We present two cases of paracoccidioidomycosis, one occurring in an AIDS patient and the other in an HIV infected man. This is the first report of such association. The first patient, which was already followed for HIV infection (group IV-A) presented with high fever and hepatosplenomegaly. Plain X-ray, ultrasound and CT-scan of the abdomen showed solid nodules in the spleen, some of them with calcification. Both the direct smear and the culture of a bone marrow aspiration revealed Paracoccidioides brasiliensis. The patient died of acute disseminated Paracoccidioidomycosis. The second patient, a man anti-HIV seropositive presented with a mass on the right lower abdomen and inguinal region. A biopsy of the mass showed the association of Hodgkin's disease of the mixed cellularity type and paracoccidioidomycosis. With the expanding AIDS epidemic we believe this report emphasizes the need to consider Paracoccidioidomycosis in HIV infected persons in countries where this mycosis is endemic. We also suggest the inclusion of Paracoccidioidomycosis as a potential opportunistic infection in these areas.
Os autores apresentam um caso de insuficiência renal aguda, hemólise aguda e trombocitopenia relacionada ao uso intermitente de Rifampicina para tratamento da Hanseníase. Estas reações adversas são observadas com extrema raridade na literatura mundial. A evolução da paciente foi benigna havendo recuperação total da função renal e regressão completa das alterações hematológicas. Este foi o primeiro registro no Brasil dos efeitos adversos da Rifampicina no tratamento de Hanseníase. São discutidos os principais aspectos patogênicos das alterações apresentadas e fez-se revisão da literatura existente sobre o assunto.
INTRODUÇÃO O primeiro transplante de córnea penetrante foi realizado com sucesso em 1905. Entretanto, cirurgias eram raras devido à baixa disponibilidade de tecidos humanos e dificuldades de técnica cirúrgica. Foi nesse período que surgiu a idéia da criação de um sistema de coleta, processamento e distribuição de tecidos oculares aos médicos que realizavam transplante. Em 15 de ABSTRACT Purpose: To describe the characteristics of human ocular tissue donors from the Cascavel Eye Bank in the State of Paraná (PR) and investigate the impact of hepatitis B serologic test positivity on discarded ocular tissues. Methods: Cross-sectional study of data collected between March 2006 and April 2007 at the Cascavel Eye Bank. Information such as age, gender, cause of death, time between death and enucleation, and serologic tests results was collected. Data were used to characterize the study population and stratified according to hepatitis B (anti-HBc) status. Results: Mean age of donors was 54.2 ± 20.6 years and a greater proportion of men (64,7%) was observed. Mean time between death and enucleation was 3.8 ± 2.4 hours, cardiovascular diseases were the main cause of death (34.5%). Positivity of anti-HBc and HBsAg tests was 47.4% and 1.5%, respectively. Hepatitis B was the main reason for discarding corneas. Donors whose serologic tests were positive for anti-HBc had higher mean age compared to those with negative tests (p<0.001). Only 0.8% of all donations tested positive for hepatitis C and donors with positive HIV test were not seen in our study. Conclusions: Activities of the Cascavel Eye Bank significantly increased the availability of corneas for transplant in the State of Paraná. The main contraindication for availability of ocular tissues was positivity of serologic tests for anti-HBc. Biosafety studies should be performed in order to investigate the potential transmission of hepatitis B virus for donors with positive tests for anti-HBc. The inclusion of these donors would increase the number of corneas available for transplant.
Pesquisamos os anticorpos anti-HBc e anti-HCV em amostras de soros provenientes de 799 candidatos a doadores, que tiveram suas unidades de sangue ou derivados transfundidas a 111 receptores. O anti-HBc e o anti-HCV foram reagentes, em respectivamente 9 e 2,1% dos doadores testados. Observamos que entre os 111 receptores, 44 haviam recebido pelo menos uma unidade anti-HBc positiva e 67 haviam sido transfundidos somente com unidades anti-HBc negativas. Houve um risco 4,5 vezes maior de aquisição de hepatite por vírus C pelos receptores que receberam pelo menos uma unidade anti-HBc positiva Se a pesquisa do anti-HBc fosse realizada na triagem sorológica dos doadores de sangue, cerca de 56% dos casos de HVC nos receptores saiam evitados. A população de receptores que recebeu pelo menos uma unidade anti-HCV reagente, apresentou um risco 29 vezes maior de adquirir esta hepatite, quando comparada aos receptores transfundidos com todas as unidades anti-HCV negativas. A realização do teste para a pesquisa do anti-HCV na triagem dos doadores de sangue, preveniria 79% dos casos de HVC pós-transfusionais. Os candidatos a doadores brasileiros parecem ser acometidos simultânea ou sequencialmente, pelos vírus B e C das hepatites, pois, 44,4% dos doadores anti-HCV positivos, também foram anti-HBc positivos. A realização dos testes para as pesquisas dos anticorpos anti-HBc e anti-HCV, nas triagens hemoterápicas, está indicada para prevenir a transmissão de hepatites pós-transfusionais, em nosso meio.
Seguimos ambulatorialmente, por no mínimo 180 dias, 111 receptores de transfusões, para avaliarmos a ocorrência de hepatites pós-transfusionais e os agentes etiológicos envolvidos com esta doença, na cidade de Campinas, Estado de São Paulo, Brasil. No final diagnosticamos esta hepatite em 18 (16,2%) receptores. Destes, tivemos 16 (89%) casos devido ao vírus da hepatite C, 1 (5,5%) causado pelo vírus da hepatite B e 1 (5,5%) caso restante, sem etiologia determinada, 15 meses após a transfusão. O período de incubação da hepatite por vírus C (HVC) foi de 71 dias, em média; e 23% dos indivíduos com esta hepatite permaneceram com aumento de AST/ALT por mais de 6 meses. Observou-se soroconversão tardia para o anti-HCV em 71,4% dos receptores, que ocorreu, em média, 135 dias após a transfusão. Uma dosagem de ALT e uma pesquisa do anti-HCV, aos 3 e 6 meses, após a transfusão, diagnosticariam, respectivamente, 71 e 93% dos casos que desenvolveram HVC pós-transfusionais.
