Objetivos: Detectar os principais achados fundoscópicos em crianças portadoras de hemoglobinopatias falciformes. Métodos: Foram estudados 26 pacientes com hemoglobinopatias falciformes, no Serviço de Oftalmologia do Hospital Universitário Onofre Lopes, Natal, RN, que foram submetidos a protocolo de pesquisa pré-estabelecido. Os resultados foram avaliados estatisticamente pelo teste qui-quadrado. Resultados: A idade média foi de 10,6 anos, com acuidade visual igual ou melhor que 20/25 na maioria, excetuando-se 3 olhos, que apresentavam outras doenças associadas. O tipo mais freqüente foi o SS com 57,7% (15/26) dos casos, seguido pelos SC e SA com 15,4% (4/26) cada, e pelo S-Thal com 11,5% (3/26). A freqüência da retinopatia por células falciformes foi maior após os 10 anos de idade, sendo mais freqüente, em valores relativos, no tipo S-Thal (100% dos casos) e, em valores absolutos, no tipo SS (9 casos). Os dois achados mais comuns foram tortuosidade venosa (12/26) e "black sunburst" (7/26). Conclusões: Observamos que a incidência de retinopatia por células falciformes aumentou após os 10 anos de idade e não evidenciamos achados da doença proliferativa. Portanto, enfatizamos a necessidade do exame oftalmológico precoce nos portadores de anemia falciforme, como forma de prevenir futuras complicações oculares.

Dois terços dessas infecções ocorreram próximas à incisão, enquanto outro terço envolveu órgãos ou espaços acessados durante as operações. Dos 551 pacientes infectados que morreram, 77% das mortes foram provocadas por infecções graves. A infecção cirúrgica portanto, segue considerada hoje o problema mais importante nas salas de operações 1 . Background. Alport syndrome is a progressive and hereditary nephropathy, characterized by hematuria and proteinuria, and extrarenal manifestations as hearing loss and eye abnormalities. The disease can be expressed as autosomal recessive or dominant, caused by variants in COL4A3 and COL4A4 loci, respectively, or X-linked caused by variants in COL4A5 locus.Methods. Two unrelated families with Alport Syndrome from northeast of Brazil were studied and whole exome sequencing were performed. DNA sequences were mapped against the human genome (GRCh38/hg38 build) to identify associated mutations.Results. Variant analysis showed deleterious variants in COL4A3 and COL4A4 loci in chromosome 2. Two variants were detected with alternative alleles in a homozygous state in the probands. One novel premature stop codon at position 481 of COL4A3 protein is present in one family and one frameshift mutation leading to a premature stop codon at position 786 of COL4A4 protein in the other family. Both Alport cases presented their variants surrounded by a broad runs of homozygosity (ROH). Conclusions. É grande o número de pessoas que inalam nicotina diariamente através do hábito de fumar e eventualmente têm que ser submetidos a intervenção cirúrgica no aparelho digestivo. Sendo a nicotina um agente que tem sido implicado no retardo da cicatrização das feridas, foi realizado estudo experimental em ratos com o objetivo de testar o seu efeito na evolução histológica da cicatrização de anastomoses intestinais. Foram utilizados 17 ratos Wistar com média de peso 275±14g, divididos aleatoriamente em grupo I (n=9) e grupo II (n=8), anestesiados com éter sulfúrico e operados com técnica asséptica. A nicotina foi administrada por via subcutânea na dose de 5mg/kg em dias alternados, durante 17 dias nos ratos do grupo I e foi usado placebo no grupo II. Após sete dias de uso da nicotina e do placebo, foi feita secção transversal do jejuno a 5 cm do duodeno e anastomose com polipropileno 6-0. No décimo dia os ratos foram mortos com superdose de éter sulfúrico e foi feita biópsia da anastomose. Após processamento dos cortes histológicos e coloração pela hematoxilina-eosina, os dados foram quantificados por escores. A análise histológica revelou que o grupo I atingiu o escore 135 (média 15±4,41) e o grupo II, 218 (média 27,25±4,89). De acordo com o teste t, a diferença mostrou-se significativa (p<0,05). Os dados permitem concluir que a nicotina, quando administrada por via subcutânea em ratos, contribui para prejudicar a cicatrização de anastomoses intestinais.
