É cada vez mais atual a idéia de que a profilaxia é o maior aliado do cirurgião, fazendo-se necessário um grande esforço para manter a infecção sob controle dentro dos padrões aceitáveis e preconizados pelas instituições nacionais e internacionais que lidam com o problema.De acordo com estatísticas oficiais norte-americanas, de 593.344 operações estudadas e realizadas entre 1986 e 1996, cerca de 15.523 (3%) foram seguidas de infecção. Dois terços dessas infecções ocorreram próximas à incisão, enquanto outro terço envolveu órgãos ou espaços acessados durante as operações. Dos 551 pacientes infectados que morreram, 77% das mortes foram provocadas por infecções graves. This review aimed to discuss the details of metabolic response to surgical trauma for medical students. All of them should known to manage these situations and patients should be treated accordingly. Methods: A review of medical literature was performed in databases Pubmed, Web of Sciences and SciElo, using the key words posttraumatic metabolism, trauma response, hormone response, stress response, trauma, surgery. Stress caused by events such as surgical trauma including endocrine, metabolic and immunological changes were discussed. Results: This review confirmed that stress hormones and cytokines play a role in these reactions, ultimately leading to greater catabolic effects. Some characteristic response that occurs in trauma patients were identified, such as protein and fat consumption body fluids and electrolytes loses, because of hypermetabolism in the early period. The oxygen and energy requirement increases in proportion to the severity of trauma. These responses are important in determining metabolic and nutritional support in the postoperative period. The insulin resistance occurs, and free fatty acids are primary sources of energy after trauma. Surgical stress results in a reduction in protein synthesis and moderate protein degradation. In severe trauma, burns and sepsis an increased protein degradation is observed. Estudos prévios têm demonstrado que o ácido hialurônico, um glicosaminoglicano que ocorre em doses elevadas e por tempo prolongado na matriz extracelular dos tecidos em cicatrização do feto, é o responsável pelo resultado da cicatrização fetal sem marcas residuais e sem contração. O presente estudo investigou a ação tópica do ácido hialurônico em queimaduras de ratos adultos, até a epitelização completa das lesões. Foram usados 20 ratos Wistar com peso médio 225± 15g, nos quais foi provocada queimadura de 5 cm2 na pele do dorso, sob anestesia com éter sulfúrico. No grupo I (n=10) as queimaduras foram tratadas com aplicação tópica diária de 1ml de ácido hialurônico 1% e no grupo II (n=10) com 1ml de solução salina 0,9%, até a completa epitelização. O tempo de cicatrização foi de 29± 1,33 dias no grupo I e 38± 2,58 no grupo II. A média dos escores histológicos foi de 27,0± 2,78 no grupo I e de 18,1± 3,66 no grupo II. As diferenças mostraram-se significantes (p<0,05). A análise do resultado estético revelou maiores deformidades nas cicatrizes do grupo II (controle). Concluiu-se que o ácido hialurônico tópico contribuiu para acelerar o tempo de cicatrização, melhorou a evolução histológica e o resultado estético em queimaduras do dorso de ratos adultos.
OBJETIVO: Avaliar o efeito da nicotina na cicatrização da camada musculoaponeurótica da parede abdominal. MÉTODOS: Estudo experimental em que foram usados 16 ratos da raça Wistar pesando em média 210± 8g, separados aleatoriamente em 2 grupos de 8. Nos animais do grupo A foi implantado disco de nicotina (Nicotinel Ò) na dose de 5mg/Kg de peso/dia no subcutâneo da região dorsal, trocado a cada dois dias, a partir do 5º dia antes da operação em que foi feita laparotomia mediana de 5 cm, até o 10º dia de observação. No grupo B (controle) foram usados discos de celulose com o mesmo diâmetro. Tubo de silicone multiperfurado foi implantado no subcutâneo a 1cm da lesão da parede abdominal. A camada musculoaponeurótica e a pele foram suturadas com fio de nylon 5-0. No 10º dia pós-operatório foi colhido 1ml de líquido seroso do tubo de silicone por punção percutânea para dosagem de pO2 e os animais receberam dose letal de anestésico. Foi ressecado um segmento da camada musculoaponeurótica com 2cm de largura para tensiometria, em seguida processado e corado em HE e tricrômico de Masson para análise quantitativa dos dados histopatológicos em sistema digitalizado. A análise estatística foi feita pelo ANOVA e teste Newman-Keuls, com significância 0,05. RESULTADOS: No grupo A a pO2 do líquido tecidual atingiu o valor 17,75± 3,4 mmHg e no grupo B (controle) a pO2 = 40,75± 6,4 mmHg (p<0,01). A resistência à tensão apresentou o valor de 728,5± 161,75gf no grupo A e 1241,6± 232gf no grupo de controle (p<0,01), coincidindo com os achados da pO2. A densidade média dos elementos histopatológicos estudados foi de 105± 17,1 nos animais do grupo A e 146,2± 8,8 no grupo B (p<0,01). CONCLUSÃO: após avaliação da pO2 tecidual, tensiometria e histopatologia, concluiu-se que a nicotina por via subcutânea exerce efeito deletério sobre a cicatrização de lesões da parede abdominal de ratos.
Purpose: Laparoscopic cholecystectomy (LC) is currently the most widely used surgical procedure for the treatment of gallstones. The aim of the study was to analyze and compare the postoperative results of patients undergoing laparoscopic cholecystectomy or open cholecystectomy (OC) with regard to complications, recovery time, hospital stay and cost. Method: This is a prospective study, which analyzed 1015 patients, 20-65 years old, diagnosed with gallstones undergoing LC or OC. We evaluated postoperative respiratory complications, surgical site infection, deep vein thrombosis, time to oral feeding and ambulation, use of antibiotics, duration of the postoperative period and the cost of surgery. Results: We analyzed 470 (46%) patients undergoing LC and 545 (54%) OC. Most patients were female (68,4%). Patients' comorbidities were hypertension (15.8%), diabetes mellitus (4.0%) and asthma (1.18%). LC resulted in lower prevalence of postoperative complications (2.9%) than OC (5.13%). Postoperative hospitalization for 18-24 hours was found in 71.68% of LC patients and in 13.4% in OC. The financial cost per surgery was US$ 495.00 in LC and US$ 435.00 in OC. Conclusion: Laparoscopic cholecystectomy showed higher benefits for patients with lower prevalence of postoperative complications, feeding earlier, shorter mean hospital stay and equivalent cost compared with open cholecystectomy.
