JULIO MENEGHELLCX JULIO GUASCH y RAUL EBERHARD En nuestro Servicio de Urgencia del Hospital Arriaran. hemos tenido oportunidad de observar en un pequeno grupo de ninos,, los efectos producidos per la ingestion accidental de pequefias cantidades de bencina o parafina, dedicando especial atencion a las manifestaciones-pulmonares, conccidas ya a traves de numeros,as publicacicnes extranjeras, con el nombre de neumonias a kerosene. Considerando la rareza del cuadro y su interes en la practica de Medicina Infantil de Urgencia, hemos unido a nuestra breve experiencia, algunas nocicnes adquiridas de otros autores. En la bibliografia extranjera aparecen numerosas referencias sobre este tema, que ha preocupado a los investie:adores desde fines del siglo pasado. Bliss (1899), Le Gludie y Turlais (1914) citadcs pot-Waring (7) describieron neumonias en ninos fallecidos a causa de la ingestion de kerosene. En 1926, Barbour publica cuatro observaciones de intoxicacion por kerosene en nines. Pcsteriormente en Norte America aparecen los trabajos de Nunny Martin (1931-1933) y Waring (1933). Este ultimo mvestigador, en un total de 23 ninos que habian ingerido bencina o parafina, comprueba en 9 de ellos la apancion de neumopatias. Basado en un interesante trabajo experimental realizado en conejos, demuestra que las lesinnes pulmcnares no se producen por la eliminacion del toxico prcr
El diagnostico de leucemia es en el adulto relativamente facil, cuando Ics hallazgos clinicos y_ hematologicos son caracteristicos. En la infancia, el problema es diferente, y sc complica por una serie de factorcs propios de la edad, que inducen a crrores diagnosticos.La frecuencia de leucemias de forma leucopenica en los ninos y la tendencia en los no .leucemicos a producir celulas blancas inmaduras en respuesta a infecciones graves, son hechos llamativos que seriala la ex,periencia y la literatura (Abt, 1924).En su curso implacablemente invasor, la leucemia compromete c'asi todos los sistemas del organismo. El esqueleto no escapa a este fenomeno y la demostracion radiologica de alteraciones oseas suministra una informacion mas en la pesquisa de la leucemia, especialmente en los casos acompanados de dolores oaeos, que simulan reumatismo, artritis u osteomielitis.Se ha senalado el hecho que las lesiones oseas en el curso de la leucemia son mas frecuentes en el nifio que en el adulto (Baty y Vogt, 1935); la explication radicaria en varias circ'unstancias propias de la edad infantil, como es, por ejemplo, la caracteristica medular del nifio que mantiene medula activa hematopoyetica, no solo en el esternon, sino que tambien en las costillas, vertebras, huesos iliacos, etc., ofreciendo asi mayores oportunidades para la infiltracion leucemica. Intimamente ligado con este aspecto, esta el hecho que la medula infantil es medula roja en su totalidad, en oposicion a la medula roja del adulto, que ocupa solo un espacio en algunos hucsos, de tal modo que poseyendo grandec cantidades de medula grasosa, esta puedo ser reemplazada por celiilas leucemicas, antes de que comience el comprcmiso oseo 1 (Poynton y col., 1932).La primera descripcion en la literatura de lesicnes esqueleticas en la leucemia, se remonta al ano 1847, en que Heschl describio en la anatomia patologica, una lesion osteolitica en un paciente leucemico. En Alemania, Von Jacksch, en 1901, es el primero en demostrar radiologicamente una lesion osea en la leucemia. Solo 23 anos despues aparcce en la literatura norteamericana el primer trabajo sobre lesiones oseas radiologicas y pertenece a Abt (1924). Viene posteriormente un pe-riodo en que se cncuentran relates aislados de lesiones esqneleticas en ninos leucemicos y discusiones limitadas a pequenos grupos de tales pacientes. Se estudia el dolor oseo y su relacion con la leucemia. Se describen los varies tipos de lesiones radiologicas y la interpretacion y errores diagnosticos a que ha dado lugar la presencia de dolor asociado a la leucemia.Vale la pena mencionar el trabajo de Baldridge y Awe (1930), quienes relatan 20 casos de leucemia en pacientes javienes, 13 con dolores oseos, y que los autores atribuyen a artritis aguda. En 1932, Wollstein describe 28 casos de leucemia, de los cuales 10 tienen dolor oseo y 6 revelan hallazgos radiologicos positives.
