0.9 per 1000. We present a case of circumcaval ureter and review the literature with a special emphasis on the paradigm shift seen during the last decade, in diagnostic and management strategies.
Gut malrotation is a congenital anomaly characterized by either lack of rotation or incomplete rotation of gut around axis of superior mesenteric artery. Most patients present within first year of life. Presentation during adult life is considered rare. Diagnosis of intestinal malrotation during adult life is most often by chance rather than by clinical acumen. Once diagnosed, treatment is based on the general principles of management of intestinal obstruction. We are presenting one such case in which CECT abdomen done to look for the diagnosis of acute pancreatitis leads us to the identification of intestinal malrotation.

Haemophilia is the oldest known rare genetic bleeding disorder that disrupts the blood clotting process. Although the level of haemophilia care has improved substantially, the problems of management in developing countries are poor awareness, high costs of treatment, inadequate diagnostic and coagulation screening facilities and scarce factor concentrates for therapy. We present here the problems in the perioperative management of a case of haemophilia A in India. It portrays the current picture of haemophilia management in many developing countries around the world.
