BackgroundLower limb lymphedema is a frequent and debilitating complication after cervical cancer treatment.ObjectiveTo evaluate the incidence and prevalence of lymphedema after treatment for cervical cancer and evaluate the factors associated with this outcome.MethodsThe study was performed according to the Preferred Reporting Items for Systematic reviews and Meta-Analysis (PRISMA) guidelines and was registered in the International Prospective Register of Systematic Reviews (PROSPERO). Eligible studies were identified through the databases Medline (via PubMed), LILACS, Scopus, and Web of Science. For the search, descriptors, keywords, and synonyms were used for cervical cancer, lymphedema, and outcomes of interest (incidence, prevalence, frequency, occurrence, morbidity, risk factors, and prognosis). Observational studies were included that were published in English, Portuguese, or Spanish, between January 1, 2013 and December 31, 2019, with frequency data and associated factors for lymphedema after cervical cancer treatment. Studies that evaluated the presence of lymphedema less than 6 months after surgery or radiotherapy and those in which frequency data were not stratified by tumor topography, were excluded.ResultsTwenty-three studies were included. The incidence of lymphedema varied between 0% and 69%. Different designs and methods were employed for lymphedema assessment. The main factors associated with lymphedema included extension of lymphadenectomy, number of lymph nodes removed, removal of circumflex iliac lymph nodes, and adjuvant radiotherapy. Other factors associated with lymphedema included cellulitis, lymphocyst formation, increased age, invasive lymph node staging, higher body mass index, and insufficient physical activity.ConclusionsThe frequency of lymphedema after treatment for cervical cancer is variable. The different designs and methodology to measure this complication make it difficult to draw definitive conclusions about its prevalence. The method used for lymphedema assessment is not standardized.
The results of the translation and cross-cultural adaptation of the Lymph-ICF-LL resulted in a Brazilian Portuguese version, which still requires validation with various samples of the local population.
Introdução: Exercícios de dilação e a relação sexual são orientações preventivas e estenose vaginal. Objetivo: avaliar a adesão das pacientes às orientações fisioterapêuticas na prevenção da estenose vaginal. Métodos: estudo retrospectivo dos prontuários de mulheres com câncer de colo uterino submetidas a braquiterapia entre janeiro e dezembro de 2018, e com seguimento de um ano no ambulatório da fisioterapia. As variáveis contínuas foram descritas por média e desvio padrão e as variáveis categóricas por frequência percentual. Aplicou-se teste QCochran, considerando um nível de significância p≤ 0,05, para avaliar as variações em 6meses e 1ano. Resultados: Foram analisados prontuários de 60 mulheres com idade média de 49,4 anos (± 11,1). Na primeira avaliação após braquiterapia 21 mulheres (35%) realizaram os exercícios de dilatação vaginal, 26 mulheres (43,3%) eram sexualmente ativas e 13 mulheres (21,7%) apresentavam estenose vaginal. Em1ano, 21 mulheres (35%) realizavam os exercícios de dilatação vaginal e 26 mulheres (43,3%) eram sexualmente ativas. Considerando as que realizavam pelo menos uma das orientações, nota-se a adesão de 43 mulheres (73,3%). Não houve alteração significativa para esses parâmetros ao longo de 6meses, nem para 1ano (p=0,368). A adesão não variou significativamente para realização dos exercícios de dilatação neste período de 6meses e 1ano, nem para prática de relação sexual neste período (p=0,097 e p=0,368, respectivamente). Conclusão: Houve uma boa adesão pelas mulheres em seguir algum tipo de orientação preventiva de estenose e manutenção da prática dessas orientações ao longo de 1ano.
Introdução: A progressão da doença e os efeitos colaterais dos tratamentos repercutem na vida das pacientes com câncer ginecológico. Ponderar sobre hábitos sexuais após diagnóstico pode facilitar entender fatores psicológicos e físicos que influenciam na sexualidade. Objetivos: Verificar os hábitos sexuais de mulheres com câncer ginecológico após diagnóstico e os fatores que impactam na sexualidade dessas mulheres. Métodos: Pacientes hospitalizadas >18 anos, com câncer ginecológico, responderam questionário com 16 questões fechadas, 11 sobre perfil clínico e 5 sobre hábitos sexuais pós diagnóstico. Resultados: Participaram do estudo, 83 mulheres, 55,7±13,5 anos. 34,8% com estadiamento >IIb. 77,11% declaram não ter vida sexual ativa, estando 36,7% atribuído à ausência de parceiro, 12,05% por dor, 16,87% por perda da libido e 13,25% por medo ou limitação clínica. 26,51% das mulheres referem impacto da sexualidade na sua qualidade de vida. Conclusão: A situação conjugal influencia na função sexual das mulheres, mas dor e perda de libido são fatores que mais impactam na sexualidade dessas mulheres. Maior exploração dessa temática faz-se necessário para melhor instruções das pacientes, acompanhantes e, sobretudo, dos profissionais de saúde, para elaboração de estratégias com foco na qualidade de vida.
