Recebido em 16/08/2010 Aceito para publicação em 06/09/2010Até o momento foram identificados mais de 100 tipos de papilomavírus humano (HPV), sendo que cerca de 40 deles podem infectar a região anogenital. O grupo dos HPVs anogenitais pode ser dividido entre tipos oncogênicos ou de alto risco (Ex.: 16, 18, 31 e 45), e tipos não oncogênicos ou de baixo risco (Ex.: 6 e 11) (1) . Os tipos oncogênicos estão fortemente relacionados à etiologia do câncer do trato anogenital e suas lesões precursoras (2) . Esses vírus podem ser isolados em aproximadamente 99% dos carcinomas de colo uterino, além de 36 -40% dos carcinomas de vulva, 64 -90% dos carcinomas de vagina, 50% dos carcinomas de pênis e 80 a 93% dos carcinomas anais. O HPV-16 é tipo mais freqüente, estando presente em até 87% dos casos de carcinoma espinocelular (CEC) do canal anal HPV positivo (1,2,3) .Apesar de ser relativamente raro, o núme-ro de doentes de CEC do canal anal vem aumentando nas últimas décadas. Entre os anos de 1973 a 2000, sua incidência teve aumento duas vezes maior em homens (160%) do que em mulheres (78%), segundo estatísticas americanas (2) . A incidência é ainda mais elevada nos grupos considerados de risco, particularmente, entre homens e mulheres HIV-positivo e homens que fazem sexo com homens (HSH). Recebido em 14/01/2009Aceito para publicação em 05/03/2009 INTRODUÇÃO O tratamento das hemorróidas pode ser classificado em três categorias: 1) modificação da dieta e estilo de vida; 2) procedimentos ambulatoriais não cirúrgicos; 3) tratamento cirúrgico 1,2 . As modificações de dieta e estilo de vida são aplicáveis a todos os pacientes, sendo muitas vezes o único tratamento necessário nos pacientes com doença de 1º e 2º grau. Consistem numa adequada ingestão de líquidos e fibras, o que pode ser obtido com a simples modificação de hábitos alimentares ou com ajuda de suplementos de fibra dietética (psyllium, plantago ovata, ágar-ágar, metilcelulose, cálcio policarbofil) disponíveis sob diferentes nomes comerciais. Modificações dos há-bitos evacuatórios e de higiene pessoal, evitando uso do papel higiênico, também são recomendáveis 1,2 . Uma série de procedimentos ambulatoriais não-operatórios está descrita na literatura, podendo-se citar entre eles: ligadura elástica, escleroterapia, crioterapia, fotocoagulação por infravermelho, diatermia e eletrocoagulação. Estas técnicas podem ser utilizadas em doença de 1º grau, alguns casos de 2º grau e casos bem selecionados de 3º grau [1][2][3][4] .

RESUMO:As verrugas genitais causadas pelo papilomavírus humano (HPV) são problemas comuns na prática clínica do coloproctologista. As opções terapêuticas são numerosas, não havendo padrão-ouro para o tratamento dos condilomas. Dentre elas, é possível destacar a crioterapia, que é eficaz, apesar de pouco utilizada. O efeito destrutivo da crioterapia ocorre por lesão celular direta, causada pelo frio, e por anóxia tecidual, consequente às alterações microcirculatórias. Dentre as vantagens do método, cita-se a segurança; a simplicidade de aplicação; o uso dentro e fora do canal anal, inclusive durante a gestação; e o desconforto local leve, dispensando anestesia. Os agentes criogênicos mais utilizados são o nitrogênio líquido e, mais recentemente, o dimetiléter-propano. Uma vantagem adicional é observada no tratamento das lesões neoplásicas e seus precursores. Postula-se que seu congelamento induziria à resposta imune antitumoral, consequente ao mecanismo de lesão tecidual, que expõe grande quantidade de antígenos intracelulares aos macró-fagos e às células dendríticas. Entretanto, a literatura médica ainda carece de estudos abordando especificamente o tema, sobretudo na área da coloproctologia. De qualquer forma, podemos afirmar que a crioterapia é método eficaz, seguro, confortável e prático para o tratamento das verrugas anogenitais induzidas pelo HPV. Segundo estatísticas americanas, estima-se que um milhão de novos casos de verrugas anogenitais causadas pelo papilomavírus humano (hpV) são diagnosticados a cada ano, sendo doença comum na práti-ca clínica do coloproctologista 1 . na literatura médica, não existem grandes estudos multicêntricos comparando as diferentes modalidades terapêuticas disponíveis, sendo que as recomendações dos diferentes guidelines são baseadas na opinião de especialistas e pequenos estudos que compararam algumas opções de tratamento 2 . as opções terapêuticas são numerosas e bem estabelecidas, não havendo padrão-ouro para o tratamento dos condilomas, que pode ser oneroso e com taxas de recidiva elevadas 1,3 . De modo geral, a eficácia do tratamento varia de 60 a 90%, com índices de recidiva entre 25 e 67%, dependendo do tamanho e número das lesões, da imunidade do portador e da terapia aplicada 1,2,4,5 . o tratamento das verrugas anogenitais pode ser classificado, de modo geral, em químico ou ablativo. dentro do grupo dos agentes químicos, tem-se a podofilina 20-25%, podofilotoxina 0,5%, imiquimode 5% e ácido tricloro-acético (ata) 60-90%.
