Instituição: HOSPITAL OPHIR LOYOLA Descrição do caso: GSP, 49 anos, submetido a by-pass gástrico em Y-de-Roux há 13 anos, evoluiu há 7 anos com dor epigástrica de moderada intensidade, relacionado a alimentação e a períodos prolongados de jejum. No mesmo período o paciente teve reganho de peso, ultrapassando o peso pré-cirurgico de 124 KG e IMC de 42,9 para 135 KG e IMC de 46,7, além de desenvolver hipertensão arterial sistêmica e síndrome metabólica, o mesmo relatou múltiplas tentativas de perda de peso, evoluindo com artrose de joelho esquerdo, com dor aos esforços. Em 2015 foi diagnosticada fistula gastro gástrico entre o pouch e o estomago excluso, porem o paciente abandonou o acompanhamento. Em 2017 retornou ao ambulatório com nova endoscopia que demonstrava comunicação ampla entre o pouch e o estomago excluso, sendo internado para correção cirúrgica da fistula gastro-gastrica, sendo submetido a correção cirúrgica da fistula. Para remoção adequada do trajeto fistuloso, foi necessário a ressecção da anastomose gastro-jejunal previa e confecção de nova anastomose manual, devido ao grande numero de aderências foi optado por preservar o estomago excluso. Durante o procedimento a anastomose jejuno-jejunal foi reavaliada, evidenciando redução do calibre, realizada confecção de nova anastomose laterolateral entre a alça bilio-digestiva e a alça alimentar, extendendo o ponto de anastomose de 100cm da gastrojejuno anastomose para 150cm da mesma. O paciente evoluiu satisfatoriamente durante a internação, sem complicações e aceitando a dieta restrita, sem dificuldades, no sexto PO foi dada a alta hospitalar do paciente com seguimento mensal com a equipe de cirurgia geral e acompanhamento nutricional e endocrinológico. Discussão: Apesar da fistula gastro gástrica ter tido significativa redução na sua incidência após a normatização da secção completa do pouch e do estomago excluso, atualmente com incidência variando entre 1,5% e 6,0%, esta entidade patológica não esta extinta, podendo ser totalmente assintomática ate mimetizar ulceras e estenoses anastomoticas, porem ao contrario destas complicações mais comuns, a fistula gastro-gastrica , habitualmente necessita de correção cirurgia para o sucesso terapêutico. Considerações Finais: A fistula gastro-gastrica ainda carece de guidelines, por esse motivo a exposição de casos como este torna-se impar, para que o adequado conhecimento desta complicação e carecemos de estudos atuais sobre o tema.
