Objective: To evaluate and validate, in our laboratory, the essay of myocardial infarction induced by isoproterenol in rats by means of analysis of hematological, biochemical, oxidative stress markers and histopathological parameters.Methods: Thirty young, male, Wistar rats (145 to 230 g) were randomly allocated in two groups: Sham group, which underwent a virtual myocardial infarction induction, and the Infarction group, which underwent a myocardial infarction induction with isoproterenol. The administrations for the infarction induction were performed during two consecutive days and a 24-hour interval between them. Twenty-four hours after the last administration, rats from both groups were anesthetized and sacrificed for blood sample collection to evaluate complete blood count (CBC) and biochemical parameters (SGOT, SGPT, troponin I, urea and creatinin), obtain myocardial fragments for oxidative stress markers analyses (catalase activity and glutathione concentrations) as well as histopathological examinations.Results: There were no death cases in the Sham group, while the mortality rate in the Infarction group was 25%. Myocardial infarction induction with isoproterenol raised leukocytes and neutrophils counts, SGOT, troponin I and urea concentrations, reduced catalase enzyme activity and glutathione concentrations in the myocardium and let to histopathological concentrations as well. It did not exert alterations in terms of hemoglobin, SGPT and creatinin concentrations.Conclusion: The isoproterenol-induced myocardial infarction essay in rats was adequately reproduced in our laboratory, causing alterations in hematological, biochemical, oxidative stress markers and histopathological parameters. Instituição: HOSPITAL OPHIR LOYOLA APRESENTAÇÃO DO CASO: J.G.S., 53 anos, diagnosticado com câncer gástrico há 3 meses e submetido à gastrectomia total com reconstrução em Y de Roux há 1 mês. No pós-operatório (PO) inicialmente evoluiu sem intercorrências, iniciando dieta via oral no 7ºPO. No 10ºPO iniciou drenagem de secreção amarelada espessa por ponto em região umbilical da ferida operatória (FO), além de febre. Diagnosticado com infecção de FO e iniciado antibioticoterapia empírica e solicitada cultura. Apresentou inicialmente melhora na febre e aspecto da secreção, porém no 14º PO apresentou aumento na drenagem e desta vez com característica e odor entérico. Realizada laparotomia exploradora com achado de pequena quantidade de conteúdo entérico na cavidade, ascite e identificada fístula duodenal bloqueada com anastomoses íntegras. Realizada lavagem da cavidade e colocação de dreno tubolaminar para observar fístula. Paciente apresentou boa evolução e diminuição progressiva no débito do dreno, sendo retirado no 21º dia após a reabordagem. DISCUSSÃO: O câncer gástrico é uma doença de ocorrência mundial, com incidência variável. No Brasil não existem estatísticas corretas de sua real incidência, mas certamente está entre os países com incidência mais elevada. O tratamento cirúrgico é o mais indicado, contribuindo consideravelmente na sobrevida. As complicações da gastrectomia total no câncer gástrico relatadas entre os autores na literatura são muito semelhantes, variando em decorrência da técnica operatória empregada ou das condições dos doentes. As complicações pós-operatórias imediatas gerais mais comuns são as respiratórias, incluindo atelectasias, pneumonias, insuficiência respiratória e tromboembolismo pulmonar. Dentre as complicações locais são citadas as eviscerações e abscessos e infecções de parede totalizando. As fístulas da anastomose esôfago-jejunal variam de 10 a 22%. Outras fístulas previsíveis, como a fístula duodenal, jejunojejunal e pancreáticas, variam de 2% a 5%, são de mais fácil manuseio e têm menor morbidade. COMENTÁRIOS FINAIS: As fístulas da anastomose esôfago-jejunal são as que mais preocupam o cirurgião, pois aumentam significativamente o tempo de permanência hospitalar e morbidade e mortalidade. Quanto às fístulas duodenais, o tratamento conservador muitas vezes é bem sucedido, principalmente nas de baixo débito. Vale ressaltar que em pacientes oncológicos, a própria condição clínica destes faz com que se dê preferência à tratamentos menos agressivos.
