ResumenLa fototoxicidad es un tipo de reacción cutánea inflamatoria que se produce como consecuencia de la exposición a una sustancia química en combinación con la exposición a radiaciones lumínicas. Ruta graveolens, comúnmente llamada ruda, ha sido descrita en numerosas publicaciones como sustancia causante de reacciones de fototoxicidad.Presentamos el caso de una pareja que sufrió una reacción de fototoxicidad que requirió ingreso en la Unidad de Quemados como consecuencia del contacto con una infusión de ruda y posterior exposición solar.Consideramos que todo profesional que se ocupa del tratamiento de quemaduras debe incluir las fitofotodermatitis dentro del diagnóstico diferencial de las pérdidas cutáneas de espesor parcial.Palabras clave Ruta graveolens, Ruda, Fototoxicidad, Fitofotodermatitis. Nivel de evidencia científica V AbstractPhototoxicity is a type of inflammatory skin reaction that occurs as a result of exposure to a chemical in combination with light radiation exposure. Ruta graveolens, commonly known as rue, has been described in numerous publications as a substance causing phototoxicity reactions.We present the case of a couple who were transferred to our hospital with first and second degree burns associated with intense pain as a result of their contact with an infusion of rue and after sun exposure, which required entry in the Great Burn Unit.We believe that every professional who deals with burn treatment should include phytophotodermatitis in the differential diagnosis of partial-thickness skin loss.
Las lesiones eléctricas de alto voltaje son poco frecuentes en nuestro medio, pero resultan altamente mutilantes y constituyen por si mismas un criterio de ingreso del paciente en la Unidad de Grandes Quemados. Presentamos el caso clínico de una adolescente de 15 años que sufre quemaduras eléctricas de alto voltaje. Tras la resucitación y estabilización inicial, la paciente requirió 5 intervenciones quirúrgicas para el tratamiento de sus lesiones, incluyendo una amputación escápulo-humeral parcial y un colgajo bipediculado fronto-occipital. Posteriormente, realizamos una craneoplastia con una placa sintética preformada de polieteretercetona. Comentamos algunos aspectos sobre el abordaje de las lesiones eléctricas de alto voltaje, así como sobre la reconstrucción de defectos de cuero cabelludo, y el uso de terapia de presión negativa y de nuevos materiales aloplásticos en este tipo de lesiones.
