Inseminasi intrauterin (IIU) merupakan prosedur yang umum digunakan dan menjadi pilihan terapi pertama dalam tatalaksana infertilitas, karena dampak risiko yang rendah, implementasi yang mudah, dan harga yang murah. Metode: Penelitian cross-sectional ini dilakukan pada seluruh pasangan yang melakukan prosedur IIU di Poliklinik Aster Rumah Sakit dr.
Uterine prolapse is a pathological condition that can negatively impact women's quality of life. It is caused by weakening of the pelvic floor muscles. Function of levator ani muscle and other striated muscles is thought to be influenced by Vitamin D levels. Vitamin D exerts its biological effects by adhering to Vitamin D receptors (VDRs) present in striated muscles. We aim to analyze the effect of Vitamin D analog supplementation on levator ani muscle strength in uterine prolapse patients. This was a quasi-experimental study with a pre-post design on 24 postmenopausal women diagnosed with grade III and IV uterine prolapse. Vitamin D levels, VDR, levator ani muscle, and hand grip muscle strength were measured before and after three months of Vitamin D analog supplementation. We found that Vitamin D level, VDR serum level, levator ani muscle strength, and hand grip muscle strength all significantly increase (p < 0.001) following Vitamin D analog supplementation. The correlation coefficient between levator ani muscle and handgrip muscle strength was 0.616 and with p value of 0.001. To conclude, Vitamin D analog supplementation can significantly increase levator ani muscle strength in uterine prolapse patients. We propose that determining Vitamin D level in postmenopausal women and replenishing its deficiency with Vitamin D analog supplementation might aid in preventing POP progression.
Objective: To determine the benefi ts of post-partum vitamin D supplementation on striated muscle strength. Methods: This is a pre-post quasi-experimental study on postpartum vitamin D3 supplement provision in primiparous women with vitamin D3 defi ciency. The effect of vitamin D3 supplement was assessed through the measurement of pelvic floor muscle strength before and after 3 months of vitamin D3 supplementation. Thirty-three primiparous postpartum women with spontaneous vaginal delivery who met inclusion criteria participated in this study. Serum vitamin D3 level, basal tone, and maximum levator ani contraction were measured by perimetry prior to and after vitamin D3 supplementation. This study was conducted in the Obstetric Clinic and Clinical Serology Laboratory of Clinical Pathology Department, Dr. Hasan Sadikin General Hospital/ Faculty of Medicine, Universitas Padjadjaran from March 1 to May 31, 2018.Results: Data analysis using the Wilcoxon test showed an increase in serum vitamin D3 level and a basal tone strength after vitamin D3 supplementation with a P-value of <0.001. However, the maximum strength of levator ani muscle contraction did not increase (P-value <0.829).Conclusions: Vitamin D3 supplementation increases basal tone strength.Keywords: basal tone, maximum levator ani muscle contraction, perimetry, primiparous vitamin D3 defi ciency, vitamin D. Abstrak Tujuan: Untuk mengetahui manfaat pemberian suplementasi vitamin D pascasalin terhadap kekuatan otot lurik.Metode: Tipe penelitian ini adalah penelitian eksperimental quasi dengan rancangan sebelum dan sesudah pemberian vitamin D3 pada ibu primipara pascasalin yang mengalami defisiensi vitamin D3, dengan mengukur kekuatan otot dasar panggul sebelum dan sesudah pemberian 3 bulan vitamin D3. Subjek penelitian adalah primipara pasca persalinan spontan yang memenuhi kriteria inklusi penelitian (n=33). Dilakukan pemeriksaan kadar vitamin D serum dan pengukuran tonus basal dan kontraksi maksimal levator ani dengan menggunakan perimetri sebelum dan sesudah pemberian suplementasi vitamin D3. Penelitian dilakukan di Poliklinik obstetri FKUP/RSHS dan Laboratorium serologi Klinik Patologi Klinik FKUP/RSHS pada bulan 1 Maret- 31 Mei 2018.Hasil: Hasil penelitian ini dengan menggunakan uji wilcoxon didapatkan peningkatan kadar serum vitamin D3 dan peningkatan kekuatan tonus basal setelah pemberian suplementasi vitamin D3 dengan nilai P<0,001. Sedangkan kekuatan kontraksi otot maksimal levator ani tidak mengalami peningkatan dengan nilai P<0,829.Kesimpulan: Suplementasi vitamin D3 meningkatkan kekuatan tonus basalKata kunci: kontraksi maksimal levator ani, perimetri, primipara defi sisensi vitamin D3, tonus basal, vitamin D.
AbstrakNyeri perineum akibat robekan yang terjadi pada saat persalinan dapat dirasakan segera setelah melahirkan atau beberapa bulan kemudian, sehingga menyebabkan dispareunia. Episiotomi sebagai profilaktik untuk melindungi integritas dasar panggul merupakan insisi bedah yang lurus dan rapi akan menggantikan laserasi kasar. Sensasi nyeri akibat penjahitan luka episiotomi dapat dikurangi dengan menggunakan benang yang cepat diabsorbsi. Tujuan penelitian ini untuk membandingkan sensasi nyeri perineum pascapenjahitan luka episiotomi menggunakan benang chromic catgut dengan fast absorbing polyglactin 910 terhadap nyeri perineum 48 jam dan 42 hari pascasalin. Penelitian ini dilakukan di Departemen Obstetri dan Ginekologi FK Universitas Padjadjaran-RS Dr. Hasan Sadikin Bandung periode Desember 2009-Maret 2010. Penilaian nyeri menggunakan visual analog scale (VAS) yang dilakukan 48 jam dan 42 hari pascasalin. Lima puluh dari 100 subjek penelitian dilakukan penjahitan dengan benang chromic catgut dan 50 subjek menggunakan benang fast absorbing polyglactin 910. Analisis uji chi-kuadrat 48 jam pascasalin, didapatkan hasil tidak ada perbedaan bermakna pada penggunaan kedua benang tersebut terhadap nyeri perineum (p=0,645). Analisis 42 hari pascasalin didapatkan perbedaan bermakna (p<0,001), kejadian bebas nyeri perineum lebih baik dengan penjahitan menggunakan benang fast absorbing polyglactin 910 (46 kasus atau 92%) dibandingkan dengan pemakaian benang chromic catgut (29 kasus atau 58%). Simpulan, penjahitan luka perineum menggunakan benang fast absorbing polyglactin 910 memberikan kejadian bebas nyeri perineum yang lebih baik dibandingkan dengan benang chromic catgut pada 42 hari pascasalin. Episiotomy as a prophylactic to protect the integrity of the pelvic floor is a straight and neat surgical incision will replace rough lacerations. Pain sensation due to episiotomy wound suturing can be reduced by using thread that quickly absorbed. The purpose of this study was to compare perineal pain sensation after suturing episiotomy wound using chromic catgut with using fast absorbing polyglactin 910. The perineal pain of both groups were evaluated 48 hours and 42 days post episiotomy using visual analog scale (VAS). This study was conducted at the Obstetrics and Gynecology Department, Faculty of Medicine Padjadjaran University/Dr. Hasan Sadikin Hospital period December 2009-March 2010.
