Resumo A prevenção ao uso de álcool e outras drogas no Brasil é marcada pela descontinuidade e pelo predomínio de um modelo não baseado em evidência. No ano de 2013, a Coordenação Geral de Saúde Mental, álcool e outras drogas, do Ministério da Saúde, implementou o programa escolar europeu Unplugged#Tamojunto, nos municípios de São Paulo/SP, São Bernardo do Campo/SP e Florianópolis/SC, envolvendo 2.161 educandos da rede pública. O objetivo da pesquisa foi a elaboração de recomendações de adequação do programa ao contexto brasileiro. O estudo qualitativo, baseado na Teoria Fundamentada Construtivista, analisou os documentos institucionais: “Diários Cartográficos”, “Matriz lógica de adaptação cultural” e relatório de pesquisa titulado “Resultados da Avaliação da Implantação Piloto do Programa Unplugged em São Paulo e Santa Catarina” (UFSC e UNIFESP). As formulações teóricas, organizadas a partir da Teoria de Difusão de Inovações, revelaram que é necessário: mudança paradigmática da abordagem de drogas pelos profissionais, maior adesão a metodologias interativas, adequação no tempo da hora-aula, compromisso da gestão escolar, fomento da intersetorialidade entre saúde e educação, consolidação de um monitoramento de processo e um alinhamento ético com os princípios da promoção da saúde.
Universalidade, equidade e integralidade são princípios do Sistema Único de Saúde -SUS que apontam para mudanças nas práticas de Saúde Pública no Brasil. Nesse aspecto, a saúde, como um direito de todo cidadão, deverá implementar ações e serviços não fragmentados, mas conectados pelos nós da rede de cooperação, conscientização e responsabilidades sanitárias.A saúde é resultado de um processo de produção em que se oferece algum valor de uso, entendido como a utilidade que bens e serviços têm para pessoas concretas vivendo em situações específicas. Mesmo no caso da Saúde Pública, em que o fim é o bem comum, e não o lucro, ela conserva o caráter de valor de uso e, nessa compreensão, os trabalhadores e usuários objetivariam traduzir a concretização de determinados valores de uso (Campos, 2000).A Política Nacional de Humanização, como política pública, nasce da necessidade do "reencantamento do concreto". Benevides e Passos (2005) indicam que o conceito de humanização ganhou destaque crescente a partir do ano 2000, em programas no campo da saúde, porém distanciado dos movimentos de mudanças nas práticas de saúde. Nesse contexto, a humanização estava vinculada a áreas da atenção, como a saúde da mulher e da criança, à assistência hospitalar, identificada a certas profissões, como a do psicólogo, exercida por mulheres, bem como focada na perspectiva da "qualidade total nos serviços".A necessidade de recolocação do problema da humanização, distanciado do humano idealizado e aproximado do humano protagonista e autônomo, coincide com a problematização da relação entre o conhecimento e a transformação da realidade, e gerou o movimento da humanização da saúde em direção da construção de uma política pública -de todos e para qualquer um.A humanização do SUS indica mudanças nas práticas de atenção e gestão, em que a aposta é nos sujeitos concretos. "É a partir da transformação dos modos de os sujeitos entrarem em relação, formando coletivos, que as práticas de saúde podem efetivamente ser alteradas" (Benevides, Passos, 2005, p.392).A Política Nacional de Humanização da Atenção e Gestão do SUS (PNH), instituída em 2003 pelo Ministério da Saúde, foi formulada a partir do reconhecimento e sistematização de experiências concretas de estados e municípios que apontam para um "SUS que dá certo", tais como: rede de atenção pública de saúde presente nos territórios; inovações na organização e oferta das
The evidence-based public policies need to address the social vulnerabilities in order to overcome the inequities. This narrative literature review analyzes the challenges of adaptation in the dissemination of the evidence-based alcohol and drugs prevention practices within the context of the Brazilian public policies. The critique of existing models is based on the theories of "Diffusion of Innovations" (Rogers) and "Implementation of the evidence-based practices" (Aarons et al). We concluded that the implantation of an evidence-based preventive action, if culturally not relevant, risks not impacting on the vulnerabilities. The adaptations may promote higher levels of acceptance and adhesion. In spite of that, if those adaptations are performed without the evidence base, they may reduce the effectiveness, as well as de-characterize the innovation, ending up in the reproduction of the inequities it aimed to reduce.
PSE: 15 ANOS PROMOVENDO SAÚDE NA ESCOLA' é resultado da intersetorialidade entre saúde e educação no longínquo compromisso desses setores na implementação do Programa Saúde na Escola (PSE) no Brasil. Trata-se de uma iniciativa do Departamento de Promoção da Saúde da Secretaria de Atenção Primária à Saúde do Ministério da Saúde (Depros/Saps/MS) e apoio da Organização das Nações Unidas para a Educação, a Ciência e a Cultura (Unesco). Tem como proposta a disseminação do conhecimento sobre a implementação, a gestão intersetorial e as ações do PSE na promoção da saúde, prevenção de doenças e agravos, bem como a atenção à saúde dos estudantes da rede pública de Educação Básica. Esta edição especial faz parte de uma série de atividades do MS e do Ministério da Educação em comemoração aos 15 anos do PSE, um dos maiores programas intersetoriais da Atenção Primária à Saúde (APS) e da Educação Básica brasileira, alicerçado na Política Nacional de Atenção Básica (PNAB), na Política Nacional de Promoção da Saúde (PNPS) e na Base Nacional Comum Curricular (BNCC) 1 .Desde a publicação do Decreto Presidencial nº 6.286, de 5 de dezembro de 2007 2 , a trajetória do PSE foi marcada por uma progressiva implantação nos municípios brasileiros, pulverizando a articulação entre esses setores no território nacional e uma implementação condescendente com os contextos institucionais e técnico-conceituais da práxis intersetorial, tanto no dia a dia das equipes de saúde e educação quanto na gestão do programa nos respectivos entes federados 3,4 .Ações intersetoriais têm sido utilizadas como estratégias no Sistema Único de Saúde (SUS) para superar a fragmentação gerada pela setorização administrativa e disciplinar das políticas sociais, condição que propicia enfrentamento dos problemas sociais de maneira efetiva e integral, apoiada em um pensamento sistêmico e interdependente das questões de saúde.A noção de rede social encontra nas relações dos setores a compreensão da complexidade do sistema, tanto no campo pessoal quanto organizacional, ao estabelecer acordos de cooperação. Essa integração dos serviços no âmbito do PSE possibilita promover a saúde e prevenir doenças e agravos no ambiente escolar, assim como incorporar a saúde ao currículo escolar por meio da BNCC ao integrar saberes e experiências, promovendo e almejando, assim, a transetorialidade do programa 1,4,5 .Vale destacar que a intersetorialidade é uma das diretrizes e pilares do PSE, e a discussão e o reconhecimento desse conceito no programa são frequentemente apontados em suas normativas e materiais de apoio à implementação 1,2 .
RESUMO Introdução A Educação Baseada na Comunidade (EBC) no contexto da formação de profissionais da saúde não é um conceito novo, pois sua origem data da década de 1970. Porém, ao longo dos anos, o modo como a EBC é aplicada na educação médica vem sofrendo transformações. No Brasil, desde 2001, a EBC foi incorporada às diretrizes curriculares dos cursos de graduação da área da saúde e, mais recentemente, em 2014, na publicação das novas diretrizes dos cursos de graduação em Medicina. Objetivos Os objetivos do estudo foram levantar os sentidos e significados que os professores atribuem à Educação Baseada em Comunidade nas matrizes curriculares em vigência no ensino médico da Uniceplac no ano de 2016 e identificar possibilidades de aprimoramento do ensino na instituição em consonância com as diretrizes nacionais em vigência. Método Estudo de caráter qualitativo, realizado no Centro Universitário do Planalto Central Apparecido dos Santos (Uniceplac). Os dados foram coletados por meio de entrevista reflexiva com 13 docentes que estão ou estiveram diretamente ligados à EBC nessa instituição no ano de 2016. O método de análise de dados foi a Análise de Conteúdo de Bardin, com a utilização do software Atlas-ti 7 para o suporte na análise. Resultados Os resultados demonstram que a maioria dos docentes afirma conhecer o conceito de EBC, mas sua compreensão diverge da literatura. Além disso, os docentes indicaram falhas na carga horária, nas relações dos professores entre si e com a coordenação, no planejamento e na conquista de cenários adequados das disciplinas ligadas à EBC, além de desafios macropolíticos de aplicabilidade da EBC nos cenários do Sistema Único de Saúde (SUS). Estes resultados estavam relacionados à burocratização das parcerias e a uma falta de corresponsabilidade entre professor, instituição de ensino e Estado. Conclusão Como a EBC vem sendo incorporada gradativamente à educação médica, as instituições também têm uma necessidade crescente de transformação e adequação às diretrizes curriculares vigentes. Sugere-se uma capacitação permanente do corpo docente sobre a compreensão e aplicação da EBC, bem como a criação de um órgão, setor ou funcionário destinado apenas à institucionalização, formalização e manutenção dos cenários, destacando-se cenários da atenção básica no SUS. É também essencial instruir os alunos sobre o tema, para que possam compreender a aplicabilidade da EBC, sua importância na formação médica e sua contribuição para a melhoria da qualidade de vida da população.
