O estudo teve como objetivo descrever o perfil epidemiológico dos casos notificados de sífilis gestacional no Estado do Amapá no quinquênio de 2014 a 2018, associando com variáveis sociodemográficas e clínicas, e avaliar a distribuição espaço-temporal da doença nos municípios atráves de dados plotados em mapas usando o SIG. Estudo epidemiológico retrospectivo transversal, de natureza quantitativa e abordagem descritiva. As informações sobre as notificações de sífilis em gestantes foram retiradas do Sistema de Informação de Agravos de Notificação (SINAN), o número de nascidos vivos do Sistema de Informação sobre Nascidos Vivos (SINASC) e as malhas geográficas do Instituto Brasileiro de Geografia e Estatística (IBGE). De 2014 a 2018, registraram-se 986 casos de sífilis em gestantes, com o maior número de casos no município de Macapá (646) e a maior incidência em Serra do Navio (26,55). A maioria das notificações ocorreu na faixa etária de 20 a 29 anos (47,7%), na raça/cor parda (74,75%), em gestantes com 5ª a 8ª série incompleta (22,21%), em donas de casa (58,82%), no terceiro trimestre gestacional (50,71%) e a classificação da doença na fase primária (27,28%). Para confirmação diagnóstica, somente 29,51% utilizaram ambos os testes, não treponêmico e treponêmico. O tratamento que prevaleceu foi Penicilina G benzatina 7.200.000 UI (49,39%). Os achados indicaram aumento anual das notificações, assim como crescimento considerável no coeficiente de incidência anual nos municípios do Amapá, revelando a necessidade de interrupção da cadeia de transmissão da sífilis.
Objetivos: Descrever o comportamento da incidência e delimitar o perfil clínico, epidemiológico e laboratorial da sífilis congênita no estado do Amapá, Amazônia brasileira, de 2014 a 2018. Métodos: Foi realizado um estudo descritivo e transversal a partir de dados disponibilizados pela Secretaria de Vigilância em Saúde do Estado do Amapá. Calculou-se a taxa de incidência anual para 1000 nascidos vivos e descreveu-se sua distribuição espacial no estado. Ademais, realizou-se estatística descritiva das variáveis sociodemográficas, clínicas e laboratoriais, e estatística inferencial de associação, por meio do Teste Exato de Fisher, para as variáveis categóricas de relevância. Resultados: Registrou-se 308 casos de sífilis congênita, com comportamento crescente da incidência da doença, e o município de Macapá, capital, representando 71,1% dos casos. No perfil dos recém-nascidos, houve predomínio da raça/cor parda (47,37%), da sífilis congênita recente (99,68%), e da icterícia como principal sintoma (68,5%). No que tange o perfil materno, caracterizou-se por mulheres menores de 20 anos (31,35%), pardas (76,3%), com escolaridade de 5ª a 8ª série completas (25,97%), e donas de casa (82,94%). Observou-se que o pré-natal foi realizado em 72,4% dos casos, porém com diagnóstico anterior ao parto em somente 36,04%, o parceiro não foi tratado em 72,4%, e o tratamento materno foi inadequado em 63,3%. Conclusões: Demonstra comportamento crescente da incidência da doença, com perfil característico de acometimento. É essencial a realização e o registro de testes e exames diagnósticos, assim como a solidificação dos serviços de pré-natal e capacitação continuada dos profissionais da saúde.
Objetivo: Realizar análise epidemiológica acerca do perfil de mortalidade por epilepsia e por estado de mal epiléptico no Brasil. Métodos: Estudo quantitativo e descritivo, elaborado por meio de dados obtidos do Sistema de Informação de Mortalidade (SIM), segundo o sexo e a faixa etária, entre 2010 a 2019. Resultados: Registraram-se 25.393 óbitos no Brasil, sendo 89,8% de mortes por epilepsia (MPE) e 10,2%, por estado de mal epiléptico (EME). Do total, 64,3% foram do sexo masculino e 35,7% do feminino, destaque para a faixa etária de 40 a 49 anos. As regiões Norte e Nordeste registraram o maior percentual de MPE e EME. Em relação às taxas de mortalidade geral, identificou-se aumento de 46,9% (MPE) e 68,4% (EME), com acréscimo em todas as regiões geográficas. A taxa de mortalidade por epilepsia na população seguiu tendência de crescimento sem flutuações estatisticamente significativas. A taxa de mortalidade por EME apresentou crescimento entre a população em geral e a masculina, com indicativo de queda entre a feminina. Conclusão: Destoando do parâmetro mundial, o Brasil apresentou incremento na taxa de mortalidade por epilepsia, o que pode estar relacionado ao aperfeiçoamento no diagnóstico, no registro adequado da causa básica do óbito e à melhoria na cobertura do SIM nos últimos anos.
RESUMO -OBJETIVO. Estudar a apresentação clí INTRODUÇÃOA sobrevida de pacientes com lupus eritematoso sistêmico (LES) vem aumentando no correr dos anos. A melhora pode ser atribuída à vários fatores, tais como diagnóstico de casos mais leves, melhor controle da pressão arterial, introdução de novos antibióticos, uso de corticosteróides e de drogas imunossupressoras [1][2][3][4] . Vários fatores tais como raça negra 1,5-9 , desenvolvimento de glomerulonefrite [8][9][10][11][12][13][14] , baixo nível só-cio-econômico 1,9 têm sido apontados como indicadores de pior prognóstico.Nos países desenvolvidos, a importância de septicemia como causa de óbito vem diminuindo. Estudos recentes têm relatado que a doença cardiovascular está se tornando importante causa de óbito 14,15 . Em países em desenvolvimento é possível que os pacientes com glomerulonefrite lúpica apresentem pior prognóstico devido a associação de infecção, baixos níveis sócio-econômicos e baixa escolaridade, usualmente encontrados nesses países. O objetivo do presente trabalho foi avaliar a apresentação clínica e evolução da glomerulonefrite lúpica em nosso meio. variáveis estudadas, à época da biopsia renal, (idade, sexo, proteinúria, presença de hipertensão arterial e creatinina sérica) a única que se associou com pior prognóstico foi a elevação da creatinina sérica. Remissão da síndrome nefró-tica ocorreu em 65% das vezes. A sobrevida atuarial foi de 96%, 82%, 70% e 70% em 1, 5, 10 e 12 anos. Cinco pacientes desenvolveram insuficiência renal crônica terminal e sete morreram, sendo infecção a principal causa de óbito (57%) CONCLUSÃO. Em pacientes com nefropatia lú-pica, o aumento da creatinina sérica, à época da biópsia, se associou com o desenvolvimento de insuficiência renal crônica ao fim do seguimento e a principal causa de óbito foi processo infeccioso.Unitermos: Lupus eritematoso sistêmico. Insuficiência renal.Glomerulonefrite lúpica: estudo da evolução a longo prazo CASUÍSTICA E MÉTODOSForam analisados retrospectivamente os prontuários de todos os pacientes com o diagnóstico de lupus eritematoso sistêmico (LES) atendidos na disciplina de Nefrologia da Faculdade de Medicina de Botucatu, UNESP, no período de 1970 a 1993. Para ser incluído no trabalho, o paciente deveria ter idade superior a 12 anos no início do seguimento, preencher pelo menos quatro dos critérios da American Rheumatism Association 15 para diagnóstico de LES, ter sido submetido à biópsia renal, apresentar seguimento mínimo de um mês e não necessitar de tratamento dialítico quando visto pela primeira vez.Considerou-se o paciente como portador de insuficiência renal, quando este apresentava creatinina sérica superior a 1,5mg/dl.
