Las lesiones eléctricas de alto voltaje son poco frecuentes en nuestro medio, pero resultan altamente mutilantes y constituyen por si mismas un criterio de ingreso del paciente en la Unidad de Grandes Quemados. Presentamos el caso clínico de una adolescente de 15 años que sufre quemaduras eléctricas de alto voltaje. Tras la resucitación y estabilización inicial, la paciente requirió 5 intervenciones quirúrgicas para el tratamiento de sus lesiones, incluyendo una amputación escápulo-humeral parcial y un colgajo bipediculado fronto-occipital. Posteriormente, realizamos una craneoplastia con una placa sintética preformada de polieteretercetona. Comentamos algunos aspectos sobre el abordaje de las lesiones eléctricas de alto voltaje, así como sobre la reconstrucción de defectos de cuero cabelludo, y el uso de terapia de presión negativa y de nuevos materiales aloplásticos en este tipo de lesiones.
El pioderma gangrenoso es una enfermedad cutánea de etiología desconocida. Las lesiones comienzan típicamente como pústulas o nódulos de rápida evolución hacia úlceras profundas, mal determinadas y violáceas. El fenómeno de patergia está presente en un 20-50% de los casos, por lo que el desbridamiento de las lesiones conduce a un recrudecimiento del cuadro.Presentamos el caso de una paciente diagnosticada de pioderma gangrenoso en la cual, debido a la lenta evolución con inmunosupresores sistémicos y al importante dolor sufrido durante las curas, se decide la aplicación de terapia de presión negativa sobre las lesiones, obteniendo resultados favorables a corto-medio plazo.
